Every year supporters ask, What’s new? Friends and colleagues ask, How is the National Alopecia Areata Foundation so successful? How
does NAAF survive primarily on individual donations? Others wonder, How does NAAF get so many people to attend its annual conference?
And why do the National Alopecia Areata Foundation people bond so well?
This report tries to answer all of these questions, explaining why everyone is so proud of this organization. The NAAF family grows every
day, and people in this organization make lifelong friends. Our annual conference is like a family reunion, welcoming new lost cousins
every year.
NAAF offers help for the newly diagnosed as well as those who have suffered with this disease for years. It is THE source for the latest
and most accurate information. Adults, children, family members, friends and parents know they can rely on our superb support network.
Parents rave about the many materials and resources we have to help them and their child through the growing years.
NAAF is also working hard to find better treatments and a cure for this disease. NAAF has either funded the vast majority of the breakthrough
research related to alopecia areata and the hair cycle, or it has been the catalyst to gain government funding for this research.
Without NAAF there would be little if any substantive research on this disease.
NAAF is your advocate in every way to educate the public, our legislators and our government officials. We generate millions of media
impressions every year, and in 2008, we helped to introduce the first ever federal legislation for insurance coverage related to alopecia
areata. We also work with government officials in Washington to advance alopecia areata research.
NAAF was created by people with alopecia areata and remains an organization for people with alopecia areata, so we FIGHT for our
rights even when nobody else will.

SuPPORT: Serving and Bonding in 2008
• NAAF held its 23rd International Patient Conference in Louisville,
Kentucky, in the summer of 2008, drawing 600 participants from
37 states and 5 foreign countries. The conference is the key to all
of NAAF’s support efforts, empowering those who attend to raise
awareness in their own communities.
• 57 Conference Scholarships were awarded to people in need, enabling
them to attend the conference. A total of 953 scholarships
have been awarded since the inception of our conference scholarship
program.
• Our Children’s Conference Camp, which has been held in tandem
with the patient conference since 1988, drew almost 200 children
and teens.
• The Ascot Fund provided financial grants to men, women, and children
so they could purchase hair prostheses. These grants truly
make a difference in the lives of people. We receive gracious thank
you letters every day. A total of 271 people have been aided by
grants from this fund since it was established in 2004.
• Alopecia areata kids rock! Our children’s materials and programs include
a nine-minute DVD/video that kids with alopecia areata can
share with family, friends, and schoolmates; a school pack for use
by parents and teachers; several children’s supports groups; our
Children’s Conference Camp; our PenPal/CyberPals program; secure
and private message boards for children and teens; a special KidNet
section of the NAAF Newsletter; and brochures for parents and kids.
• The Alopecia Areata Marketplace helps people with alopecia areata
access hard-to-find and much-needed products to ease their daily
life. It now features 43 different merchants offering a wide range of
products.
• Our International Support Network includes 147 loyal volunteers in
132 cities worldwide, organizing local support groups and providing
telephone support.
• Our website continues to provide information and connections for
new and returning visitors. It is increasingly interactive to provide
you with real-time person-to-person interactions and all of the
latest information!
• Our PenPal/CyperPal program now has over 1,300 participants, including
adults as well as children.
• NAAF responds to thousands of emails, phone calls, and fax requests
for information.
• NAAF gets by with a little help from our friends, like volunteer Matt
Kelley, who not only has been instrumental in organizing support for
our alopecia areata legislation, but also stops by the office for a few
hours each week to help “pick up the slack!”

PuBLIC AWARENESS
AND EDuCATION:
Reaching Out and Fighting In 2008
• On May 1, 2008, the first ever piece of federal legislation for alopecia
areata patients, the Alopecia Areata Medicaid Improvement
and Parity Act, was introduced in the House of Representatives
as HR 5936. If enacted, this bill would require Medicaid to provide
coverage for the cost of hair prostheses when prescribed due
to an alopecia areata diagnoses. NAAF advocates came together
from across this country in support of this bill and encouraged
their legislators to support it as well. By the end of the congressional
session, twenty Representatives had co-sponsored this bill—
a strong display of support that enabled the bill to be immediately
reintroduced, as HR 1142, in 2009. Support continues to build,
and it is hoped that this bill will be passed soon.
• NAAF has a great celebrity spokesperson, National Basketball Association
(NBA) player Charlie Villanueva, who played for the Milwaukee
Bucks in 2008. Charlie has been there to help others with alopecia
areata, making himself available for a multitude of Meet and Greet
events with children, their families, and their friends throughout the
basketball season. Charlie says he has helped himself in the process—and
he can’t wait for his new contract to take effect, enabling
him to help NAAF financially as well as with his time.
• NAAF’s publicity efforts generated over 160 print, TV, and web media
publications during 2008. A highlight of our publicity campaign
occurred when an article that was written in Palo Alto, California,
about the Alopecia Areata Medicaid legislation was picked up nationally,
resulting in over 50 reprints nationwide.
• NAAF hosted informational exhibits to educate medical professionals
about alopecia areata at key medical and scientific conferences,
including the American Academy of Dermatology, the Dermatology
Nurses’ Association, the Society for Investigative Dermatology, and
the American Academy of Pediatrics.
• Our quarterly NAAF Newsletter continues to win awards and is recognized
as a model for patient newsletters.
• Our NAAF brochures continue to help those with alopecia areata
and educate the public.

