All employees must sign thedrug test policy and initial where required.If the new handbook page (from Dec 2006 forward ) is used, this is substituted for(and takes the place of) the signed drug test policy, signed key control policy &signed cash control policy.To evaluate:Check each employee file to make sure that the correct drug test policy form is on fileand properly filled out and signed. Mark “NO” if any policy is missing, incompletely orincorrectly filled out or not signedSECTION TWO - Drug test results must be present in the personnel file.Points 15RESULTS from the drug test should be in the file. The test must be done and resultsreceived PRIOR TO OFFERING THE APPLICANT A POSITION. Corporate approvalfor hire must accompany any failed drug test result.Any team member hired before December 1, 2004 is exempt from the requirement tohave a copy of their drug test results in their file at the property. As this is a change inprior procedure, any previous deficiencies noted during an internal audit will not beaffected.


Our drug testing company, Medtox, stores all drug test results in their databaseindefinitely. If a team member is missing results, the GM should pull up and reprintprevious drug test results by logging onto the WebTox site. Access to WebTox can beobtained from the HR department at the corporate office.Test results must be negative. If results are positive, permission to hire must be presentfrom the Vice President of Human Resources.To evaluate:Check each employee file to make sure that the drug test results are on file. Mark “NO”if any form is missing.Check each drug test result to ensure that results are negative. Mark “NO” if any resultis positive and contact the Director of Loss Prevention and the Vice President of HumanResources.
