A FINAL WORD FROM THE PRESIDENTThis will be my last contribution to the WFSA <strong>annual</strong> <strong>report</strong> since 1996, a strange sensation. Having served eightyears as your Secretary, I have now entered my last year as President. To follow Kester Brown was a difficulttask. Only those close to him will remember the struggles he went through in his Presidency such as a retiringExco chair halfway; a secretariat in disarray immediately following the Montréal World Congress and a breakdown<strong>of</strong> the stock market. Few outsiders appreciate the enormity <strong>of</strong> some <strong>of</strong> WFSA tribulations, all carried out in therole <strong>of</strong> scientific volunteer.I have ensured WFSA’s Executive Committee involvement in my activities and hopefully our membership.Those who know me better and some around me, are aware <strong>of</strong> my enthusiasm and drive. However, I have cometo realise it takes more to roll WFSA up the mountain into the 21 st century from a group <strong>of</strong> enthusiastic pioneers inthe fifties into a global <strong>federation</strong> encompassing 122 <strong>societies</strong>.WFSA is an interesting organisation and quite unique joining all national member <strong>societies</strong> <strong>of</strong> <strong>anaesthesiologists</strong>in the <strong>world</strong> together in one body aimingto make available the highest standards <strong>of</strong> anaesthesia pain treatment, trauma management and resuscitation toall peoples <strong>of</strong> the <strong>world</strong>.WFSA’s aims are generally wide and embrace its core activities education, publication, safety and quality <strong>of</strong>practice. Needless to say, money is required to support these activities. Creating more resources was why WFSAcommenced the Foundation. The dream behind this was to raise funds externally thus decreasing thedependence on subscriptions and a WCA surplus.Times in the 1 st <strong>world</strong> are changing rapidly due to prosperity in some countries, economic growth in others anddevelopment such as internet, Skype and cell phones. People realise slowly that peace brings more welfare thanwar, at least so we noticed the last 60 years in Western Europe. The previously called 2 nd <strong>world</strong> such as EasternEurope, Middle East, India and China is swiftly catching up and promises much for the future. To translate thesetechnological and economical developments into the WFSA <strong>of</strong> this century, day and age has met with a number<strong>of</strong> hurdles which I would like to share with you.A new Constitution or the rules <strong>of</strong> the gameAn organisation based on scientific, charitable, voluntary contributions should not unnecessarily loose its timeover who directs what, how and why workings take place. Guidelines, statutes and bylaws should be transparent,understandable and accessible to all members. WFSA’s procedures and jobs should in my eyes be clearlyadvertised, applied for by willing and able people and selected in a transparent way. In the past, WFSA was reallyrun by few volunteers with time and interest available. Funds were scarce so requests sparse. Now thatEducation, Publications and Safety have obtained fair budgets, an increase in interest to organise the <strong>world</strong>congress or participate in WFSA activities is observed. Where in the past it was difficult to identify willing and ableparticipants, now they seem to be knocking at the door. This change into a true global <strong>federation</strong> with a fairbudget and permanent <strong>of</strong>fice requires a change in structure and thus in constitution. All <strong>of</strong> you have received thedraft in April, approx. one third <strong>of</strong> member <strong>societies</strong> has commented and suggested additions or alterations, onethird has more or less agreed and one third has not responded. Please let your voice be heard!Communication may be understood as I hear what you say; or I understand what you mean to say; or let metranslate it into the language I think in - so I may consider my reply; subsequently I need to chew my answer intothe right language- words – colour; non verbal body language <strong>of</strong> the sender and receiver, behaviour and attitudetoresult finally in the actual reply I care to give. Most <strong>of</strong> us in WFSA think in another language and thus have togo through this process which is <strong>of</strong>ten considered time consuming, cumbersome and awkward for those withEnglish as its first language. All <strong>of</strong> us are shaped by our family, education, culture and working environment. Wetend to project a comparable framework in others before we indeed wonder and reflect about the wordsexpressed. In this speedy <strong>world</strong> we assume rather than verify: we are só busy! Yet, the process <strong>of</strong> truly listeningand trying to understand your partner in conversation makes an enormous difference in the outcome <strong>of</strong>constructive deliberations.Working together with the members <strong>of</strong> the Committee on Statutes and Bylaws chaired by the cautious guidance<strong>of</strong> Bill Owens on this new draft WFSA constitution, the members <strong>of</strong> the Executive Committee have realised theimportance <strong>of</strong> real communication and the art <strong>of</strong> diplomacy respecting each other’s traditions and cultures.Understanding, supportive listening and patience will be required if WFSA truly continues to build on the past andremains a multi-lingual, international global <strong>federation</strong>, unless the Treasurer is prepared to spend heaps <strong>of</strong> moneyon simultaneous interpretation. Let us not forget our first and foremost goal: to provide better educated and safercare for our patients!The airline <strong>world</strong> has realised the importance <strong>of</strong> communication following some major accidents. Crew resourcemanagement was established as a result <strong>of</strong> that. Rhona Flin and her group in Aberdeen 1 have translated this into3


