TARGET AUDIENCE<br />

This workshop will <strong>in</strong>terest health care providers,<br />

adm<strong>in</strong>istrators and other relevant stakeholders who<br />

want to advance the <strong>in</strong>tegration of evidence-based<br />

practices <strong>in</strong> chronic disease prevention and<br />

management across the cont<strong>in</strong>uum of care.<br />

WORKSHOP OBjECTIVES:<br />

1. To develop the skills for early recognition of chronic<br />

conditions and effectively manage the conditions<br />

with<strong>in</strong> a busy practice; <strong>in</strong>clud<strong>in</strong>g how to:<br />

• <strong>in</strong>corporate comprehensive screen<strong>in</strong>g for<br />

select chronic conditions; and,<br />

• <strong>in</strong>tegrate effective chronic disease<br />

prevention and management <strong>in</strong>to daily<br />

practice.<br />

2. To develop the strategies and tools to use<br />

evidence-based care and cont<strong>in</strong>uous quality<br />

improvement with<strong>in</strong> your practice.<br />

3. To identify practical collaborative approaches to chronic<br />

disease prevention and management that provide<br />

seamless transitions across care sett<strong>in</strong>gs.<br />
