DEAR FRIENDS,<br />

Every year supporters ask, What’s new? Friends and colleagues ask, How is the <strong>National</strong> <strong>Alopecia</strong> <strong>Areata</strong> <strong>Foundation</strong> so successful? How<br />

does NAAF survive primarily on individual donations? Others wonder, How does NAAF get so many people <strong>to</strong> attend its annual conference?<br />

And why do the <strong>National</strong> <strong>Alopecia</strong> <strong>Areata</strong> <strong>Foundation</strong> people bond so well?<br />

This report tries <strong>to</strong> answer all of these questions, explaining why everyone is so proud of this organization. <strong>The</strong> NAAF family grows every<br />

day, and people in this organization make lifelong friends. Our annual conference is like a family reunion, welcoming new lost cousins<br />

every year.<br />

NAAF offers help for the newly diagnosed as well as those who have suffered with this disease for years. It is THE source for the latest<br />

and most accurate information. Adults, children, family members, friends and parents know they can rely on our superb support network.<br />

Parents rave about the many materials and resources we have <strong>to</strong> help them and their child through the growing years.<br />

NAAF is also working hard <strong>to</strong> find better treatments and a cure for this disease. NAAF has either funded the vast majority of the breakthrough<br />

research related <strong>to</strong> alopecia areata and the hair cycle, or it has been the catalyst <strong>to</strong> gain government funding for this research.<br />

Without NAAF there would be little if any substantive research on this disease.<br />

NAAF is your advocate in every way <strong>to</strong> educate the public, our legisla<strong>to</strong>rs and our government officials. We generate millions of media<br />

impressions every year, and in 2008, we helped <strong>to</strong> introduce the first ever federal legislation for insurance coverage related <strong>to</strong> alopecia<br />

areata. We also work with government officials in Washing<strong>to</strong>n <strong>to</strong> advance alopecia areata research.<br />

NAAF was created by people with alopecia areata and remains an organization for people with alopecia areata, so we FIGHT for our<br />

rights even when nobody else will.<br />

SuPPORT: Serving and Bonding in 2008<br />

• NAAF held its 23rd International Patient Conference in Louisville,<br />

Kentucky, in the summer of 2008, drawing 600 participants from<br />

37 states and 5 foreign countries. <strong>The</strong> conference is the key <strong>to</strong> all<br />

of NAAF’s support efforts, empowering those who attend <strong>to</strong> raise<br />

awareness in their own communities.<br />

• 57 Conference Scholarships were awarded <strong>to</strong> people in need, enabling<br />

them <strong>to</strong> attend the conference. A <strong>to</strong>tal of 953 scholarships<br />

have been awarded since the inception of our conference scholarship<br />

program.<br />

• Our Children’s Conference Camp, which has been held in tandem<br />

with the patient conference since 1988, drew almost 200 children<br />

and teens.<br />

• <strong>The</strong> Ascot Fund provided financial grants <strong>to</strong> men, women, and children<br />

so they could purchase hair prostheses. <strong>The</strong>se grants truly<br />

make a difference in the lives of people. We receive gracious thank<br />

you letters every day. A <strong>to</strong>tal of 271 people have been aided by<br />

grants from this fund since it was established in 2004.<br />

• <strong>Alopecia</strong> areata kids rock! Our children’s materials and programs include<br />

a nine-minute DVD/video that kids with alopecia areata can<br />

share with family, friends, and schoolmates; a school pack for use<br />

by parents and teachers; several children’s supports groups; our<br />

Children’s Conference Camp; our PenPal/CyberPals program; secure<br />

and private message boards for children and teens; a special KidNet<br />

section of the NAAF Newsletter; and brochures for parents and kids.<br />

• <strong>The</strong> <strong>Alopecia</strong> <strong>Areata</strong> Marketplace helps people with alopecia areata<br />

access hard-<strong>to</strong>-find and much-needed products <strong>to</strong> ease their daily<br />

life. It now features 43 different merchants offering a wide range of<br />

products.<br />

• Our International Support Network includes 147 loyal volunteers in<br />

132 cities worldwide, organizing local support groups and providing<br />

telephone support.<br />

• Our website continues <strong>to</strong> provide information and connections for<br />