the non-technical skills <strong>of</strong> <strong>anaesthesiologists</strong>. The paper and philosophy behind it is essential for futurepractitioners.Angela Enright and her team (Education) have once again been able to assist numerous <strong>anaesthesiologists</strong>around the <strong>world</strong> achieving better insight, knowledge and skills. The fellowships in cardiac, neuro and paediatricanaesthesia with support from Dräger developed further and wider as did the basic training programmes andrefresh courses. The ESA has notably supported the Education team in Eastern Europe for which we are grateful.NVA and CAS have supported fellowships the support for these was essential for them to take place.Iain Wilson and his gang (Publication) steadily continue to publish Update and would really like to increase theirnumbers. So far it is published in five (5) languages: French, English, Mandarin, Spanish and Russian.Alan Merry works with his group (Safety and Quality <strong>of</strong> Practice) diligently on a low cost oximetry project in whichthe AAGBI’s and GE’s contributions are well appreciated.The Foundation group ably inspired by Phil Bridenbaugh explores uncharted roads to facilitate more WFSAeducation, publication and safety activities. Should you be aware <strong>of</strong> methods to bring them further: don’t hesitateto contact them at wfsahq@<strong>anaesthesiologists</strong>.orgWorking in the background and not so visible to you are Michael Dobson maintaining relationship with WorldHealth Organisation, Nick Newton with International Standardization Organisation and Joshe Rupreht with theWood Library Museum.The past year has been a melting pot <strong>of</strong> pride, admiration, awe and respect whilst at the same time counterbalanced by downright frustration, disbelief and stupefaction.Asked to join WFSA in an active role following my term as Secretary <strong>of</strong> the Dutch national anaesthesiologysociety for eight (8) years I wondered how much work could it be: meeting once in four (4) years and one hundredten (110) members – at the time? Colleagues such as Richard Walsh, John Moyers, David Wilkinson and JerryFontenot will join Mohamed Takrouri and Florian Nuevo in admitting a low assessment and appreciation <strong>of</strong> thework involved before we agreed! Some <strong>of</strong> us took less or other employment; others sacrificed their vacation,income and/or <strong>of</strong>f days to dedicate ourselves to the WFSA expectations.Subspecialties.Anaesthesia has seen breath-taking developments in special fields <strong>of</strong> interest and subspecialties. Each in its turn<strong>of</strong> course would like to organise specialty based meetings. Patronage is <strong>of</strong>ten requested from WFSA suggestingcertain standards. What is involved with WFSA patronage and how both parties can meet certain criteria – shouldreally be discussed at the upcoming World Congress and will be tabled at the first General Assembly on the 2 nd <strong>of</strong>March 2008, credentialing to open from 11.00 onwards.Some are concerned about the specialty breaking into a number <strong>of</strong> subspecialties. Worries were also raisedabout the interference with World Congresses and the effect on registrations <strong>of</strong> speakers (limited time available)and registrants, industry and trade support for local, international and global meetings. Meeting all anaesthetists<strong>of</strong> all subspecialties at the <strong>world</strong> congress and only once in four years has been such a cross-fertilisingexperience. Now anaesthesia seems to break apart into highly specialised subspecialties gathered insubspecialty regional, international and global meetings. At the same time, 80% <strong>of</strong> <strong>anaesthesiologists</strong> still practisethe full scope <strong>of</strong> anaesthesia and need updates/ continuing medical education on all topics by experts. In order togive anaesthetists an opportunity to listen to more than one subspecialty expert and unite at least once in 4 yearsthose in general practice and those who have as yet not decided what subspecialty to take, I propose to create anInternational Scientific Committee. All subspecialties will be invited to become a member or send a representativeto Cape Town. This ISC may help realise the organisers <strong>of</strong> future <strong>world</strong> congresses maintain the scientificstandards aimed for by the current and previous <strong>world</strong> congress organisers whilst at the same time giving thespecialty groups a voice in WFSA.Finally, the organisers <strong>of</strong> the World Congress in Cape Town (www.wca2008.com) deserve your full attention.They have aimed to bring us all together under a wonderful scientific umbrella in the so called rainbow nation. She is truly vital in the administration and organization <strong>of</strong> the myriad <strong>of</strong> WFSA activities and is a largeportion <strong>of</strong> the “glue” that holds the Federation together. The Secretariat continues to strive to fulfill itsresponsibilities to the WFSA and National Member Societies while managing expenses with frugality. Proposed WFSA ConstitutionA proposed revision <strong>of</strong> the WFSA Statutes and Bylaws, now termed the WFSA Constitution, prepared by theWFSA Committee on Statutes and Bylaws has been distributed to all National Societies. It is an attempt tosimplify the old document and toReflect current WFSA policy and procedure. A final draft will be distributed to the National Societies later thisyear in time for placement on the agenda for a vote during the General Assembly in Cape Town in March 2008.3. WCA 