Registration Policies <strong>and</strong> ProceduresInstructions:1. Complete all portions of the Registration Form <strong>and</strong> mail it to: <strong>IACLEA</strong> Annual <strong>Conference</strong>, 342 NorthMain Street, West Hartford, CT 06117-2507. A check payable to <strong>IACLEA</strong> Annual <strong>Conference</strong>, orcopy of approved purchase order or credit card authorization for registration fees (or any combination)MUST accompany the form. Correct fees must be received at <strong>IACLEA</strong> for an attendee to beregistered. All registration fees must be paid in U.S. dollars.2. Your name <strong>and</strong> institution will appear exactly as you indicate on this form.3. Print or type all information.4. Make copies of this form for any additional persons who wish to register. Send a separate form foreach registration.5. Please copy your completed Registration Form for your records.6. To register by fax, call (860) 586-7550.Registration Policies:1. Only individuals registered <strong>and</strong> badged may attend conference events. Note spouse/guest name onthe Delegate Registration Form if an additional badge is requested. Children need badges so includetheir names.2. No registration may be processed without payment or receipt of a purchase order. For registrationpurposes, an institution’s purchase order will be treated as cash.3. Payment or purchase order must accompany the Registration Form in order to receive the earlyregistration discount.4. All registration cancellations must be sent in writing to <strong>IACLEA</strong> Headquarters, 342 North MainStreet, West Hartford, CT 06117-2507.Any registration cancellation postmarked (or emailed) prior to June 10, 2005 will incur a $50 cancellationfee.Any registration cancellations postmarked (or emailed) after June 10 will forfeit all registration fees.Forfeited fees cannot be applied to future meetings.Substitutions from within the same institution are permitted at no additional cost. All substitutionsmust be made in writing <strong>and</strong> accompanied by a completed <strong>IACLEA</strong> <strong>Conference</strong> Registration form.5. Checks or purchase orders received by <strong>IACLEA</strong> without registration forms are automatically mailedback to your accounting office.Housing Policy:Only those registered for the conference, or special guests recognized by <strong>IACLEA</strong>’s office, will be eligiblefor hotel rooms at the special conference rate.