new and returning visi<strong>to</strong>rs. It is increasingly interactive <strong>to</strong> provide<br />

you with real-time person-<strong>to</strong>-person interactions and all of the<br />

latest information!<br />

• Our PenPal/CyperPal program now has over 1,300 participants, including<br />

adults as well as children.<br />

• NAAF responds <strong>to</strong> thousands of emails, phone calls, and fax requests<br />

for information.<br />

• NAAF gets by with a little help from our friends, like volunteer Matt<br />

Kelley, who not only has been instrumental in organizing support for<br />

our alopecia areata legislation, but also s<strong>to</strong>ps by the office for a few<br />

hours each week <strong>to</strong> help “pick up the slack!”<br />

AND EDuCATION:<br />

Reaching Out and Fighting In 2008<br />

• On May 1, 2008, the first ever piece of federal legislation for alopecia<br />

areata patients, the <strong>Alopecia</strong> <strong>Areata</strong> Medicaid Improvement<br />

and Parity Act, was introduced in the House of Representatives<br />

as HR 5936. If enacted, this bill would require Medicaid <strong>to</strong> provide<br />

coverage for the cost of hair prostheses when prescribed due<br />

<strong>to</strong> an alopecia areata diagnoses. NAAF advocates came <strong>to</strong>gether<br />

from across this country in support of this bill and encouraged<br />

their legisla<strong>to</strong>rs <strong>to</strong> support it as well. By the end of the congressional<br />

session, twenty Representatives had co-sponsored this bill—<br />

a strong display of support that enabled the bill <strong>to</strong> be immediately<br />

reintroduced, as HR 1142, in 2009. Support continues <strong>to</strong> build,<br />

and it is hoped that this bill will be passed soon.<br />

• NAAF has a great celebrity spokesperson, <strong>National</strong> Basketball Association<br />

(NBA) player Charlie Villanueva, who played for the Milwaukee<br />

Bucks in 2008. Charlie has been there <strong>to</strong> help others with alopecia<br />

areata, making himself available for a multitude of Meet and Greet<br />

events with children, their families, and their friends throughout the<br />

basketball season. Charlie says he has helped himself in the process—and<br />

he can’t wait for his new contract <strong>to</strong> take effect, enabling<br />

him <strong>to</strong> help NAAF financially as well as with his time.<br />

• NAAF’s publicity efforts generated over 160 print, TV, and web media<br />

publications during 2008. A highlight of our publicity campaign<br />

occurred when an article that was written in Palo Al<strong>to</strong>, California,<br />

about the <strong>Alopecia</strong> <strong>Areata</strong> Medicaid legislation was picked up nationally,<br />

resulting in over 50 reprints nationwide.<br />

• NAAF hosted informational exhibits <strong>to</strong> educate medical professionals<br />

about alopecia areata at key medical and scientific conferences,<br />

including the American Academy of Derma<strong>to</strong>logy, the Derma<strong>to</strong>logy<br />

Nurses’ Association, the Society for Investigative Derma<strong>to</strong>logy, and<br />

the American Academy of Pediatrics.<br />

• Our quarterly NAAF Newsletter continues <strong>to</strong> win awards and is recognized<br />

as a model for patient newsletters.<br />

• Our NAAF brochures continue <strong>to</strong> help those with alopecia areata<br />

and educate the public.<br />

• <strong>The</strong> American Academy of Derma<strong>to</strong>logy awarded NAAF three Gold<br />

Triangle Awards for our awareness efforts in furthering the understanding<br />

of skin diseases among the general public. We won<br />

awards for:<br />

1. <strong>Alopecia</strong> <strong>Areata</strong> Awareness Day on the Hill (when 600 people<br />

visited their legisla<strong>to</strong>rs on Capi<strong>to</strong>l Hill as a prelude <strong>to</strong> our 22 nd<br />

annual Conference)<br />

2. Our 25 th Anniversary Gala Concert and Reception with pianists<br />

Misha and Cipa Dichter<br />

3. Our Spanish documentary video, featuring NAAF spokesperson<br />

Charlie Villanueva, <strong>to</strong> expand our educational borders<br />

• <strong>The</strong> Tor<strong>to</strong>ise & Hair run/walk is growing in<strong>to</strong> NAAF’s premier<br />