2008 - Cape Town, March 2-7The 14th World Congress <strong>of</strong> Anaesthesiologists (www.wca2008.com) boasts the <strong>world</strong>’s foremost educationalprogram for anesthesiologists. The World Congress Faculty will present over 320 lectures and discussionsessions covering a wide range <strong>of</strong> topics in anesthesia and related medical fields, as well as workshops in areas<strong>of</strong> technical expertise. The pr<strong>of</strong>essional development <strong>of</strong>ferings are created for each level <strong>of</strong> expertise – fromtrainees and residents to pioneering experts – allowing each to gain knowledge and enhance pr<strong>of</strong>essionalperformance, reputation and career.WCA 2008 <strong>of</strong>fers a multitude <strong>of</strong> pr<strong>of</strong>essional development <strong>of</strong>ferings crucial for all anesthesiologists and allieddoctors. Its objective is to be the most authoritative international anesthesia meeting and to set the standard <strong>of</strong>education in the pr<strong>of</strong>ession. Through the scientific sessions, social program, exhibits and satellite meetings thecongress will bring anesthesiologists from all over <strong>of</strong> the <strong>world</strong> together in the Rainbow Nation – the perfectsetting for the global anesthesia family.4. WFSA Committee ActivitiesThe work <strong>of</strong> the WFSA is carried out through its committees and working parties. There are “standingcommittees” on Education, Publications, Statutes & Bylaws, Finance, Safety & Quality <strong>of</strong> Practice and the WFSAFoundation. “Specialist committees” exist for Obstetric Anaesthesia, Paediatric Anaesthesia, Pain Relief, andResuscitation, Trauma & Intensive Care Medicine. Currently, there is a working party on <strong>world</strong>wide anesthesiaworkforce and a Primary Trauma Care Working Party. A complete description <strong>of</strong> committee activities is availableon the WFSA website. A few <strong>of</strong> the highlights from committee <strong>report</strong>s are presented below.Education Committee:The Chair <strong>of</strong> the Education Committee, Dr. Angela Enright, <strong>report</strong>s that the major focus <strong>of</strong> the educationcommittee is on developing training programs where young physicians from the developing <strong>world</strong> can acquireeither basic or advanced training in anesthesia. It is a strongly held belief that this training should be <strong>of</strong>fered asclose to home as possible so as to be relevant to the environment in which the trainee will eventually practice.A. Basic Anaesthesia Training ProgramsBangkok, Thailand -This program accommodates four trainees per annum. All the residents spend seven months in a universityhospital before rotating to a provincial hospital for three months to prepare them better for work at home. Theyspend their final month back at the university and then sit for their exit examination. Since 1996 the Bangkokprogram has trained anesthetists from the following countries: Mongolia 13, Lao PDR 12, Cambodia 10, Vietnam4, Myanmar 3 and Bhutan 2 for a total <strong>of</strong> 44. Almost all <strong>of</strong> these are continuing to practice anesthesia in theirhome countries. There has been a recent concentration on Mongolia in order to develop a sufficient cohort <strong>of</strong>5


anesthetists there. This has begun to pay dividends as the young, Bangkok-trained anesthetists are alreadymaking a huge difference. They have organized conferences, upgraded techniques and greatly changed the face<strong>of</strong> anesthesia in Mongolia.Cluj-Napoca, Romania -This centre has been growing steadily since its inception in 2002 and has focused on the training <strong>of</strong> anesthetistsfrom Moldova. The WFSA and the European Society <strong>of</strong> Anaesthesiology National Anaesthesia SocietiesCommittee (NASC) support the center. This allows for the training <strong>of</strong> 9 young anesthetists per year for 6 monthseach. Because the language <strong>of</strong> Moldova is Romanian, the trainees integrate very rapidly into the system rotatingthrough areas in anesthesia and also ICU. They learn many new procedures and have an opportunity to practicethem. They attend all teaching rounds and give presentations. They also have the opportunity to attend scientificmeetings such as the National Congress <strong>of</strong> the Romanian Society and also FEEA (Foundation for EuropeanEducation in Anaesthesiology) courses.B. Subspecialty Training - PediatricsSantiago, Chile -This was the first WFSA program in pediatric anesthesia. It has now trained 13 pediatric anesthetists for Centraland South America and is very highly regarded. Most trainees attend for 6 months but in the recent past severalhave stayed for one year. They have also trained one pediatric cardiac anesthetist for Venezuela.Vellore, India -This program began in 2005 with a trainee from Bellary, India. The second Fellow was from Bangladesh. Theyhave both returned home and are practicing successfully. Their third Fellow is from Afghanistan and hopefully thefourth will be from Myanmar. The need for anesthetists with pediatric expertise is great and Vellore provides anexcellent experience. One <strong>of</strong> the Fellows wrote: “I think I have learned more in Vellore, than in the last five years<strong>of</strong> my anesthesia practice”Cape Town, South Africa -This program also began in 2005. The first Fellow was from Nairobi, Kenya and has recently returned home aftera fantastically successful fellowship. During her year in Cape Town, she performed over 1000 pediatricanesthetics. Besides the clinical activity, she was exposed to research and to many educational activities. Sheachieved her first publication and poster presentation and was an invited speaker at the <strong>2006</strong> PediatricAnesthesia