Medical Homes in LIHPEach enrollee will be assigned to a MH that coordinates all healthcare services. At a minimum the MH shall provide :• primary health care contact;• intake assessment;• care coordination, care management, case management, and transitionsamong levels of care, if needed and as agreed to between the medicalhome and LIHP;• use of clinical guidelines and other evidence-based medicine whenapplicable, focus on continuous improvement in quality of care;• timely access to qualified health care interpretation as needed and asappropriate; and• health information, education, and support in a culturally competentmanner.
