PhysicianInsightsFALL 2007 • VOLUME 3 • ISSUE 2Teaching Affiliations ExpandDMC <strong>Huron</strong> <strong>Valley</strong>-<strong>Sinai</strong> <strong>Hospital</strong> (HVSH)began its American Osteopathic Association(AOA)-accredited teaching programs in 2004.HVSH was the first DMC hospital to offerosteopathic graduate and undergraduatetraining.Currently, Michigan State Universityosteopathic students are offered rotations in21 specialties at DMC <strong>Huron</strong> <strong>Valley</strong>-<strong>Sinai</strong>.Residencies are offered in internal medicine andradiology and a residency in ear, nose andthroat is approved for July 2008. We currentlyhave 13 residents doing their training here andwe welcome them to our campus.This is solid progress in becoming ateaching hospital and there are plans to expandfurther. In conjunction with DMC <strong>Sinai</strong>-Grace<strong>Hospital</strong>, residency programs are beingdeveloped in eight additional specialties andsub-specialties: internal medicine, neurology,neurosurgery, obstetrics/gynecology,otolaryngology, cardiovascular surgery, andplastic and reconstructive surgery.Gastroenterology is offered as a fellowship.For students, a joint DMC <strong>Sinai</strong>-Graceand HVSH residency provides the advantageof a broader range of patients and clinicalexperiences – both urban and suburbanresidents, inpatients and outpatients. Bocknek, D.O.Chris Heberer, M.D.HVSH Reports on Quality MeasuresDMC <strong>Huron</strong> <strong>Valley</strong>-<strong>Sinai</strong> is providingextensive quality and outcomes data that isbeing compiled by the Centers for Medicareand Medicaid Services (CMS) and JCAHO.This data is available on their websites and isintended for the general public and third partypayors to assist in health care decision-making.These core measures and <strong>Huron</strong> <strong>Valley</strong>-<strong>Sinai</strong>’syear-to-date summary results are shown below.Acute myocardial infarction (AMI) –perfect care indicatorThe AMI perfect care indicator is a patientspecificappropriate care measure that requiresa hospital to meet all applicable measuresfor each patient. If one or more of themeasures is not met and the measure was notcontraindicated, the hospital will not receivecredit for any of the measures for that patient.For the year-to-date, the HVSH overall score was.97, compared to a DMC goal of .94.InsideCommunity Acquired Pneumonia (CAP)This is a measure of appropriate care fornon-ICU patients with community acquiredpneumonia. Approved antibiotic regimens mustbe used and blood cultures must be obtainedprior to the start of antibiotic treatment. Thiscare indicator also measures compliance withpneumoccocal/influenza vaccination andsmoking cessation advice. For the year to date,the hospital’s overall score is .87, comparedto the DMC goal of .90.Congestive Heart Failure (CHF)Appropriate care measures for CHF patientsinclude Left Ventricular Ejection Fractionassessment, use of ACEI or ARB with patientswith an EF of 40 percent or less, smokingcessation advise, and discharge instructionsthat include all of the following:• Guidance for what to do if symptoms worsen• Weight management instruction• Discharge medicationsPAGE 2 PAGE 3 PAGE 3• Follow-up appointment date• Diet• ActivityThe HVSH overall result was .91,compared to a DMC goal of .93.Surgical Care Improvement Project (SCIP)SCIP measures infection prevention for abroad scope of surgical procedures. SCIP willalso assess VTE prophylaxis, intra-operativecardiac arrest prevention measures (betablockers prior to surgery), as well as intraoperativenormo-thermia. Historically at <strong>Huron</strong><strong>Valley</strong>-<strong>Sinai</strong>, the primary concern has been thediscontinuation of prophylactic antibiotics within24 hours of surgical cut time, but throughthe collaborative efforts of our surgeons, thePharmacy Department and nursing staff, thishas improved significantly. Small volume casesstill pose compliance challenges. HCAHPSData TracksPhysician-PatientCommunicationsThe Centers for Medicare and Medicaid Services(CMS) began compiling patient satisfaction datathis year, including interactions between patientsand their physicians in the hospital. For the first twoquarters of 2007, <strong>Huron</strong> <strong>Valley</strong>-<strong>Sinai</strong> physicians’ratings in the <strong>Hospital</strong> Consumer Assessment ofHealthcare Providers and Systems (HCAHPS)exceeded the national averages. Vice-Chief Appointedfor Emergency MedicineVinesh Patel, M.D. has been appointedvice-chief for <strong>Huron</strong> <strong>Valley</strong>-<strong>Sinai</strong>’s Departmentof Emergency Medicine. Dr. Patel graduatedfrom the Medical College of Ohio (Toledo)and completed his residency in emergencymedicine at DMC Detroit Receiving<strong>Hospital</strong>/Wayne State University School ofMedicine in 2003. Since then, he has dividedhis clinical time between Detroit Receivingand <strong>Huron</strong> <strong>Valley</strong>-<strong>Sinai</strong> hospitals.Dr. Patel’s responsibilities will includeFor the fourth consecutive year, DMC<strong>Huron</strong> <strong>Valley</strong>-<strong>Sinai</strong> <strong>Hospital</strong> received theGovernor’s Award of Excellence for ImprovingPatient Safety and Quality Care. These awards are given to Michiganhospitals that embrace evidence-basedmedicine and quality improvement initiatives.<strong>New</strong> ProcessWill Help ReduceFraudulentPrescriptionsThe DMC has implemented a new processto help reduce fraudulent prescriptions in thecommunity. Prescriptions generated throughEMR now must also include a bar-coded sticker.External pharmacies will not fill them without thissticker. This applies only to computer-generatedprescriptions, not handwritten prescriptions.As always, the prescriber’s signature mustbe included.
