Thestatement must also list any failure or refusal to complete a contract, includingdetails and dates.4. List containing at least 3 professional references who can verify the respondent’sprevious experience and outcomes.List should include at a minimum: name of reference, title, company, phonenumber, address, and email address as well as relationship to the project. If lettersof reference are submitted, current contact information for each reference is stillrequired.8