PuBLIC AWARENESS AND EDuCATION
• The American Academy of Dermatology awarded NAAF three Gold
Triangle Awards for our awareness efforts in furthering the understanding
of skin diseases among the general public. We won
awards for:
1. Alopecia Areata Awareness Day on the Hill (when 600 people
visited their legislators on Capitol Hill as a prelude to our 22 nd
annual Conference)
2. Our 25 th Anniversary Gala Concert and Reception with pianists

3. Our Spanish documentary video, featuring NAAF spokesperson
Charlie Villanueva, to expand our educational borders
• The Tortoise & Hair run/walk is growing into NAAF’s premier
nationwide awareness and fundraising event.
• NAAF volunteers hosted fundraising/awareness events around the
world, generating almost $130,000 for NAAF. All of these events
are listed by state (or province) in this annual report. We offer
our thanks to all the individuals and families who hosted—or participated
in—these fantastic events!
• NAAF representatives and other members of the Coalition of Skin
Diseases, the American Academy of Dermatology, and the Society
for Investigative Dermatology met with nearly 90 members of
Congress and their staffs to ask for increased investment in funding
for medical research.
• NAAF and the National Coalition of Autoimmune Patient Groups
sent letters to the World Health Organization, providing input to
the strategic plan for Public Health, Innovation and Intellectual
Property.
• NAAF worked with the Genetic Alliance to pass the Genetic Information
Nondiscrimination Act (GINA). President George Bush
signed GINA into law on May 21, 2008. It provides protections
against discrimination based on an individual’s genetic information
in health insurance coverage and employment settings, and it is
the first and only such federal legislation of the new millennium.

RESEARCH:
Exploring Every Avenue and Planning for Future Research
NAAF RESEARCH SUMMIT SETS PRIORITIES TO ADVANCE
ALOPECIA AREATA RESEARCH
NAAF conducted a high-powered, fast-paced, collaborative,
and interactive summit on July 11 in Bethesda, Maryland, to
accelerate knowledge of and movement to therapies for alopecia
areata. NAAF invited some of the most respected geneticists,
immunologists, hair researchers, and mouse model experts
to interact with our alopecia areata researchers as well
as scientists from the National Institutes of Health. We invited
representatives from other voluntary health agencies, like the
Leukemia and Lymphoma Society, to talk about how they are
accelerating their path to therapies. There were also representatives
from companies that are repurposing and repositioning
drugs to see if that is an avenue NAAF wants to pursue.
This summit led us to set the following priorities for future NAAF
research efforts:
Priority One: The Research Summit Planning Committee
overwhelming agreed that use of the National Alopecia Areata
Registry (NAAR) will provide the greatest advancement of a
broad research program, as well as development of treatments
for all types of alopecia areata.
Priority Two: Pursue Immunological Studies to determine if
MHC Class I genes (the genes in a specific subgroup of the Major
Histocompatibility Complex) can be down-regulated as a possible
treatment for alopecia areata. How can we block the immune
cells from attacking the hair follicle?
Priority Three: use the C3H/HeJ Animal Model and other
models to screen FDA-approved drugs.
Priority Four: Explore other avenues for new therapies.
1. Try to find drugs in the market or in clinical research that may
be viable therapies.
2 Explore the possibility of repurposing drugs and tap into current
knowledge. (This could mean partnering with a pharmaceutical
or a repurposing drug company to get the drug
farther down the pipeline.)
THE NATIONAL ALOPECIA AREATA REGISTRY HAS BEEN
SUPPORTED BY NAAF FROM 2000 THROUGH 2008
Alopecia areata tends to run in families, and NAAF continued to
be involved in the national registry of these families. This registry,
which is currently supported with funding from the National
Institutes of Health (NIH), is being used in part to determine
which genes contribute to a susceptibility to alopecia areata.
Identification of these genes will provide a starting point for further
investigation and perhaps new therapies.
By the end of 2008, over 6,000 people had completed the
initial registration questionnaire. Over 2,000 of these people had
also completed the second phase of registration, providing blood
samples and undergoing a medical exam. The more people we
have in the registry, the more we learn about alopecia areata
and the more clues we gain to find a possible cure or better
treatments.

Analysis of Candidate Susceptability Genes
Related to Alopecia Areata
These investigators are studying a particular gene (named
the AIRE gene) that has been associated with alopecia areata
and seems to be involved in autoimmunity in general. So far, this
project has not shown a direct link between the AIRE gene and
human alopecia areata, but this study is not yet complete. When
it is complete, we hope it will put this matter to rest, one way
or the other.

