Implicated hoists should be quarantined pendingcorrective action.BACKGROUND1. A report has been received of a fatality involving an Inter Care “All-in-One” patient hoist. Thespreader bar assembly became detached while the patient was being transferred from a chair to a bed.Investigation found that the bush attached to the eyebolt had worn away at the interface with thespreader bar assembly.2. “All-in-One” hoists are manufactured by Inter Care Scandanavia A/S, Denmark and were distributed inthe UK by Care & Mobility Ltd prior to 1997. Since 1997 they have been distributed by Talley GroupLtd.3. The hoists affected by this problem were manufactured between March 1996 and August 1999 andhave serial numbers in the range 96/03/126 to 99/06/282 inclusive. Figure 1 illustrates the location ofthe serial number.ACTION4. This notice should be brought to the attention of all appropriate managers, staff and users.5. All Inter Care “All-in-One” patient hoists should be checked to identify those with serial numbers inthe range 96/03/126 to 99/06/282 inclusive. These should be withdrawn from use immediately andplaced in quarantine.6.
