<strong>Achieving</strong> <strong>good</strong> <strong>end</strong> <strong>of</strong> <strong>life</strong> <strong>care</strong><strong>for</strong> <strong>people</strong> <strong>with</strong> <strong>dementia</strong>FacilitatorsMin Stacpoole, Research Nurse, End <strong>of</strong> Life Care <strong>for</strong> People <strong>with</strong>Dementia, St Christopher’s HospiceMin managed a long stay ward <strong>for</strong> older <strong>people</strong> be<strong>for</strong>e coming to work atSt Christopher’s Hospice. She worked in the in-patient unit and then asa Clinical Nurse Specialist in Lambeth, Southwark, Lewisham, Bromleyand Croydon. She managed a caseload <strong>of</strong> palliative <strong>care</strong> patients inCroydon Care Homes <strong>for</strong> 3 years. From 2009, she worked <strong>for</strong> theModernisation Initiative’s End <strong>of</strong> Life Care Programme as NurseFacilitator <strong>for</strong> <strong>end</strong> <strong>of</strong> <strong>life</strong> <strong>care</strong> <strong>for</strong> <strong>people</strong> <strong>with</strong> <strong>dementia</strong>.Jessica Barnes-Webb, Dementia and End <strong>of</strong> Life Clinical NurseSpecialist, Trinity HospiceJessica has a degree in Mental Health Nursing from Ox<strong>for</strong>d BrookesUniversity and initially worked as a staff nurse on an acute mental healthward <strong>for</strong> older adults. She then moved to London in 2006 and worked <strong>for</strong>4 years as a CPN in a mental health <strong>care</strong> <strong>of</strong> older <strong>people</strong> team. She alsoworked as a research nurse on clinical trials <strong>for</strong> anti-<strong>dementia</strong>medication and, enjoying working <strong>with</strong> <strong>people</strong> <strong>with</strong> <strong>dementia</strong>, decided tospecialise in this area. Jessica started work at Trinity Hospice as aDementia and <strong>end</strong> <strong>of</strong> <strong>life</strong> Clinical Nurse Specialist in March 2010. This isa new and developing post recognising the need <strong>for</strong> improved access topalliative <strong>care</strong> <strong>for</strong> <strong>people</strong> <strong>with</strong> <strong>dementia</strong>.Target audience: GP’s, District Nurses, Community Matrons, CPNs,Care Home nurses, Domiciliary Care Managers, Social Workers, AlliedHealth Pr<strong>of</strong>essionals, and other <strong>care</strong> workers who would like theopportunity to reflect on their work <strong>with</strong> <strong>people</strong> <strong>with</strong> <strong>dementia</strong>.Estimates suggest that by the <strong>end</strong> <strong>of</strong> the decade there could be up to amillion <strong>people</strong> living <strong>with</strong> <strong>dementia</strong> in the UK. All <strong>of</strong> us, especially healthand social <strong>care</strong> pr<strong>of</strong>essionals, need to understand the impact <strong>of</strong><strong>dementia</strong> on the lives <strong>of</strong> individuals and families.Research has found that <strong>end</strong> <strong>of</strong> <strong>life</strong> <strong>care</strong> <strong>for</strong> <strong>people</strong> <strong>with</strong> <strong>dementia</strong> is<strong>of</strong>ten poor and that they are <strong>of</strong>ten hospitalised inappropriately because<strong>of</strong> lack <strong>of</strong> planning and co-ordination. The primary <strong>care</strong> team, <strong>care</strong>home staff and social <strong>care</strong> pr<strong>of</strong>essionals play a key role in recognisingdisease progression and supporting the family to plan ahead and ensurethat the <strong>end</strong> <strong>of</strong> <strong>life</strong> <strong>care</strong> wishes <strong>of</strong> the person <strong>with</strong> <strong>dementia</strong> arerespected and the person dies peacefully and com<strong>for</strong>tably.End <strong>of</strong> <strong>life</strong> <strong>care</strong> <strong>for</strong> <strong>people</strong> <strong>with</strong> <strong>dementia</strong> does not usually needspecialist palliative <strong>care</strong> intervention. However, planning <strong>end</strong> <strong>of</strong> <strong>life</strong> <strong>care</strong><strong>for</strong> someone who lacks capacity to make their own decisions is differentfrom advance <strong>care</strong> planning <strong>with</strong> someone who is cognitively able;assessing pain and other physical symptoms is complicated when theperson cannot communicate their distress verbally; and supportingfamilies through the long dwindling <strong>end</strong> stages <strong>of</strong> <strong>dementia</strong> can bechallenging.
