Children Self Sign In / OutThe Zone has an open door policy, which stipulates that children are permitted to sign themselvesin and out. Parents must first sign a permission form to be kept on file at The Zone site, whichgrants children permission to sign themselves in and out. Once children have signed themselvesout for the day they may not return to The Zone.<strong>Permission</strong> <strong>Slip</strong> for the Computer Lab for The Zone Summer Camp<strong>Gilbert</strong> <strong>Public</strong> <strong>Schools</strong> limit the programs that students can access for a variety of reasons.Unfortunately, there are so many sites available that we can’t monitor and block all of them.MySpace and Facebook are currently blocked, as well as all instant messaging and chat sites.<strong>Gilbert</strong> <strong>Public</strong> <strong>Schools</strong> <strong>Permission</strong> to Photograph / Videotape Parent Release Form<strong>Permission</strong> is required for your child to be photographed / videotaped within the school settingwhile being involved in a school sponsored activity. Pictures may be used in newspapers, ontelevision, in publications, other media activities, or on the district’s internet website.I understand that this assignment will be in effect only during that period in which the child isenrolled in The Zone Summer Camp for the summer of 2013. Student Conduct for The Zone Summer CampAwareness Contract and Receipt of Student Handbook that OutlinesStudent Conduct at <strong>Gilbert</strong> Jr. High <strong>Schools</strong>This information is presented in order to ensure the safety and well-being of all students at <strong>Gilbert</strong> <strong>Public</strong> <strong>Schools</strong>. Itis also meant to ensure that all students and parents/guardians have received communication regarding theGoverning Board's special policies dealing with drug/alcohol abuse, dangerous items/deadly weapons, theattendance policy, and the student Electronic Information Services (EIS) - User Agreement. Policy ACB regarding harassment (e.g., sexual, race, color, national origin, religion or disability). Policy JICC regarding student conduct on district vehicles. Riding the bus is a privilege, not a right. * Policy JICH regarding drug/alcohol abuse. (I understand that drug/alcohol abuse includes illegal drugs, alcohol,prescription drugs and other mind-altering substances. Abuse includes the possession, use, distribution, sale, orpurchase of any of the above.) Policy JICI regarding dangerous instruments and deadly weapons in school. Policy JICF regarding negative student group or gang affiliation. Policy JKD regarding student suspension and expulsion/due process rights. Students will be suspended ten campdays for participation in a fight. I agree to abide by the Internet Agreement as presented in the Electronic Information Services (EIS)presentation.*ZONE Addendum to Policy JICC – Students may be suspended from riding the school bus if they are found in violationof Policy JICC. Those found in violation may not attend field trips.The ZONE has implemented a “NO TOLERANCE” policy for fighting. Any intentional physical contact, which results in theharm of an individual, constitutes fighting. Under these circumstances the following will apply:Any student witnessed in a physical altercation or bullying will be suspended indefinitely and, depending on theseverity of the incident, may be removed for the remainder of the summer program (refunds will not be given ifstudents are removed due to their involvement in a physical altercation).An Incident Report will be created documenting the incident and specifying the duration and terms of the suspension.A copy of this document will be made available to the parent’s of any individual involved.Any student under suspension may not return to campus until their suspension has ended.Prior to any student returning to the program site following a suspension, the individual suspended and a parent orguardian must meet with the Program Director in order to review Student Conduct policies.If I am found in violation of these policies and agreements, I may be suspended or expelled from school. My signaturebelow indicates I have listened to, or read, an explanation of the following Governing Board policies and theElectronic Information Services (EIS) - User Agreement and that my signature will be in force for the remainder of the2013-2014 school year.
