How to Pick a Primary Care Provider (PCP) for Your Child<br />

Use this Provider Directory to pick your child’s Primary Care Provider. Your child’s Primary Care Provider is also called the “medical home.” You can pick one Primary<br />

Care Provider for each child. You can also pick the same Primary Care Provider for all your children. Primary Care Providers can be a Family Doctor, General Doctor,<br />

or Pediatrician. Each Primary Care Provider has a unique number assigned to their office location. Please use this number when completing your child’s CHIP<br />

Enrollment Application.<br />

lf you ever want to change your child’s Primary Care Provider, call Member Services at 1-888-814-2352. We will be glad to help.<br />

lf your child has a Chronic and Complex Special <strong>Health</strong> Care Need (CCSHCN), contact Member Services at 1-888-814-2352. We can help you pick a special doctor<br />

(specialist) to be your child’s Primary Care Provider, dedicated to your child’s specific health care needs.<br />

Additional providers not listed in this directory may be available in counties outside the Service Area. Please log on to www.parklandhmo.org or<br />

call the Member Services phone number on your ID card for more information.<br />

How to Use Obstetrical/Gynecology (OB/GYN), Shots and Mental <strong>Health</strong> Services without a Referral<br />

With <strong>Parkland</strong> CHIP, your child gets OB/GYN care, shots, and mental health services without a referral. To get these services, you can do the following:<br />

Obstetrics and Gynecology (OB/GYN)<br />

1. You do not need a referral from your daughter’s Primary Care Provider<br />

to get OB/GYN services. Talk with your daughter’s Primary Care Provider<br />

if you have questions. The Primary Care Provider will explain these<br />

services to you. If your child is an adolescent female you can also pick<br />

an OB/GYN as her Primary Care Provider.<br />

2. You can call Member Services at 1-888-814-2352 for more information<br />

or for help finding an OB/GYN.<br />

Shots<br />

1. You do not need a referral from your child’s Primary Care Provider to get<br />

needed shots for your child.<br />

2. Use your Provider Directory for a listing of providers that give shots, or<br />

call Member Services at 1-888-814-2352.<br />

Mental <strong>Health</strong><br />

1. <strong>Parkland</strong> CHIP Members<br />

<strong>Parkland</strong> CHIP provides mental health (drug and alcohol abuse) services.<br />

If your child is having a mental health/alcohol or drug abuse problem,<br />

you can call for an appointment with your child’s Primary Care Provider.<br />

You can also call behavioral health directly, toll-free at 1-800-945-4644.<br />

You do not need a referral from your child’s Primary Care Provider to get<br />

these services.<br />

If your child has a mental health emergency and needs help, go to the<br />

nearest Emergency Room or call CompCare toll-free at 1-800-945-4644.<br />

Someone will help your child get care right away. You or someone on<br />

your child’s behalf will need to call the mental health provider line and<br />

let them know about the emergency.<br />

For a listing of Mental <strong>Health</strong> providers, please contact CompCare<br />

at 1-800-945-4644.<br />

2. If you need immediate Mental <strong>Health</strong> Services or if you do not know<br />

what you need to do, call your child’s Primary Care Provider. You can also<br />

call Member Services at 1-888-814-2352 or the <strong>Parkland</strong> Nurse Line at<br />

1-800-357-3162 and get the help you need. Service is available in<br />

Dallas, Collin, Ellis, Hunt, Kaufman, Navarro, and Rockwall counties.<br />

Independent Practice Association (IPA) Network Disclosure Statement<br />

<strong>Parkland</strong> CHIP has an open provider network for all <strong>Parkland</strong> CHIP members. <strong>Parkland</strong> CHIP does not limit your child’s pick of a Primary Care Provider, or a referral, to a<br />

Specialist or Ancillary Provider. lf you have any questions about our provider network, call Member Services at 1-888-814-2352.<br />
