Importance of Site Rotation
Site Rotation
• Essential to successful insulin pump therapy
• Change site location with every cannula change
• Plan your rotation pattern to ensure sites have time
to heal
Benefits of Site Rotation
• Lower infection risk
• Optimized insulin absorption
• Reduced potential for fatty tissue overgrowth
• Reduced potential for scarring
• Reduced variability in blood sugar levels
Site Rotation Frequency
• Immediately
- Pain, irritation or bleeding at the infusion site
- Two correction boluses fail to reduce high blood
sugar level
- Ketones are present in your blood or urine
- Blood appears in the infusion set tubing or around
the cannula
• Every 24-48 hours
- Steel cannula
- Pregnancy
• Every 48-72 hours
- Soft cannula
Note: Do not change your infusion set before going
to bed since your blood glucose must be measured
1 to 3 hours after insertion.
Infusion Set Change
• If your blood glucose level is unexplainably high or
your pump sounds an occlusion alarm, check for
clogs and leaks
• If in doubt, change your entire infusion set

Potential Consequences
of Poor Site Rotation

• Increased risk of infection
• Unpredictable insulin absorption
• “Pump bumps” found at the point of insertion
• Scar tissue
• Lipohypertrophy—enlargement in the infusion
site area
- Can result in poor insulin absorption and
increased insulin use
- Infusion sites outside of the lipohypertrophy
area may absorb insulin differently. Frequent
blood sugar monitoring is recommended.
TIP Write the date on the infusion set adhesive
dressing as an infusion set change reminder.