Congress for South Africa. Our second Fellow in this program is from Mbarara in Uganda.Tunis, Tunisia -This program continues to train young anesthetists from French speaking Africa in the art and science <strong>of</strong> pediatricanesthesia. Trainees have returned to their home countries <strong>of</strong> Cameroon, Niger, Ivory Coast, Senegal, Mali andRwanda.C. Subspecialty Training – ObstetricsMedellin, Colombia -This is a new program hosted by the Universidad Pontificia Bolivariana. It <strong>of</strong>fers 3 months <strong>of</strong> training in obstetricanesthesia to young anesthetists from South and Central America. The first Fellow was from Merida, Venezuelawhere she is a faculty member at Universidad de los Andes (Merida's public University). She had an excellent andvery busy three months in Medellin. The second Fellow, from Paraguay, will begin in February.D. Subspecialty Training - Intensive CareVellore, India -Building on the success <strong>of</strong> the pediatric anesthesia program, the faculty <strong>of</strong> Christian Medical College agreed towork with the WFSA to develop a Fellowship in intensive care. The first Fellow, who is from Nepal, has justbegun. The training is for one year but could be extended to two years if desired. This is the first WFSAFellowship in intensive care.E. Subspecialty TrainingWolfson Medical Centre, Holon, Israel -Starting in January 2007, 5 six-month subspecialty Fellowships are planned per year at Wolfson. There will beone position per country per year. The starting countries will be Bulgaria, Kenya, Moldova, Serbia and Slovakia.Choices <strong>of</strong> subspecialty will include cardiac, obstetric and pediatric anesthesia, pain management and intensivecare.6


F. Primary Trauma Care (PTC):Dr. Douglas Wilkinson, Chairman WFSA Working Committee on PTC, and Dr. Rob McDougall, Secretary PTCand Education Committee WFSA, <strong>report</strong> that in 1997 the first Primary Trauma Care (PTC) course was run in Fijiwith funds from the Education Committee <strong>of</strong> the WFSA. From this course, a comprehensive trauma programmeincorporating a 2-day course and 1 day instructor course has spread <strong>world</strong>wide.Since 1997, the PTC course has been taught in over 33 countries and PTC materials have been translated intoover eleven languages. The PTC manual has also been published by the WHO in their “Essential Surgical Care”manual and has extracts in the new Oxford Handbook <strong>of</strong> Tropical Medicine.This 2-day course is run in countries <strong>world</strong>wide to train doctors and nurses is the management <strong>of</strong> the severelyinjured patient. It is followed by a 1-day instructor course, which is taught with the educational aid <strong>of</strong> the PTCinstructor Manual and slides. These courses are run free to a country, and rely on generous sponsorship from theWFSA, ASA, and AAGBI, RCA and other organizations.Primary Trauma Care (PTC) maintains strong formal links with the WFSA, with Douglas Wilkinson heading up theWFSA Working Committee on PTC and via the Education Committee, through Rob McDougall. It becamenecessary for PTC to expand and the Primary Trauma Care Foundation Charity was formally established.Individual PTC programs, whilst initially run solely by anaesthetists are now administered by a variety <strong>of</strong> healthworkers including surgeons, emergency specialists, intensivists and nurses. This more accurately reflects thebackground <strong>of</strong> health workers who involve themselves in primary care <strong>of</strong> trauma victims.The PTC Trustees include Haydn Perndt, Michael Dobson, Frank Walters, John Beavis, Roy Fenner, RobMacDougall and Douglas WilkinsonThe PTC manuals, in many translations, can be found free on the PTC Foundation website, as well as the regularnewsletter and other information. This is on www.primarytraumacare.orgA detailed <strong>report</strong> covering the last 4 years <strong>of</strong> PTC activity will be available at the 2008 World Congress <strong>of</strong>Anaesthesiologists in Cape Town next year.G. Other educational activitiesA wide range <strong>of</strong> educational activities are supported including visiting teachers, refresher course lecturers,assistance with regional and national meetings, sponsorship <strong>of</strong> young anesthetists for extra training, assistancewith external examiners, equipment repair and maintenance courses and examination preparation. Since 2000these efforts have taken place in about 60 countries <strong>world</strong>wide.A new initiative was undertaken in <strong>2006</strong> in an effort to improve the teaching <strong>of</strong> anesthesia. The WFSA supportedtwo Teach the Teachers courses, one for Eastern Europe and the other for the Philippines. These were focusedon the younger faculty members who will be the future <strong>of</strong> the specialty.In all <strong>of</strong> its activities, the education committee coordinates and works with many other groups. National <strong>societies</strong><strong>of</strong> anesthesia, individual anesthetists, universities, hospitals, subspecialty <strong>societies</strong> and industry <strong>of</strong>fer directfinancial aid or assistance in kind which allow the WFSA to pursue its goals <strong>of</strong> improving anesthesia educationthroughout the <strong>world</strong>.Publications Committee:Chair, Dr. Iain Wilson, <strong>report</strong>s that the role <strong>of</strong> the Publications Committee is to supply effective educationalmaterials to those working in areas <strong>of</strong> the <strong>world</strong> where anesthesia publications are in short supply.The ideal anesthesia publication would be written in language that can be easily understood, address theparticular problems experienced by the reader at a practical level for their learning and clinical circumstances. Itshould be high