Instructions: Only current FSR members are eligible to vote. All categories of members may vote
(i.e., regular, affiliate, associate members). Clearly select one for each category. Do not enter a
“Write in” name unless you are sure the person wants to run for and participate in the FSR elected
position – declinations after the election are awkward. Print names of “write in” candidates so they are
legible. FSR Bylaws require you to sign the ballot at the bottom for it to be valid. You may use photocopies
of this form as long as you only vote once and an original signature is made at the bottom.
