Upon completion <strong>of</strong> the OverallEvaluation <strong>and</strong> credit claiming information, you will beable to immediately access, save <strong>and</strong>/or print yourcertificate. Physicians will receive a certificate <strong>of</strong> credit <strong>and</strong>other healthcare pr<strong>of</strong>essionals will receive a certificate <strong>of</strong>attendance.Locations to access the evaluations, claim credit, <strong>and</strong>obtain certificates:• Kiosks near the Auditorium (2nd Floor <strong>of</strong> the HynesConvention Center)• Other available internet sources onsite• Office or home computers (recommended)Online access: http://www.pswebsurvey.com/ACAAIYou will be asked to enter your Last Name <strong>and</strong> ID Numberin order to complete the evaluations. The Evaluation site will close on December 15,<strong>2011</strong>. Up until then, you will be able to complete theevaluations, claim credit <strong>and</strong> obtain your certificates fromyour home or <strong>of</strong>fice computers.Please note that nurses may receive a certificate <strong>of</strong>attendance via the online system. However, if you requirea CBRN certificate for the Allied Health <strong>and</strong> AdvancedPractice sessions, please complete the paper forms thatwill be available at your session.
