Audit Requirements All account holders must use theFMCSA-provided consent language Consent must include driver name andsignature (hand-written or electronic) Consent must include the date Consent date must be on or prior to theday the account holder requested thedriver’s PSP record12

Submitting Audit Information Consent forms may be uploaded into thePSP system or emailed toPSPaudit@egov.com Account holders receive confirmationemail after the audit document(s) arereceived13

Audit Penalties Account holders who do not comply withaudits are subject to penalties Penalties severities vary and includewarnings, suspensions, and terminations Penalty severity increases with multipleaudit failures Account holders return to “goodstanding” after 12 months of acceptableaudit activity14
