Introduction to the<br />
Supported Living Service<br />
We want to ensure that we provide you with<br />
a place of change that enables you to plan<br />
for your future. We will provide you with a<br />
temporary home and one to one support to<br />
help prepare you to live on your own.<br />
<strong>About</strong> <strong>Erw</strong> <strong>Groes</strong> <strong>Family</strong> <strong>Centre</strong><br />
<strong>Erw</strong> <strong>Groes</strong> <strong>Family</strong> <strong>Centre</strong> is an 11 flat Project<br />
in Holywell that provides high quality short<br />
term / emergency accommodation for<br />
homeless families. <strong>Erw</strong> <strong>Groes</strong> <strong>Family</strong> <strong>Centre</strong><br />
can accommodate vulnerable families in one<br />
of its 11 fully furnished self contained flats<br />
that include kitchen, lounge, bedrooms and<br />
bathroom. There is also secure parking on<br />
site for Residents accessing the Project.<br />
We provide staff support during 7 days a<br />
week in safe and secure premises, and the<br />
opportunity to break free from a homeless<br />
lifestyle.<br />
How do we help you?<br />
The type of support provided will vary<br />
according to your specific needs however,<br />
listed below are some examples of the type<br />
of support we can provide.<br />
l Staff support 365 days of the year.<br />
l Designated support worker.<br />
l On call system.<br />
l Parenting advice.<br />
l Domestic violence support.<br />
l Welfare Benefits advice.<br />
l Money Management.<br />
l Advice on health care e.g. Doctor, Dentist etc.<br />
l Drug and Alcohol advice and referral.<br />
l Structured move on plan.<br />
l Access to move on accommodation.<br />
l Access to employment and training.<br />
l Life skills advice.<br />
Getting Involved<br />
There are plenty of ways that you can get<br />
involved for example the Project arranges<br />
and delivers with the support of Residents<br />
a range of Resident Involvement activities<br />
such as art, gardening, fun days out, and<br />
fundraising events.<br />
The annual “Resfest” allows you to meet<br />
with Residents from other Projects and<br />
participate in a number of exciting activities<br />
such as music,dance, sports and falconry.<br />

Your family can apply to live at <strong>Erw</strong> <strong>Groes</strong><br />
<strong>Family</strong> <strong>Centre</strong> by contacting Flintshire<br />
County Council‛s Homeless and <strong>Housing</strong><br />
Options Team on % 01352 703815 and<br />
they will make a referral on your behalf.<br />
Assessing Your<br />
Support Needs<br />
On your first visit to <strong>Erw</strong> <strong>Groes</strong> <strong>Family</strong><br />
<strong>Centre</strong>, we will meet with you and assess<br />
your initial needs and risk. We do this so<br />
that we can find out if we can meet all your<br />
support and the best way to assist your<br />
family during their time at the Project.<br />
There are times when we may not be able<br />
to accommodate you, if this turns out to be<br />
the case, please don‛t be alarmed, as we will<br />
explain our reasons and we will do our best<br />
to help you find alternative accommodation.<br />
Throughout your stay at <strong>Erw</strong> <strong>Groes</strong> <strong>Family</strong><br />
<strong>Centre</strong> our staff will help you to work<br />
through an Individual Support Plan, which<br />
will help you to achieve your short and longterm<br />
goals and aspirations.<br />
How much does it<br />
cost to stay at <strong>Erw</strong> <strong>Groes</strong><br />
<strong>Family</strong> <strong>Centre</strong>?<br />
Once you become a Resident, we will help<br />
you to fill in a <strong>Housing</strong> Benefit Form so that<br />
<strong>Housing</strong> Benefit can pay your weekly rent.<br />
You will be asked to pay a weekly “personal<br />
charge” to help with the cost of heating<br />
and lighting..
