educational objectivesThis activity has been designed to meet the educationalneeds of nephrologists, nephrology practice managersand advanced practitioners involved in the care of kidneypatients.At the conclusion of this activity participants should bebetter able to:• Explain the unique nature of the incident dialysis patient.• Describe the impact on mortality, hospitalization andcomorbid disease that results from lack of preparation /lack of access to care for the CKD patient.• Identify a process to integrate timely referral andstructured care of the CKD patient by utilizing therelationship between the nephrologist and the primarycare physician.• Describe the impact of structured CKD care onpathophysiologic changes and late stage CKDmanagement.• Discuss the impact that health care reform and CMSpolicy will have on nephrology practice and the abilityto move towards higher quality at lower cost.• Employ appropriate goals and management strategiesthat are most beneficial to patients that are unable toproceed with rehabilitative renal replacement therapy.• Evaluate the means by which clinical quality measuresquantify mortality and morbidity changes in the dialysispopulation.• Describe strategies for dealing with patients that arecounterproductive to the smooth workflow and deliveryof care to the larger population in both the outpatientpractice and the dialysis facility.• Assist uninsured patients with finding appropriateresources.• Select the most appropriate tools or mechanisms formanaging uninsured patients within the practiceenvironment.• Apply proper coding, billing and documentationstandards for a nephrology practice• Define meaningful use according to the provisionswithin the HITECH Act.• Discuss how telehealth technology has the abilityto optimize continuous contact and monitoring ofpatients with fewer asynchronous health delivery systeminteractions between the patient and their caregivers.• Develop population management tools designedto incorporate performance risk through pay forperformance, shared savings, accountable careorganizations and capitated risk.• Describe techniques to reduce catheter use inESRD patients on hemodialysis.• Summarize the roles and responsibilities of thenephrologist, surgeon, dialysis provider and hospitalfor reducing catheter use in hemodialysis patients.• Discuss the differences between providing administrativemedical oversight to populations of kidney patientsversus individual patients under your care.• Explain the impact of bundling on how nephrologistsoversee ESRD care and medical direction of dialysisfacilities.• Discuss risk management techniques for the provider inthe current environment for malpractice in nephrology• Demonstrate techniques for improving communicationwith patients to achieve greater adherence with theirtreatments.• Describe an approach that the quality improvementorganization can implement within the hospitalenvironment and the outpatient environment topromote more effective CKD care.• Discuss the impact of physician public reporting onthe local community and the potential implications onmaintaining physician-patient relationships.• Evaluate the role of alternative dialysis modalities forrenal replacement therapy.• Explain the current federal regulations on theappropriate use of erythropoiesis stimulating agentsin the care of CKD patients.• Compare nephrology practice benchmarkingperformance against national and regional standardswith regard to configuration of providers, productivity ofproviders, and the distribution of revenue and expenses.www.renalmd.org 2011 preliminary program & <strong>registration</strong>5