nationwide awareness and fundraising event.<br />

• NAAF volunteers hosted fundraising/awareness events around the<br />

world, generating almost $130,000 for NAAF. All of these events<br />

are listed by state (or province) in this annual report. We offer<br />

our thanks <strong>to</strong> all the individuals and families who hosted—or participated<br />

in—these fantastic events!<br />

• NAAF representatives and other members of the Coalition of Skin<br />

Diseases, the American Academy of Derma<strong>to</strong>logy, and the Society<br />

for Investigative Derma<strong>to</strong>logy met with nearly 90 members of<br />

Congress and their staffs <strong>to</strong> ask for increased investment in funding<br />

for medical research.<br />

• NAAF and the <strong>National</strong> Coalition of Au<strong>to</strong>immune Patient Groups<br />

sent letters <strong>to</strong> the World Health Organization, providing input <strong>to</strong><br />

the strategic plan for Public Health, Innovation and Intellectual<br />

Property.<br />

• NAAF worked with the Genetic Alliance <strong>to</strong> pass the Genetic Information<br />

Nondiscrimination Act (GINA). President George Bush<br />

signed GINA in<strong>to</strong> law on May 21, 2008. It provides protections<br />

against discrimination based on an individual’s genetic information<br />

in health insurance coverage and employment settings, and it is<br />

the first and only such federal legislation of the new millennium.<br />

RESEARCH:<br />

Exploring Every Avenue and Planning for Future Research<br />

NAAF RESEARCH SUMMIT SETS PRIORITIES TO ADVANCE<br />

ALOPECIA AREATA RESEARCH<br />

NAAF conducted a high-powered, fast-paced, collaborative,<br />

and interactive summit on July 11 in Bethesda, Maryland, <strong>to</strong><br />

accelerate knowledge of and movement <strong>to</strong> therapies for alopecia<br />

areata. NAAF invited some of the most respected geneticists,<br />

immunologists, hair researchers, and mouse model experts<br />

<strong>to</strong> interact with our alopecia areata researchers as well<br />

as scientists from the <strong>National</strong> Institutes of Health. We invited<br />

representatives from other voluntary health agencies, like the<br />

Leukemia and Lymphoma Society, <strong>to</strong> talk about how they are<br />

accelerating their path <strong>to</strong> therapies. <strong>The</strong>re were also representatives<br />

from companies that are repurposing and repositioning<br />

drugs <strong>to</strong> see if that is an avenue NAAF wants <strong>to</strong> pursue.<br />

This summit led us <strong>to</strong> set the following priorities for future NAAF<br />

research efforts:<br />

Priority One: <strong>The</strong> Research Summit Planning Committee<br />

overwhelming agreed that use of the <strong>National</strong> <strong>Alopecia</strong> <strong>Areata</strong><br />

Registry (NAAR) will provide the greatest advancement of a<br />

broad research program, as well as development of treatments<br />

for all types of alopecia areata.<br />

Priority Two: Pursue Immunological Studies <strong>to</strong> determine if<br />

MHC Class I genes (the genes in a specific subgroup of the Major<br />

His<strong>to</strong>compatibility Complex) can be down-regulated as a possible<br />

treatment for alopecia areata. How can we block the immune<br />

cells from attacking the hair follicle?<br />

Priority Three: use the C3H/HeJ Animal Model and other<br />

models <strong>to</strong> screen FDA-approved drugs.<br />

Priority Four: Explore other avenues for new therapies.<br />

1. Try <strong>to</strong> find drugs in the market or in clinical research that may<br />

be viable therapies.<br />

2 Explore the possibility of repurposing drugs and tap in<strong>to</strong> current<br />

knowledge. (This could mean partnering with a pharmaceutical<br />

or a repurposing drug company <strong>to</strong> get the drug<br />

farther down the pipeline.)<br />

THE NATIONAL ALOPECIA AREATA REGISTRY HAS BEEN<br />

SUPPORTED BY NAAF FROM 2000 THROUGH 2008<br />

<strong>Alopecia</strong> areata tends <strong>to</strong> run in families, and NAAF continued <strong>to</strong><br />

be involved in the national registry of these families. This registry,<br />

which is currently supported with funding from the <strong>National</strong><br />

