If you prefer to use apaper FAFSA, a PDF version is available atwww.federalstudentaid.ed.gov. Students canalso call (800) 433-3243 to order a paper FAFSA.1. Accessing FAFSA online:a) Be sure to apply for a PIN before submittingyour FAFSA; for dependent students, aparent must apply for a PIN as well. You mayregister for a PIN through www.pin.ed.gov.b) Once you have submitted your onlineFAFSA, and if you provided your emailaddress when you applied, you will receivean email notification containing a direct linkto your Student Aid Report (SAR) online.There are two versions of the SAR available,either HTML or PDF. Please send a copy of allpages of your SAR to the Blandin Foundation.2. If you make any corrections to your SAR,please submit a corrected copy of yourSAR to us.3. Complete the Application included in thisbrochure and return it to our office no laterthan May 1, 2012.
4. Send us a copy of your 2011 federal taxreturn and your parents’ federal tax return(for dependent students) form 1040,1040A or 1040EZ. If your family owns abusiness/corporation, please send us theappropriate schedule for that particularbusiness/corporation. If your family operatesa farm or has experienced high medicalcosts, include Schedules A and F. Please donot send us your Minnesota state incometax form. The Blandin Foundation may askfor additional financial information, suchas W-2s, or Form K-1/subcorp forms, toverify income.5. Complete and return the enclosed StudentRelease Form along with your Application.Note:1. It is important that you submit all necessaryfinancial aid papers to the Financial AidOffice at the school you plan to attend assoon as possible. This will enable us toget the additional information we needto complete your file.2. Please be sure to complete the FAFSAcorrectly – pay attention to instructions.Have you received anEducation Award fromus before?You may reapply each year by followingsteps 1–5 in the application process.To receive an award, you must meet thefinancial aid criteria each year based oncurrent financial data. Once you receive abaccalaureate degree or reach age 25 beforeSeptember 1, 2012, you may no longer applyfor an award. If you receive an award from us,we will send a new application directly to youthe following year.
