The worker may waive the lienrequirement at recertification only after the worker verifies that the lien<strong>for</strong>m obtained previously during the application/reapplication processwas signed and witnessed by either a Deputy County Clerk or NotaryPublic.Exception tothe <strong>Lien</strong><strong>Requirement</strong><strong>for</strong> <strong>Mail</strong>-inRecertificationThe requirement to complete and sign the lien <strong>for</strong>m is not required aspart of the <strong>CMS</strong> mail-in recertification process.Note: The beneficiary will be required to sign a new lien <strong>for</strong>m andhave it witnessed at the next face-to-face recertification orreapplication.AutomationImpactNone.Forms ImpactNone.