quality and be either freely available or at least affordable. Currently, existing anesthesia texts<strong>of</strong>ten cost more than a month’s salary for many colleagues.A. Update in AnaesthesiaUpdate is a journal that was started in 1992 to provide an effective educational resource for anesthetists in thedeveloping <strong>world</strong> who did not have access to literature. The current editor Dr B M McCormick took up post in<strong>2006</strong>. The journal is now produced twice a year in a paper format in English, French, Chinese and Russian.Around 12,000 paper copies are printed and distributed free <strong>of</strong> charge.Initially Update covered many <strong>of</strong> the basic techniques used in anesthesia to provide a practical overview <strong>of</strong>anesthesia topics in poorer countries. Basic science articles have proved popular, particularly those written in anaccessible style. Update now includes a variety <strong>of</strong> other clinical topics such as ICU and pain.Update in Anaesthesia is available electronically in each language as well as in Spanish, and has been presentedboth in a CDROM format and also on the web at www.<strong>world</strong>anaesthesia.org and www.nda.ox.ac.uk/wfsa.Downloading is free <strong>of</strong> charge and there are no copyright concerns, preferring to ensure maximum availability.UK anesthetists read many <strong>of</strong> the articles as they began training in anesthesia.7


B. Anaesthesia Tutorial <strong>of</strong> the WeekWith increasing accessibility to email and the Internet, the concept <strong>of</strong> an anesthesia tutorial <strong>of</strong> the week (ATOTW)was developed in <strong>2006</strong>. It takes the form <strong>of</strong> an electronic tutorial, which is emailed out, free <strong>of</strong> charge around the<strong>world</strong>. Peer reviewed tutorials are provided free <strong>of</strong> charge and each one appears on www.<strong>world</strong>anaesthesia.organd www.anaesthesiauk.com. The archive contains many useful topics and is slowly building into an excellenton-line resource useful for both teachers and students.C. Book and Journal ExchangeFor some years the WFSA has encouraged exchange <strong>of</strong> books and journals between those with and thosewithout. This is coordinated by Carol Wilson (<strong>world</strong>anaesthesia@mac.com) who matches <strong>of</strong>fers <strong>of</strong> donations <strong>of</strong>books and journals with a database <strong>of</strong> recipients, which may be individuals or department libraries. A fewpractical points need taken into account for those considering the scheme. Books and journals should be inreasonable condition and not out <strong>of</strong> date (journals 3 years and books 5 years). Non-specialist books are best,although those dealing with pediatric or obstetric topics are very popular. The cost <strong>of</strong> posting these items is notinexpensive, and it is amazing how many <strong>of</strong>fers dry up at that point! The Committee is regularly <strong>of</strong>fered entirelibraries <strong>of</strong> old journals reflecting a career-long collection – these are rarely practical or cost effective to transport.Although occasional parcels go missing, despite all the drawbacks the pleasure that a package <strong>of</strong> books orjournals brings to an isolated colleague working without is enormous. The WFSA is grateful to all those whocontinue to support the scheme. Particular thanks are due to the Association <strong>of</strong> Anesthetists <strong>of</strong> Great Britain andIreland who have distributed more than 3000 anesthesia handbooks to colleagues in Africa.Global Oximetry Project (GO):The Global Oximetry Project (GO) arose from a resolution <strong>of</strong> the Safety and Quality <strong>of</strong> Practice Committee <strong>of</strong>WFSA at the 2004 World Congress Anaesthesiologists. It is collaboration between the World Federation <strong>of</strong>Societies <strong>of</strong> Anaesthesiologists, the Association <strong>of</strong> Anesthetists <strong>of</strong> Great Britain and Ireland, and GE Healthcare(the Founder Organizations).The spirit <strong>of</strong> this partnership is informal, with a focus improving patient safety by facilitating the provision andeffective use <strong>of</strong> pulse oximetry during anesthesia throughout the <strong>world</strong>, in particular in low and middle-incomeregions where the technology is <strong>of</strong>ten not available. The development <strong>of</strong> low cost, robust technology is a crucialpart <strong>of</strong> this initiative.In 2007, 4 Phase I sub-projects will take place, one each in India, the Philippines, Uganda and Vietnam,introducing pulse oximetry into clinical areas <strong>of</strong> several hospitals.John R. TREASURER’S REPORTfrom Richard G Walsh – 29 July 2007<strong>2006</strong> was a successful financial year in terms <strong>of</strong> overall income and expenses <strong>of</strong> the WFSA, and budgetedexpectations. This is very comforting as the middle year between World Congresses <strong>of</strong>ten results in a financialdownturn, particularly as it is marked by the additional expenses <strong>of</strong> holding a major meeting <strong>of</strong> the ExecutiveCommittee. This <strong>report</strong> includes an overall summary <strong>of</strong> WFSA finances as presented in the audited financialstatements <strong>of</strong> <strong>2006</strong>, some specific points and comments on individual items <strong>of</strong> income and expense, and asummary <strong>of</strong> the WFSA investment portfolio.1. Audited Financial Statements <strong>2006</strong>Statements for the years 2004, 2005 and <strong>2006</strong> will shortly to be posted on the WFSA website and all readers <strong>of</strong>this Newsletter are encouraged to view these financial statements. The