The first perforated tag on to provides apersonal property receipt for valuablesbelonging to the victim that may becontaminated.


Patient TrackingThe second perforated tag provides for patienttracking. This part of the tag shall be removed atthe ambulance loading area to record patentdestination. Patient’s name is recorded on backof tag. With the bar code capability informationcan be recorded electronically as well as by hand.


The triage tag is constructed of synthetic paper,100% resistant to all commonly useddecontamination solutions.• SLUDGEM-alerts first respondersto the possibility ofnuclear, biological orchemical agents usingthis mnemonic.


•The tag provides spaceto record the use of autoinjectors•perforated coloredcontaminated stripprevents the tag frombeing used until patientcontamination is ruledout. If decon is donebarcode is bagged withvictims clothing.


Decon: The tag provides an area to recordpatent decon information, gross decon,secondary decon as well as the solutionsused. This information should be recordedprior to the victim leaving the decon area.Also the type of agent can be recorded atthis time whether radiological, biological orchemical.


On the back of the card is the mnemonicRPM used to initiate <strong>START</strong> triage.


Split immediate, delayed orminor tear off tags. These go tothe <strong>Triage</strong> Unit Leader and areused by the MedicalCommunications Coordinatorand IC to obtain an accuratecount of the injured.
