C<strong>on</strong>cept Permanence• We believe the <str<strong>on</strong>g>Multum</str<strong>on</strong>g> Lexic<strong>on</strong> providesan excellent example of c<strong>on</strong>ceptpermanence in a large-scale healthterminology system• We are asking the committee to rec<strong>on</strong>siderits findings regarding c<strong>on</strong>cept permanence<strong>and</strong> the <str<strong>on</strong>g>Multum</str<strong>on</strong>g> Lexic<strong>on</strong>


A Proposal


Background• For years <str<strong>on</strong>g>Cerner</str<strong>on</strong>g> has publicly advocated the creati<strong>on</strong> of acentral code-assigning authority for drugs <strong>and</strong> drugproducts.• We believe the c<strong>on</strong>tinued use of disparate, generallyproprietary, drug terminology systems is a potential threatto the safety of individual patients <strong>and</strong> to the public healthAND limits the ability of care providers, payers, <strong>and</strong> othersto perform their work efficiently, effectively, <strong>and</strong> safely.• For the same general reas<strong>on</strong>s the Federal government issp<strong>on</strong>soring <strong>and</strong> subsidizing use of SNOMED-CT, webelieve that the Federal government (FDA) should establish<strong>and</strong> maintain drug terminology st<strong>and</strong>ards <strong>and</strong> code sets.


Assumpti<strong>on</strong>s• Creati<strong>on</strong> of a drug terminology st<strong>and</strong>ard is expensive <strong>and</strong>time c<strong>on</strong>suming.• Multiple well-developed drug terminology systems exist.• FDA should profit from the experience of commercialvendors of drug informati<strong>on</strong> systems <strong>and</strong> select <strong>on</strong>e as astarting point.


Our Proposal• FDA should invite proprietary drug informati<strong>on</strong> providers toc<strong>on</strong>tribute drug terminology data in an open-source fashi<strong>on</strong>.• <str<strong>on</strong>g>Cerner</str<strong>on</strong>g> will make the complete VantageRx Databaseavailable for evaluati<strong>on</strong> by FDA (or other appropriateFederal agency).• FDA may determine what tables, fields, <strong>and</strong> data etc… itmay need in order to achieve the goal of creating a nati<strong>on</strong>aldrug terminology st<strong>and</strong>ard <strong>and</strong> code set.• <str<strong>on</strong>g>Cerner</str<strong>on</strong>g> will provide to FDA any tables, fields, <strong>and</strong> dataselected by FDA for use by the FDA <strong>and</strong> US public under a“liberal open-source agreement”*.*unlimited right for FDA to distribute, redistribute, use, <strong>and</strong>modify data


Benefits of Proposal• Creates a fast-track <strong>and</strong> low-cost path to a comprehensivenati<strong>on</strong>al st<strong>and</strong>ard.• Encourages multiple vendors to participate <strong>and</strong> c<strong>on</strong>tributeto a st<strong>and</strong>ard code set via an open, transparent process.• Facilitates interoperability (all drug vendors will have accessto the selected data).
