Since the MentalHealth Act allowing for commitment in broader spectrum of diagnoses was passed in 1991, the number ofinvoluntary commitments of minor patients has doubled as proportion of all admissions, and increasedalmost fourfold in absolute numbers. This study set out to evaluate how psychiatrists working with childrenand adolescents understand the concept of "severe mental disorder". The study was requested by theMinistry of Health and Welfare that sought to produce guidelines for interpreting the Mental Health Act inthis sense. 44 psychiatrists in different positions within C&A psychiatric services were interviewed with thehelp of a semi-structured interview regarding their views of problems in current mental health legislation,reasons for regional variation in commitment figures, difficulties in interpreting the concept of severemental disorder, and what they think should be considered as severe enough to allow commitment of minorpatients in mood disorders, anxiety disorders, eating disorders, conduct disorders, substance use disorders,and developmental disorders. The C&A psychiatrists strongly emphasized the need to evaluate a minor'ssituation beyond descriptive diagnosis. In the child and adolescent psychiatrists' opinion, severity of aminor's disorder is related to progression / regression of childhood / adolescent development presented infunctioning in family and peer relations and ability to schoolwork / play.
