Professional Development Early Warning ReportAppendix JAny Carson-Newman faculty member may complete an Early Warning Report for any student who is admitted tothe Teacher Education Program or for one who is planning to enter the program. The purpose of the warning is tocorrect problems before they become major stumbling blocks to success.1. The faculty member completes the report, discusses it with the student, has the student sign and date theform, and files the completed report with the Admissions and Services Advisor, who will get a copy tothe student’s academic advisor, and file a copy in the student’s permanent record.2. If the Admissions and Services Advisor accumulates two early warnings on a particular student, a copyof those forms will be sent to the Teacher Education Department Chair who will schedule a conferencewith the student to discuss the disposition(s). The chair will include referring faculty members in theconference with the student.3. The student will receive counseling and advice from the Teacher Education Department Chair. A copyof this form will go to the academic advisor and Admissions and Services Advisor.4. If a student fails to satisfactorily follow through on recommendations, the student will not be admitted tothe Teacher Education Program and will not be admitted to the student teaching semester.Student Name: ____________________________ ID Number: ____________________________Statement of Concern:Evidence to Support the Concern:Recommended Course of Action:Faculty Member (print): _________________ Signature: ___________________________________Student Signature: ________________________________ Date: _____________________________Advisor (print): ____________________________ Signature: ________________________________Date of Conference: ___________Student Signature: ______________________________________The purpose of the Early Warning Report is to alert a student that he/she needs to improve in an area of professionaldevelopment. The intent is to be corrective rather than punitive; however, a series of these reports can, at any point, result in astudent not being able to enter or continue in the Teacher Education Program. ―Early‖ means the student has time forremediation. However, when the Admissions and Services Advisor has two reports on a student, that student will be broughtto the attention of the department chairperson for action. The reports are normally related to the Dispositions for Pre-ServiceTeacher Candidates.