Institutes of Health (NIH), is being used in part <strong>to</strong> determine<br />

which genes contribute <strong>to</strong> a susceptibility <strong>to</strong> alopecia areata.<br />

Identification of these genes will provide a starting point for further<br />

investigation and perhaps new therapies.<br />

By the end of 2008, over 6,000 people had completed the<br />

initial registration questionnaire. Over 2,000 of these people had<br />

also completed the second phase of registration, providing blood<br />

samples and undergoing a medical exam. <strong>The</strong> more people we<br />

have in the registry, the more we learn about alopecia areata<br />

and the more clues we gain <strong>to</strong> find a possible cure or better<br />

treatments.<br />

Analysis of Candidate Susceptability Genes<br />

Related <strong>to</strong> <strong>Alopecia</strong> <strong>Areata</strong><br />

<strong>The</strong>se investiga<strong>to</strong>rs are studying a particular gene (named<br />

the AIRE gene) that has been associated with alopecia areata<br />

and seems <strong>to</strong> be involved in au<strong>to</strong>immunity in general. So far, this<br />

project has not shown a direct link between the AIRE gene and<br />

human alopecia areata, but this study is not yet complete. When<br />

it is complete, we hope it will put this matter <strong>to</strong> rest, one way<br />

or the other.<br />

Dr. Christiano is using samples from<br />

the <strong>National</strong> <strong>Alopecia</strong> <strong>Areata</strong> Registry <strong>to</strong><br />

study the genetics of alopecia areata. Bit<br />

by bit, Dr. Christiano is narrowing down<br />

the list of possible genes that may be associated<br />

with alopecia areata. It takes<br />

both time and money <strong>to</strong> sort through the<br />

“genetic haystack” searching for the “alopecia<br />

areata needle,” but once we have<br />

found the “needle”—or “needles” (there<br />

are probably multiple genes involved in<br />

alopecia areata)—we will understand more<br />

about what causes this disease and also<br />

have better targets for therapy.<br />

Gene Expression <strong>to</strong> Identify <strong>Alopecia</strong><br />

<strong>Areata</strong>–Prone People in Correlation with<br />

the Mouse Model of the Disease<br />

In this study, blood from patients with<br />

alopecia areata is being studied with a new<br />

technology <strong>to</strong> evaluate the activity of a<br />

large variety of genes. Similar studies had<br />

already been done on mice, and it was time<br />

<strong>to</strong> try it on humans. <strong>The</strong> data is yet not<br />

available, but when it is, it should at the<br />

very least show the similarities between<br />

the mouse model and human disease. It<br />

might also identify genes that prove <strong>to</strong> be<br />

important as a cause of the disease or targets<br />

for therapy.<br />

Molecular Analysis of Hair Follicle Structural<br />

Changes in <strong>Alopecia</strong> <strong>Areata</strong> Mice<br />

This study attempts <strong>to</strong> find genes that<br />

might be involved in causing alopecia areata.<br />

<strong>The</strong> researchers are using mice that develop<br />

a disease similar <strong>to</strong> that of alopecia<br />

areata in humans, and they are studying a<br />

group of genes that in the past have been<br />

found <strong>to</strong> be overactive or underactive in<br />

patients with alopecia areata. Performing<br />

this work is tedious and time consuming,<br />

and the investiga<strong>to</strong>rs have not yet reported<br />

any important data. However, finding the<br />

genes that are involved in causing alopecia<br />

areata in mice might lead us <strong>to</strong> better understand<br />

the human disease. For example,<br />

a gene that is over- or under-expressed<br />

might be a target for a drug that increases<br />

or decreases gene expression.<br />

This study is looking at a particular<br />

mouse mutation (the calcineurin mutant<br />

mouse) <strong>to</strong> see if it would serve as a good<br />

mouse model for alopecia areata. <strong>The</strong> findings<br />

<strong>to</strong> date suggest that it is not likely <strong>to</strong><br />

be a useful model. This was NAAF’s firstever<br />

Graduate Fellowship award.<br />

Cd39 Null Mice: A New Mouse Model for<br />

This mouse represents another possible<br />

new animal model for alopecia areata.<br />

However, the studies so far do not show<br />

any direct association of this mouse’s hair<br />

loss with human alopecia areata. <strong>The</strong>refore,<br />

this mouse may not be a good model<br />

for human disease.<br />

Studies of gene expression have shown<br />

that certain genes involved in vitamin A<br />

metabolism are altered during the hair<br />

cycle. Since alopecia areata is a hair cycle<br />

disorder, the investiga<strong>to</strong>rs decided <strong>to</strong> evaluate<br />