statements are in accordance with strictauditing guidelines for acceptance in the USA, where the Federation is incorporated as a tax-free and charitableorganisation. The statements for each year include figures from the previous year as a comparison. The required<strong>report</strong>ing style <strong>of</strong>ten makes it quite difficult to fully comprehend the figures presented and therefore an overallsummary for <strong>2006</strong> is presented as follows.During <strong>2006</strong>, overall income was about $480,000 and this was similar to that in 2005. However, there was a largeshift in income sources, as 2005 saw significant income from a financial surplus <strong>of</strong> the 2004 World Congress(shown as Donations), while <strong>2006</strong> saw increased income from dues (subscriptions) paid to the WFSA by itsmember <strong>societies</strong> around the <strong>world</strong>. On the expenses side during <strong>2006</strong>, administration costs <strong>of</strong> the WFSA werestable at about $180,000 and expenditure on Education and Publication projects around the <strong>world</strong> reached$214,000 – a record for the Federation. The value <strong>of</strong> WFSA Investments remained stable at about $2.6 millionand these continue to provide a very significant proportion <strong>of</strong> WFSA <strong>annual</strong> income, the others being nationalsociety subscriptions for WFSA membership and any surplus due to the WFSA from World Congresses andregional congresses. The net effect <strong>of</strong> overall income and overall expenses was a bottom line deficit <strong>of</strong> $20,000for <strong>2006</strong> – significantly far less than the deficit <strong>of</strong> $148,000 originally budgeted for <strong>2006</strong>.2. Specific Income & Expense Items in <strong>2006</strong>The WFSA runs it internal finances with Management Accounts and with Budgets which respectively <strong>report</strong> oncurrent income and expenditure, and projected income and expenses for at least the next five years. They areused to guide the Executive Committee and Executive Management Group for responsible management <strong>of</strong> WFSAfinances. These accounts and budgets are discussed and varied at regular periods between World Congressesat which all financial <strong>report</strong>s are presented. The result <strong>of</strong> each year’s Management Accounts is reflected in theAudited Financial Statements.With respect to the <strong>2006</strong> Audited Financial Statements, the following special points are made:INCOME• Subscription income for <strong>2006</strong> has increased dramatically following the slow response in 2005 to asignificant rise in national member dues as agreed at the 2004 Paris World Congress. National<strong>societies</strong> responded favourably to recognition that WFSA membership fees had not increased inthe 24 years prior to 2004.• Interest and other income from investments increased in <strong>2006</strong> and this reflects excellentpr<strong>of</strong>essional advice and management by the WFSA Financial Advisor, Mr Francis Wirgman.• Pr<strong>of</strong>it on investments (realised) was encouraging for <strong>2006</strong>, maintaining the progress establishedin 2004 and again reflecting superb management via Francis Wirgman.• WFSA Foundation raised $47,000 mostly in the UK and USA with a much higher budgeted target.The remainder <strong>of</strong> this item was income from the Glostavent project in the UK.EXPENDITURE• Secretariat expenditure was 15% over budget and this was all due to the increased demands <strong>of</strong>holding an Executive Committee meeting in November <strong>2006</strong>. The enormous efforts <strong>of</strong> Ms RuthHooper, the WFSA Administrator in London, deserve great praise.9


• Officers & Executive Committee meetings, including travel, reflects the costs <strong>of</strong> the <strong>2006</strong>Singapore Executive Committee meeting and normal costs <strong>of</strong> an Executive Committeeperforming its duties with minimal expenditure.• Education & Publication Committees, administering expenditure on WFSA international projects(the Treasurer’s favourite expenditure!), were both on budget with a total expenditure <strong>of</strong>$214,000. This reflects excellent management by each committee Chairperson.3. Investment PortfolioThe WFSA Investment Portfolio is managed by an investment firm in New York, USA. All investments are held inthe USA because the WFSA is incorporated only in the USA, where it receives tax-free status as an endorsedcharitable organisation. Mr Francis Wirgman, Financial Advisor to the WFSA, receives detailed monthly <strong>report</strong>son all transactions undertaken by the WFSA appointed fund managers. Progress summary <strong>report</strong>s are providedevery 3 months to the Treasurer and Deputy Treasurer and further <strong>report</strong>s are forwarded at regular intervals tothe Executive Management Group and the Executive Committee.Certain important principles are maintained in management <strong>of</strong> the investment portfolio. The first principle is thatthe Executive Committee has agreed that the value <strong>of</strong> the portfolio should not fall below $2 million in order toensure a continuing income for its Education and Publications activities. The second principle is to recognise thatunrealised losses or gains in any single year reflect the fickle nature <strong>of</strong> the investment market and cannot justifyany immediate change in budgeting for coming years. The third principle is that changes in the value <strong>of</strong> theportfolio do not necessarily result in proportional changes to WFSA income through interest, dividends, etc. Thefinal principle is that a diversified portfolio is maintained <strong>of</strong> carefully selected and monitored equity and fixedinterest investments to balance risk and reward.