Pharmacy Technician Application Checklist<br />
Thank you for your interest in the upcoming <strong>Virginia</strong> <strong>Western</strong> Pharmacy Tech program that<br />
begins October 16, 2012! Please complete this form, including each of the steps listed below, to<br />
be considered for acceptance to the program:<br />

Complete each of the steps listed below.<br />
1. Complete a VWCC non-credit college application online at:<br />
http://www.virginiawestern.edu/studenthelp/student/applyonline.php .<br />
2. Provide your assigned Student EMPLID #________________. Please make a note of your<br />
Student EMPLID#. This will be used throughout your studies at VWCC.<br />
3. Complete VWCC COMPASS testing at the VWCC Main Campus, Library basement (visit<br />
http://www.virginiawestern.edu/ltc/ for details). Attach a copy of your test results to<br />
this form. If your test scores fall below acceptable limits, you may re-take the test up to<br />
two additional times. For practice testing, please visit<br />

4. Attend a MANDATORY Information Session on the program on Sept. 18, Sept. 20 &<br />
Sept. 25 from 5:30pm until 7:00 pm. Call (540) 857-6076.<br />

Program Dates:<br />
Student Information for Pharmacy Technician Program Fall 2012<br />
Tuesdays and Thursdays, October 16, 2012 through March 14, 2013, 6:00 pm until 9:30 pm in the<br />
Fishburn Hall Workforce Development Training Room, Suite F01 on the Main Campus of <strong>Virginia</strong><br />
<strong>Western</strong> <strong>Community</strong> <strong>College</strong>.<br />
Costs:<br />
$1,599 includes all books, materials and instructions for the class. While students in this non-credit<br />
program are not eligible for Federal Pell grants or traditional State/local tuition assistance programs,<br />
information on possible assistance options will be shared at the Information Sessions.<br />

Requirements:<br />
Students must be 18 years of age and have a high school diploma or GED; however, current high school<br />
students may be considered for the program under some circumstances. High school students, please<br />
call <strong>Dan</strong> <strong>Semones</strong> at (540) 857-6476 to discuss your individual situation.<br />
Students must be computer literate and have access to a computer with Internet access, an email<br />
address and printing capabilities. Students must complete <strong>Virginia</strong> <strong>Western</strong>’s Compass Tests and show<br />
the ability to read, write and do math equivalent to the 9 th -10 th grade level (Math-PreAlgebra, Writing<br />
33, Reading 69). While no specific limits are prescribed by this program, students are advised that the<br />
State and National Pharmacy Technician Certification Board and many employers will have restrictions<br />
and limits on:<br />
Individuals who have had disciplinary action against their pharmacy technician license,<br />
certification, or registration in any other jurisdiction, or have been prohibited from<br />
performing the duties of a pharmacy technician by any other state, or prohibited by a<br />
health regulatory board of any state or by any federal agency from practicing, or<br />
assisting in the practice of, any health profession
Individuals who have been convicted of, pled no contest to, or have charges pending of<br />
any felony, or any crime involving moral turpitude, or a violation of any federal, state, or<br />
local drug law.<br />
Individuals who have been physically or emotionally dependent upon the use of alcohol<br />
or drugs or treated by, consulted with, or been under the care of a professional for any<br />
substance abuse within the last two years.<br />
Individuals who have a physical disease, mental disorder, or any condition, which could<br />
affect their performance of duties<br />

PHARAMCY TECHNICIAN PAYMENT & REFUND POLICY<br />
Tuition for the Pharmacy Technician program in the amount of $1,599.00 is due on October 12, 2012.<br />
Tuition payments can be made after the following process has been completed by the Administrative<br />
Staff of <strong>Virginia</strong> <strong>Western</strong> <strong>Community</strong> <strong>College</strong>:<br />
� After the admission decision date of Oct 5, 2012, a letter will be mailed to you offering you a<br />
seat in the Pharmacy Tech Program<br />
� You will return the response form indicating your intention to accept a seat in the program<br />
� After we receive your letter accepting a seat in the Pharmacy Tech Program, the Administrative<br />
Staff will enroll you in the program. If you want to confirm that your letter was received and to<br />
verify your registration in the program, please contact <strong>Dan</strong> <strong>Semones</strong>, (540) 857-6476.<br />
� Payment can be made at any of the <strong>Virginia</strong> <strong>Western</strong> locations, i.e. Main Campus, Greenfield<br />
Education and Training Center or the Franklin Center. Your payment may be made by check,<br />
money order or credit card (NO CASH except at the Main Campus Cashier’s office location). No<br />
financial assistance for the Pharmacy Technician program is available through <strong>Virginia</strong> <strong>Western</strong><br />
<strong>Community</strong> <strong>College</strong>’s Financial Aid office. Additional options for assistance with paying the costs<br />
of the program will be shared at the Information Sessions.<br />
Students who are unable to complete the Pharmacy Technician program will be considered for refunds<br />
using the schedule shown below when a written request for refund is received by <strong>Virginia</strong> <strong>Western</strong>’s<br />
Workforce Development Division Office; Attention: <strong>Dan</strong> <strong>Semones</strong> (PO Box 1400, Roanoke, VA 24038).<br />
Attendance and continuation requirements will be discussed at the Information Sessions.<br />
REFUND SCHEDULE<br />
Students who withdraw in writing before October 23, 2012: Refund amount $1,400.00<br />
Student who withdraw in writing before November 6, 2012: Refund amount $1,200.00<br />
Students who withdraw in writing before November 20, 2012: Refund amount $720.00<br />
No refunds will be provided to students who withdraw on or after November 20, 2012.