the affect of vitamin A on the current<br />

alopecia areata mouse model. <strong>The</strong> investiga<strong>to</strong>rs<br />

are looking at both gene expression<br />

and the effect of altering vitamin A in the<br />

diets of affected mice. <strong>The</strong> results are not<br />

yet final, and the study is ongoing, but if<br />

vitamin A is actively participating in alopecia<br />

areata, it would offer an obvious target<br />

for therapy.

This study is using a technology for studying the expression of large<br />

genes are being studied at various points during the progression of disease<br />

in the mice. If the important genes can be identified, it might<br />

suggest logical drugs <strong>to</strong> be used at different stages of the disease in<br />

Defining MHC Expression in Human Hair Follicles in <strong>Alopecia</strong> <strong>Areata</strong><br />

This study looks at the immune-related proteins on the pigmentproducing<br />

cells in hair follicles. <strong>The</strong> investiga<strong>to</strong>rs are first studying normal<br />

follicles, but will be comparing those findings with alopecia areata<br />

hair follicles. This should offer some information about whether these<br />

pigment-producing cells are related <strong>to</strong> the cause of alopecia areata.<br />

Follicle Immune Privilege<br />

<strong>The</strong>se investiga<strong>to</strong>rs are studying the<br />

immune system as it relates <strong>to</strong> hair follicles.<br />

Normal hair follicles do not express certain<br />

proteins that are required for an attack by<br />

white blood cells. <strong>The</strong>refore, normal hair<br />

follicles seem <strong>to</strong> be relatively safe from attack<br />

by white blood cells. But we know that<br />

in alopecia areata the hair follicle is indeed<br />

attacked. <strong>The</strong>se investiga<strong>to</strong>rs are studying<br />

why normal hair follicles are protected from<br />

immune attack, while alopecia areata follicles<br />

are not. <strong>The</strong> technology for doing this<br />

is complex, since they cannot experiment<br />

on live patients! This area of study is still in<br />

its infancy, but information is slowly emerging.<br />

At the very least, we will understand<br />

human hair biology better. At the very best,<br />

we will find some good targets for therapy<br />

in alopecia areata. Several more years of<br />

work will be required for this.<br />

Development of Co-Culture Models <strong>to</strong> Investigate<br />

Hair Follicle Lymphocyte<br />

<strong>The</strong>se researchers have grown human<br />

hair follicles isolated from redundant human<br />

skin and have modeled both the collapse of<br />

immune privilege and its res<strong>to</strong>ration. <strong>The</strong>y<br />

have also investigated how the cells of the<br />

immune system respond <strong>to</strong> cultured hair<br />

follicles following both the collapse and res<strong>to</strong>ration<br />

of immune privilege. This project is<br />

ongoing and results are not yet complete.<br />

on aspects that may indicate its short- and<br />

long-term safety. Since most patients with<br />

severe alopecia areata are subjected <strong>to</strong> intralesional<br />

triamcinolone ace<strong>to</strong>nide, which<br />

is a corticosteroid, at some point, the results<br />

are of practical importance. However,<br />

only a few patients have been recruited for<br />

Alefacept Study in <strong>Alopecia</strong> <strong>Areata</strong><br />

This study was performed as a<br />

multicenter, double-blind, randomized,<br />

placebo-controlled clinical trial at four<br />

academic institutions. Forty-five subjects<br />

were enrolled. unfortunately, it was shown<br />

that alefacept is no more likely <strong>to</strong> improve<br />

alopecia areata than a placebo.<br />

of this registry is one of the most important<br />

projects supported by NAAF because it can<br />

provide valuable material <strong>to</strong> many different<br />
