At The McGuire Rehabilitation Centers, we
understand how pulmonary disease
and related conditions impact your life.
Today, more than 12 million people in the
United states are diagnosed with COPD, a
form of pulmonary disease.
Our program focuses on providing
treatment for all types of Pulmonary-related
conditions as well as educating patients
about lifestyle changes that will help them
feel better, stay active and slow the disease
process.
Our Outpatient Pulmonary Rehab programs
are available at Autumn View in Hamburg
and Harris Hill in Williamsville. These
specialized programs has been developed
specifically to treat problems affecting the
lungs and lung function.

YOUR RECOVERY PLAN
The McGuire Group Outpatient Pulmonary Rehab
Program is designed for all individuals who have
pulmonary disease or breathing problems.
The goal of the program is to provide exercise,
education and counseling to individuals with
pulmonary conditions in a comfortable
outpatient setting.
The program will assist attendees in leading the most
active and comfortable lives possible while decreasing
the restrictions of their disease process.
Patients who attend the program will have a better
understanding of their disease, medications, the
importance of exercise and proper diet, and ways to
improve their quality of life.
A PRESCRIPTION
fOR RECOVERY
• 8 week program
• Classes are Monday, Wednesday & Friday
• 8 a.m. at Autumn View and 3 p.m. at Harris Hill
• Initial evaluation and plan of care developed on
the first visit
• Re-evaluation of program goals upon each visit
• 1 hour of exercise and informal instruction on
Monday and Wednesday
• 1/2 hour of exercise and formal education
class on Friday
• The formal education class will repeat every
4 weeks
Educational topics include:
3 Self monitoring care
3 Prevention of infection
3 Disease specific medication regimens
3 Nutrition & exercise and smoking cessation
3 Energy conservation and lifestyle modification
fLEXIBILITY
Participants are welcome to attend rehab at
Autumn View in Hamburg or Harris Hill in
Williamsville and repeat formal classes, if needed.
The program can be started directly from home or
after a hospital stay.
REQUIREMENTS
3 A physician referral or prescription is required
for admission to the program
3 Insurance coverage will vary according to the
type of insurance plan
3 Co-payments are dependent upon the type
of insurance plan
3 Private pay/no insurance rates are available
3 All insurance authorization and billing
performed by The McGuire Group personnel
3 Participants are required to call the facility of
their choice to sign up and schedule attendance
for the program (see back panel)