<strong>2006</strong> saw a small improvement in the market value <strong>of</strong> WFSA equities stock and improved interest and investmentincome. Market value <strong>of</strong> the WFSA investment portfolio at 31 December <strong>2006</strong> was over $2.6 million, well abovethe lower limit <strong>of</strong> $2 million set by the Executive Committee. However, WFSA expenditure and income budgetswill significantly change over the next year, mainly due to expenses associated with meetings <strong>of</strong> the ExecutiveCommittee and the General Assemblies <strong>of</strong> the WFSA at the World Congress in Capetown in March 2008, anddue to income uncertainties related to any surplus generated from the 2008 World Congress. It would, therefore,be unwise to alter investment strategies until at least mid-2008.4. ConclusionThe above <strong>report</strong> should be read in conjunction with the WFSA audited financial statements <strong>of</strong> <strong>2006</strong>, which willappear on the WFSA website in the near future. This <strong>report</strong> reflects a stable and healthy state <strong>of</strong> WFSA finances.WFSA members can be reassured that the finances are managed on a daily basis, that progress is carefullymonitored throughout each year, that financial policies and strategies are reviewed regularly by the ExecutiveManagement Group, and that regular finance <strong>report</strong>s are sent to the Executive Committee for discussion andreview.Finally, the outstanding efforts and contributions <strong>of</strong> Mrs Ruth Hooper in the WFSA London Headquarters and MrFrancis Wirgman, Financial Advisor to the WFSA, must be acknowledged in their highly successful management<strong>of</strong> WFSA finances. IN MEMORIAMDr. John M. ZorabPast President 1988-1992It was with sadness that we heard <strong>of</strong> the passing <strong>of</strong> John Zorab recently. He was one <strong>of</strong> the greatest contributorsto international anaesthesia and the World Federation <strong>of</strong> Societies <strong>of</strong> Anaesthesiologists.John trained in anaesthesia with some <strong>of</strong> the greats <strong>of</strong> English anaesthesia (Sir Ge<strong>of</strong>frey Organe and Cyril Scurrat Westminster, Phillip Helliwell at Guys and Patrick Shackleton at Southhampton) before becoming a dynamicconsultant at Frenchay Hospital where he shared a small <strong>of</strong>fice with Peter Baskett and together they contributedto raising the standards <strong>of</strong> anaesthesia in Bristol. Latterly due to the high regard in which he was held there hebecame increasingly involved in the hospital's administration. John Zorab served the AAGBI on their Council andas Secretary, was on the Board <strong>of</strong> the Faculty <strong>of</strong> Anaesthetists, Royal College <strong>of</strong> Surgeons. Internationally hespent 6 months in Vietnam after the war and he also became involved in the European Regional Section <strong>of</strong> WFSABecoming Secretary General <strong>of</strong> the 1982 European Congress held in London and later became Chairman <strong>of</strong> ERSJohn Zorab was one <strong>of</strong> the founders <strong>of</strong> the European Academy and for a long time was involved in its Diplomaexaminations. He was a man <strong>of</strong> vision and an independent thinker. He served as a very progressive secretary <strong>of</strong>the WFSA. He produced a lecture series <strong>of</strong> books twice a year for four years, put forward the idea <strong>of</strong> Scholarshipsfor Eastern Block anaesthetitsts to visit a centre in the West for 3 months; several <strong>of</strong> the recipients becameleaders in their countries and served WFSA. Israel supplemented this programme enabling several Roumaniansto go there and later others went from Bulgaria and Moldova as well. This programme was magnificentlyorganised by Gaby Gurman for many years. He introduced the concept <strong>of</strong> WFSA courses, the first being held inNairobi. During the years 1988-92 when John Zorab was President <strong>of</strong> WFSA, Kester Brown Chairman <strong>of</strong> theEducation Committee with the help <strong>of</strong> Philippe Scherereel, Roger Eltringham and others our EducationCommittee sent teachers from 23 countries to 50 countries. This has continued and is a foremost contributionfrom the WFSA to the National Societies, along with the training centres that have developed and Update inAnaesthesia.John did not stop then. Despite having two cardiac arrests he contributed to the 'History <strong>of</strong> the First Fifty years <strong>of</strong>WFSA', a volume all committee members should read so that they understand the background <strong>of</strong> theorganization. Then he produced an illustrated CD about all the World Congresses, which he presented in Paris. Itis an invaluable archive <strong>of</strong> the history <strong>of</strong> all the key players <strong>of</strong> the WFSA and the Congresses over the past halfcentury.John was supported by his wife Shirley who acted as his Secretary while he held the busy executive positions <strong>of</strong>Secretary and President. Anaesthetists owe her much gratitude for all she did before the Federation had an <strong>of</strong>fice.Thank you, Shirley.John's passing saddens us but he led such a full life with so many worthwhile achievements that one cannot helpappreciating it and all he did for our specialty.Kester BrownImmediate Past President WFSADr Douglas D C HowatDouglas Howat was born on the 10 th January 1920 in the Scottish borders and brought up in London, where hisfather was a general practitioner. He studied medicine at King’s College, London, qualifying in 1943. Here he methis future wife Joan who was also a student. After service in the RAF he eventually became consultantanaesthetist to St George’s