<strong>Dear</strong> <strong>CLLD</strong> <strong>Participant</strong>:<strong>Thank</strong> <strong>you</strong> <strong>for</strong> <strong>being</strong> <strong>part</strong> <strong>of</strong> <strong>our</strong> <strong>Center</strong> <strong>for</strong> Life Long Development.We hope that <strong>you</strong> and <strong>you</strong>r families have fared well through the hurricaneand that <strong>you</strong> are back into <strong>you</strong>r own swing <strong>of</strong> things!We are excited to announce that we have lifted the freeze <strong>for</strong> 400 <strong>of</strong>those who are on the waiting list. We now have over 2,000 <strong>part</strong>icipants enrolledin <strong>CLLD</strong>!We have received great feedback and are eager to share with <strong>you</strong> some improvementsto the program and registration process.Here are the CHANGES to the registration process:1) No registration <strong>for</strong> any Sports & Wellness c<strong>our</strong>ses: Sports & Wellnessc<strong>our</strong>ses no longer require registration in advance; they are now on a firstcomefirst serve basis. Space is still limited to the number <strong>of</strong> slots indicatedin the program guide.2) C<strong>our</strong>se limits: You may only register <strong>for</strong> six c<strong>our</strong>ses under “1st choice”and f<strong>our</strong> c<strong>our</strong>ses under “2nd choice” on the C<strong>our</strong>se Registration Form. Youmay only register <strong>for</strong> three trips on <strong>you</strong>r Trip Registration Form.Registration <strong>for</strong> <strong>CLLD</strong> Classes and ProgramsMonday, April 15 – Friday, June 21, 2013Please read this section carefully and follow the instructions <strong>for</strong> Steps #1through #4 to assure that <strong>you</strong>r registration <strong>for</strong>ms are correctly filled outand submitted on time.Step #1: On Wednesday, April 3, Thursday, April 4, Friday, April 5, andMonday, April 8, the April-June Program Guide and registration <strong>for</strong>m(s) willbe available at the Avis/South Shore JCC, the Joan & Alan Bernikow JCC(in the Lewis Stolzenberg Social Hall during business h<strong>our</strong>s) and the NorthShore JCC. For those who are away, the Program Guides and RegistrationForms are available online at clld.sijcc.org.Step #2: You may take <strong>you</strong>r materials home to review OR fill them out onthe spot. There are two registration <strong>for</strong>ms:1) C<strong>our</strong>se Registration Form – You can register up to six c<strong>our</strong>ses <strong>for</strong> <strong>you</strong>r“1st choice” and f<strong>our</strong> c<strong>our</strong>ses <strong>for</strong> <strong>you</strong>r “2nd choice”.Continued on reverse side


You can register <strong>for</strong> unlimited one-time session programs (highlighted on <strong>you</strong>r C<strong>our</strong>seRegistration Form) – <strong>you</strong> may select them in addition to <strong>you</strong>r 1st choices.No registration is required <strong>for</strong> Thursday night dinners. A $5 fee is required <strong>for</strong> allThursday night entertainment, along with a $3 suggested contribution <strong>for</strong> dinner. Bothwill be collected at the door to the event.2) Trip Registration Form – If <strong>you</strong> are interested in trips, complete the separate TripRegistration Form <strong>for</strong> up to three trips.The number <strong>of</strong> sessions in the Program Guide may differ from c<strong>our</strong>se to c<strong>our</strong>se.Step #3: The deadline <strong>for</strong> returning the C<strong>our</strong>se and/or Trip Registration Form(s) isMonday, April 8 by 3:00 p.m. The <strong>for</strong>m(s) may be returned in person by <strong>you</strong>, afriend, or family member, to the <strong>CLLD</strong> volunteer table located in the Lewis StolzenbergSocial Hall <strong>of</strong> the Joan & Alan Bernikow JCC. Spouses, <strong>part</strong>ners or significantothers should staple <strong>for</strong>ms together. Y<strong>our</strong> name should appear on each page <strong>of</strong> the<strong>for</strong>m(s).The registration will again be done via randomization s<strong>of</strong>tware.There will be a dual randomization process: one <strong>for</strong> those who submitted the C<strong>our</strong>seRegistration Form and another <strong>for</strong> those who submitted the Trip Registration Form.If <strong>you</strong> need assistance completing <strong>you</strong>r registration <strong>for</strong>ms, please contact the <strong>CLLD</strong> <strong>of</strong>ficeduring regular business h<strong>our</strong>s. Volunteers will also be available in the Lewis StolzenbergSocial Hall April 9-11 to answer any questions.Step #4: C<strong>our</strong>se confirmations can be picked up in the Lewis Stolzenberg Social Hallon Friday, April 12th 10:00am-3:00pm and Sunday, April 14th 10:00am-3:00pm.If <strong>you</strong> would like to add or drop a c<strong>our</strong>se or program, <strong>you</strong> may complete a C<strong>our</strong>seAdd/Drop Form in the <strong>CLLD</strong> Office on Monday, April 15th.March 28 and 29: Highlights!