Dr. Christiano is using samples from
the National Alopecia Areata Registry to
study the genetics of alopecia areata. Bit
by bit, Dr. Christiano is narrowing down
the list of possible genes that may be associated
with alopecia areata. It takes
both time and money to sort through the
“genetic haystack” searching for the “alopecia
areata needle,” but once we have
found the “needle”—or “needles” (there
are probably multiple genes involved in
alopecia areata)—we will understand more
about what causes this disease and also
have better targets for therapy.

Gene Expression to Identify Alopecia
Areata–Prone People in Correlation with
the Mouse Model of the Disease
In this study, blood from patients with
alopecia areata is being studied with a new
technology to evaluate the activity of a
large variety of genes. Similar studies had
already been done on mice, and it was time
to try it on humans. The data is yet not
available, but when it is, it should at the
very least show the similarities between
the mouse model and human disease. It
might also identify genes that prove to be
important as a cause of the disease or targets
for therapy.

Molecular Analysis of Hair Follicle Structural
Changes in Alopecia Areata Mice
This study attempts to find genes that
might be involved in causing alopecia areata.
The researchers are using mice that develop
a disease similar to that of alopecia
areata in humans, and they are studying a
group of genes that in the past have been
found to be overactive or underactive in
patients with alopecia areata. Performing
this work is tedious and time consuming,
and the investigators have not yet reported
any important data. However, finding the
genes that are involved in causing alopecia
areata in mice might lead us to better understand
the human disease. For example,
a gene that is over- or under-expressed
might be a target for a drug that increases
or decreases gene expression.

The Role of Calcineurin Signaling
This study is looking at a particular
mouse mutation (the calcineurin mutant

This mouse represents another possible
new animal model for alopecia areata.
However, the studies so far do not show
any direct association of this mouse’s hair

for human disease.

Role of Dietary Vitamin A in Alopecia
Areata Disease Progression in the
C3H/HeJ Mouse Model
Studies of gene expression have shown
that certain genes involved in vitamin A
metabolism are altered during the hair
cycle. Since alopecia areata is a hair cycle
disorder, the investigators decided to evaluate
the affect of vitamin A on the current
alopecia areata mouse model. The investigators
are looking at both gene expression
and the effect of altering vitamin A in the
diets of affected mice. The results are not
yet final, and the study is ongoing, but if
vitamin A is actively participating in alopecia
areata, it would offer an obvious target
for therapy.

This study focuses on developing a new technology for screening

results may prove to be interesting, but no data have been submitted
yet to confirm its utility. Studies are ongoing.

Unraveling the Genetics of Alopecia Areata
This study is using a technology for studying the expression of large
numbers of genes in the current mouse model of alopecia areata. The
genes are being studied at various points during the progression of disease
in the mice. If the important genes can be identified, it might
suggest logical drugs to be used at different stages of the disease in
humans.

This study looks at the immune-related proteins on the pigmentproducing
cells in hair follicles. The investigators are first studying normal
follicles, but will be comparing those findings with alopecia areata
hair follicles. This should offer some information about whether these
pigment-producing cells are related to the cause of alopecia areata.

Exploration of Clinically Relevant Strategies
for the Restoration of Human Hair
Follicle Immune Privilege
These investigators are studying the
immune system as it relates to hair follicles.
Normal hair follicles do not express certain
proteins that are required for an attack by
white blood cells. Therefore, normal hair
follicles seem to be relatively safe from attack
by white blood cells. But we know that
in alopecia areata the hair follicle is indeed
attacked. These investigators are studying
why normal hair follicles are protected from
immune attack, while alopecia areata follicles
are not. The technology for doing this
is complex, since they cannot experiment
on live patients! This area of study is still in
its infancy, but information is slowly emerging.
At the very least, we will understand
human hair biology better. At the very best,
we will find some good targets for therapy
in alopecia areata. Several more years of
work will be required for this.

Development of Co-Culture Models to Investigate
Hair Follicle Lymphocyte
These researchers have grown human
hair follicles isolated from redundant human
skin and have modeled both the collapse of
immune privilege and its restoration. They
have also investigated how the cells of the
immune system respond to cultured hair
follicles following both the collapse and restoration
of immune privilege. This project is
ongoing and results are not yet complete.

Triamcinolone Acetonide in Patients with

This study uses a well-established
therapy for alopecia areata, but focuses
on aspects that may indicate its short- and
long-term safety. Since most patients with
severe alopecia areata are subjected to intralesional
triamcinolone acetonide, which
is a corticosteroid, at some point, the results
are of practical importance. However,
only a few patients have been recruited for
this study so far.

Alefacept Study in Alopecia Areata
This study was performed as a
multicenter, double-blind, randomized,
placebo-controlled clinical trial at four
academic institutions. Forty-five subjects
were enrolled. unfortunately, it was shown
that alefacept is no more likely to improve
alopecia areata than a placebo.

Support Grant for the National Alopecia
Areata Registry
This registry, which is currently supported
in large part with a grant from the
NIH, is not only gathering actual patient
descriptions of their disease, but also samples
of their blood, which can be used for

of this registry is one of the most important
projects supported by NAAF because it can
provide valuable material to many different
investigators who are studying various
aspects of alopecia areata.
