RISK ASSESSEMNT RECORDESTABLISHMENT/WORKPLACE: The Kxxxxx Txxxxx School,BaldockWORK ACTIVITY: Year 9 Geography Field Trip, Peak District – Jxxx McxxxDESCRIPTION OF ACTIVITY:Jxxxx is a diabetic and suffers from asthma.The trip to the Peak District includes a visit to theBlue John Cavern, a steep underground mine. Thearea to be visited is very hilly <strong>with</strong> steep inclines,and rough terrain.The trip is being carried out in one day whichincludes long coach journeys and all day at thePeak District.FREQUENCY: DURATION:1 day Approx.13 hoursSIGNIFICANT HAZARDS:Insufficient/irregular eatingMissed insulin intakeLong journeyIrregular hoursStrenuous exerciseLong periods outdoors in possible inclementweatherEffects of diabetesSteep access to mineRemote locationPEOPLE AT RISK:JxxxxADVERSE EFFECTS:FatigueStress/AnxietyFaintingLow blood sugar levelsComaPossible physical injuriesHypothermiaEXISTING CONTROL MEASURES:Existing generic RA’s for trip activities in accordance <strong>with</strong> Offsite Visits Policy.GP/Specialist advice.School’s knowledge and experience of his condition.Supervision.RISK FACTOR = 8ACTION AND TIMESCALE(S):Arrange for additional support to be provided on trip to assist JXXXsupport to assist Jxxx if he needs to rest or cannot keep up <strong>with</strong> the groupAlternative arrangements to be organised (eg visitors centre etc) if Blue John Cavern or Winnats Passassessed as inappropriate for Jxxx once at the locationMother to agree that a dynamic assessment at the location may limit or alter Jxxx’s participation inevents and the Group Leader has final decision.Jxxx to be monitored to ensure he takes his time and takes regular breaksJxxx to be monitored to ensure he takes his medication as necessaryJxxx to ensure he carries appropriate food at all times and eats regularlyJxxx to be monitored to ensure he takes blood count at correct intervals.Head Teacher/Group Leader to consider procedures should Jxx collapse – i.e call ambulance, medicalassistance.Jxxx’s mother to inform the school of any GP/Specialist advice regarding emergencies.Note:Undertake a further Risk Assessment following the introduction of additional controlmeasures.ASSESSED BY - SIGNATURE – Date-REVIEWED BY – J.Hambrook, Health, Safety and Risk Manager CSF 12/05/05-47
