<strong>Tulsa</strong> <strong>City</strong>-<strong>County</strong> <strong>Library</strong> PoliciesThere is a fee for replacing a lost card (See appendix). If the cardholder wants to replace the lost orstolen card, this fee is assessed at the time the card is reported lost or stolen, or upon request fromcustomer.Report lost or stolen cards by phone, in person at any <strong>Library</strong> location, or by email.<strong>Library</strong> Notices The <strong>Tulsa</strong> <strong>City</strong>-<strong>County</strong> <strong>Library</strong> provides a notification service for held items, overdue materials andother service-specific reasons. Notices are available via email or telephone. The method by whichyou receive notices is chosen during application for a card. It is the responsibility of the borrower tonotify the library if any of their contact information changes. In the case of cardholders who receive their notices by e-mail, it is the cardholder’s responsibility tomake sure that e-mails sent from addresses with @tulsalibrary.org in the sender information beallowed through any filters they may have in place on their Inbox. <strong>Library</strong> notices are sent as a courtesy. Failure to receive a notice does not absolve the borrower fromany fines or fees attached to their customer record.Confidentiality of Customer RecordsThe <strong>Tulsa</strong> <strong>City</strong>-<strong>County</strong> <strong>Library</strong> supports and complies with applicable Oklahoma Law (including, withoutlimitation, the Oklahoma Open Records Act, OKLA. STAT. tit. 51, §§ 24A.1 et seq., and theOklahoma <strong>Library</strong> Code, OKLA. STAT. tit. 65, § 1-105) regarding confidentiality and disclosure oflibrary records. All library records relating to an individual customer’s use of the library and its resourcesare confidential. These records may be consulted by library staff and released to the library’s contractorsand consultants for use in the course of carrying out library operations, or for conducting surveys andevaluations of the library’s delivery of services, but will not be disclosed to others except upon request orconsent of the library user or pursuant to court order. As the legal custodian of records, the <strong>Tulsa</strong> <strong>City</strong>-<strong>County</strong> <strong>Library</strong> CEO is responsible for responding to any requests or information about a library user. Ifthe CEO is absent or unavailable, a director is designated to respond to any request for library records orinformation about a library user.The circulation and registration records of the <strong>Tulsa</strong> <strong>City</strong>-<strong>County</strong> <strong>Library</strong> shall not be made available toany third party nor any law enforcement agency of a local, state, or federal government except when acourt order, issued by or through a court of competent jurisdiction, is presented to the library by the lawenforcement agency or other person seeking the records. In accordance with the library’s securitycameras policy, digital video images of the library’s external areas or indoor common areas that do notdisclose or lead to the disclosure of any specific library resources or materials loaned to or used by alibrary customer will be released by the CEO, or authorized library executive team member, upon requestof a law enforcement agency or member of the public.The <strong>Tulsa</strong> <strong>City</strong>-<strong>County</strong> <strong>Library</strong> keeps records of paid and unpaid fines or bills and checked-out materials.Customers are in good standing when checked-out materials are returned and any applicable fines or billsare paid in full. Customers with unpaid fines or bills will not be allowed to check out any library materials.After a customer’s fine or bill is paid in full, if the customer’s fines or bills were excessive, it is within thelibrary’s discretion to require a security deposit from the customer before allowing the customer to checkout any library materials. No information regarding checked-out materials returned by the date due iskept on the customer’s record. Log-in data for computer usage is purged after one month.No library employee or volunteer may release library records or reveal information about a library user toany third party, including a law enforcement agency, unless authorized to do so by the <strong>Library</strong> CEO, or adirector in the absence of the CEO.4 - 5
