These instructions are meantto assist you in completing the application for nursing licensure (you may complete the applicationwithin a few weeks of graduation).1. Review your SAIN and confirm that the requirements for graduation have been met.*Make sure you have 30 residency credits at SCCC*2. Apply for Graduation/Diploma on the SCCC Website (Note The Deadlines):Log onto “MySCCC” and locate the “Apply for Graduation” link.Graduation Date on DiplomaJanuaryJuneAugustDeadline for Filing <strong>Application</strong> for GraduationDecember 1 stApril 1 stJuly 1 stStudents completing the curriculum mid-Semester please note: Your informationwill be submitted to the NYS Education Dept. at the end of the semester.3. Access the licensing packet online.The licensing packet is available at http://www.op.nysed.gov/prof/nurse/nurseforms.htmRead the entire licensing application packet thoroughly. The process is spelled out simplyand completely. General requirements and fees are listed on this site. You may wish toprint the booklet out for your reference but DO NOT COMPLETE THE FORMS IN THEBOOKLET. The correct forms are on the SON webpage, under “forms”.4. Training in Infection Control and Child Abuse is included in the school’s curriculum so noother documentation is required.5. Apply for the licensing exam.Complete Form 1NYS – <strong>Application</strong> for licensure [See link on nursing web page].Follow instructions carefully and include fee. Be sure to provide your Social SecurityNumber on both the licensure application and the examination application as this willenable the NYS Education Department to speed your application to test. You may submitthis form within a few weeks of graduation.6. Complete the Data card given in class, include your name exactly as you want it to appearon your license, include Social Security number, date of birth, phone number and address.This information will be submitted to the Nursing Department for inclusion on the list tobe sent to the NYS Education Department, Office of the Professions, New York StateBoard of Nursing.1


7. Form 2PG – Verification of Education is sent by the Nursing Department aftergraduation with a list of names of those permitted to take the NCLEX exam (This can takeapproximately 4 weeks). You do not have to contact the school to complete this form.However, if you still need to take a course, such as a summer course, you must contactyour campus Nursing Department when you have completed all the requirements.8. After submitting your completed application to NYS Education department, apply on lineto Pearson VUE to take the RN Examination at www.vue.com/nclexVERY IMPORTANT!!! When you apply to Pearson VUE, you MUST use your SocialSecurity Number (even though Pearson VUE indicates that this is optional) because notincluding a Social Security Number significantly delays testing approval. The programcode for Grant Campus is 03-433. Program code for Selden Campus is 03-438. You cansubmit this application and the fee, anytime within weeks of graduation. If needed, theRequest for Reasonable Testing Accommodations form can be downloaded at:http://www.op.nysed.gov/documents/pls1ra.pdf9. The Office of Professions, NY State Board of Nursing will contact the NCLEX – RNtesting center and you will be sent an authorization to test (ATT) letter with instructions onselecting a testing date.10. Limited permit Form 5NYS – <strong>Application</strong> for Limited Permit [See link on nursing webpage] is only completed if an institution has hired you and is willing to sign your permit.Some institutions require a letter from the school stating you have met the requirements.The Academic Chair on your campus will prepare this letter and it will be made availableat the end of the semester.11. Students are encouraged to go to:https://www.ncsbn.org/2012_NCLEX_Candidate_Bulletin.pdffor information concerning requirements.You can also find useful recourses at Pearson VUE Websitehttp://www.pearsonvue.com/nclex/ .12. Name: When you sit for the exam, you must bring in 1 valid primary photo ID withsignature (drivers license or passport for example). The name on the authorization to test(ATT) and photo ID must match13. It is the students’ responsibility to notify the nursing department on the campus where theytook their last nursing course to be included on the list to be sent to the State EducationDepartment in Albany.14. Don’t wait too long to take the NCLEX exam! Studies show that the best scores areobtained within 6 months of graduation.