and the Royal Masonic Hospitals in London.He served on the Council <strong>of</strong> the Association <strong>of</strong> Anaesthetists <strong>of</strong> Great Britain and Ireland (AAGBI) and when in1969 a Treasurer was due to be elected, someone was inspired to say ‘let’s make Dr-and-Mrs-Howat ourTreasurer’. He became Treasurer, with Joan’s help for she was always as much a part <strong>of</strong> his pr<strong>of</strong>essional andpublic life as <strong>of</strong> his family life. Both contributed together.Later Douglas was elected to the Board <strong>of</strong> the Faculty <strong>of</strong> Anaesthetists where he served with competence onnumerous committees, as examiner for the Fellowship, and delivering the Frederick Hewitt lecture in 1977. WhenVice-Dean he breathed new life into what was then a rather vague post, whilst his contribution to surgical affairswas so effective that in 1984 the surgeons elected him FRCS. He was elected Honorary Member <strong>of</strong> AAGBI in1986.11


Douglas soon became involved in international affairs. He had an ability to establish rapport with all sorts <strong>of</strong>people and where diplomacy was needed, he was asked to go. As early as 1966 he was one <strong>of</strong> the UKrepresentatives considering the implications for medicine <strong>of</strong> Britain joining the European Economic Community.Not only his linguistic skills but even more his wise counsel was immensely valuable When the UK eventuallyjoined in 1973 he continued to represent British anaesthesia on the Council <strong>of</strong> the UEMS and chaired itsAnaesthetic Monospecialist Committee. From 1976-1980 he served as Chairman , and later Vice–President <strong>of</strong> theExecutive Committee <strong>of</strong> the World Federation <strong>of</strong> Anaesthesiologists, and also chaired its European Section. Hewas involved too in the founding <strong>of</strong> the European Academy. During this period links with our overseas colleagueswere notably strengthened and anaesthesia was established as one <strong>of</strong> the leaders amongst the medicalspecialties.In his home country Douglas was President <strong>of</strong> the Royal Society <strong>of</strong> Medicine’s Section <strong>of</strong> Anaesthesia and <strong>of</strong> theHistory <strong>of</strong> Anaesthesia Society. During his retirement he continued to pursue studies into the history <strong>of</strong> medicine,attending pr<strong>of</strong>essional meetings and <strong>report</strong>ing his researches in a highly entertaining way.St George’s is the hospital where John Hunter worked as a surgeon over 200 years ago. It is tempting to suggestthat they might have got on well together, John impatient, demanding and occasionally irascible, Douglas workingin an unobtrusive yet effective way, never losing his sense <strong>of</strong> humour and always just as willing to carry out themundane chores as the more prestigious ones. Although a national and international figure, he never forgot thatthe prime responsibility <strong>of</strong> a clinician is to serve his patients with skill and consideration, and to support hissurgeons and his trainees.(Adapted from text by Aileen K. Adams)Dr Richard JackDr Jack was actively involved with the WFSA in organizing the first refresher course in Moscow, working with thelate Dr John Zorab and with Dr Elena Damir, then President <strong>of</strong> the Russian Federation <strong>of</strong> Anaesthesiologists andReanimatologists. Dr Jack also organized courses in St Petersburg and, as a result <strong>of</strong> his contacts, newavenues were opened for courses in the Baltic region, and new working relationship developed with anaesthetistsfrom Eastern Europe. He was also honoured with a diploma by the Russian Federation for his achievements inthat region. Friends and family have given donations to WFSA educational projects in Eastern Europe, inmemory <strong>of</strong> Dr Jack.Dr Jack was highly respected throughout the anaesthesiology community and pr<strong>of</strong>ession, and his good workschanged the lives <strong>of</strong> many.12

Some suggested that the<strong>annual</strong> subscriptions should be based on the financial capabilities <strong>of</strong> the individual national <strong>societies</strong>.Others suggested that the more affluent national <strong>societies</strong> could assist their neighbouring nationalmember <strong>societies</strong> who may encounter difficulties in paying the WFSA <strong>annual</strong> subscriptions.On the proposal <strong>of</strong> the President, the General Assembly agreed to defer the decision on the motions tothe 2 nd General Assembly.A.Meursing appealed to the national <strong>societies</strong> to declare their true membership numbers in their <strong>annual</strong>survey to WFSA.11. Report <strong>of</strong> the WFSA Committee on Statutes & BylawsDavid Wilkinson presented his <strong>report</strong>. The General Assembly initially accepted the changes as proposedin a document circulated to national <strong>societies</strong> six months previously. However, comments were receivedfrom the floor that what was accepted was not clear to the delegates. The General Assembly wasreassured that an attachment will be made available at the 2 nd General Assembly specifying the acceptedchanges.The Executive Committee has approved <strong>of</strong> motions to amend the Statutes and Bylaws. According to Art3.13 (Amendment <strong>of</strong> the Statutes), the General Assembly needs to establish a Reference Committee toconsider these changes.The General Assembly accepted the creation <strong>of</strong> a Constitution to replace the current Statutes andBylaws. The Reference Committee on Statutes and Bylaws is composed <strong>of</strong> the Committee on Statutesand Bylaws, Honorary Secretary and any delegate who would like to express his/her opinion.
