The worker may waive the lienrequirement at recertification only after the worker verifies that the lienform obtained previously during the application/reapplication processwas signed and witnessed by either a Deputy County Clerk or NotaryPublic.Exception tothe LienRequirementfor Mail-inRecertificationThe requirement to complete and sign the lien form is not required aspart of the CMS mail-in recertification process.Note: The beneficiary will be required to sign a new lien form andhave it witnessed at the next face-to-face recertification orreapplication.AutomationImpactNone.Forms ImpactNone.
