Overview of the CCO Community Advisory Council and RuralAdvisory CouncilAccord<strong>in</strong>g to ORS 414.625, Coord<strong>in</strong>ated Care Organizations (CCO) must have a Community AdvisoryCouncil (CAC) which makes recommendations to the CCO Govern<strong>in</strong>g Board <strong>in</strong> order to ensure thathealth care needs of the consumers and community are be<strong>in</strong>g addressed.Rural Community Advisory CouncilIn addition to the Community Advisory Council, a Rural Community Advisory Council will also bedeveloped to ensure that the health care needs of the rural consumers and community are be<strong>in</strong>g addressed.The Rural Advisory Council will work collaboratively with the Community Advisory Council and theGovern<strong>in</strong>g Board to transform the health care delivery system <strong>in</strong> rural parts of <strong>Lane</strong> <strong>County</strong> as well asensure that the care provided <strong>in</strong> the urban area is responsive to the needs of rural residents.The CAC and RAC must:• Include representatives of the community;• Have consumer representatives constitute a majority of the membership;• Meet no less frequently than once every 3 months; and• Have its membership selected by a committee composed of equal numbers of countyrepresentatives and members of the govern<strong>in</strong>g body of the organization.CAC/RAC Member Roles and ResponsibilitiesThe duties of the CCO Community Advisory Council and Rural Advisory Council <strong>in</strong>clude:• Identify<strong>in</strong>g and advocat<strong>in</strong>g for preventive care practices to be utilized by the organization;• Develop<strong>in</strong>g a community wellness program; and• Annually publish<strong>in</strong>g a report on the health of the organization that <strong>in</strong>cludes a review of the workof the organization and recommendations to the govern<strong>in</strong>g body of the organization for changeimprovements;• Members should be will<strong>in</strong>g to commit to an estimated 2 year term limit.How to ApplyAll <strong>in</strong>dividuals <strong>in</strong>terested <strong>in</strong> apply<strong>in</strong>g for the CCO-RAC should fill out the application on the front of thisform by September 15 th and mail to:L<strong>in</strong>dsey Adkisson<strong>Lane</strong> <strong>County</strong> <strong>Health</strong> & Human Services151 W. 7 th Ave., Room 520Eugene, Oregon 97405Or, by email at L<strong>in</strong>dsey.Adkisson@co.lane.or.us or fax at (541)682-3804 attn: L<strong>in</strong>dsey Adkisson.Upon receipt of the application, <strong>in</strong>terviews will be scheduled.For questions, please call L<strong>in</strong>dsey at (541)682-8772.<strong>Application</strong>s Due by September 15
