TARGET AUDIENCE
This workshop will interest health care providers,
administrators and other relevant stakeholders who
want to advance the integration of evidence-based
practices in chronic disease prevention and
management across the continuum of care.
WORKSHOP OBjECTIVES:
1. To develop the skills for early recognition of chronic
conditions and effectively manage the conditions
within a busy practice; including how to:
• incorporate comprehensive screening for
select chronic conditions; and,
• integrate effective chronic disease
prevention and management into daily
practice.
2. To develop the strategies and tools to use
evidence-based care and continuous quality
improvement within your practice.
3. To identify practical collaborative approaches to chronic
disease prevention and management that provide
seamless transitions across care settings.

history of the Canadian Opiod Guidelines.
• Demonstrate how the guidelines and
website can be used to solve clinical
issues using clinical examples provided
by the participants.

• To understand the different ways in which
depression can affect the outcome of any
chronic disease.
• To look at simple interventions that can
improve mood symptoms in any clinical
encounter with someone with a chronic

• To better understand CKD and when to
refer to nephrology.
• To review prevention and management
