<strong>Clotrimazole</strong><strong>Introduction</strong>: <strong>Clotrimazole</strong> <strong>is</strong> <strong>an</strong> <strong>an</strong>tifungal <strong>medication</strong> commonly used in thetreatment of fungal infections of both hum<strong>an</strong>s <strong>an</strong>d <strong>an</strong>imals such as vaginal yeastinfections <strong>an</strong>d ringworm. It <strong>is</strong> also used to treat athlete's foot <strong>an</strong>d jock itch.Mech<strong>an</strong><strong>is</strong>m of action: <strong>Clotrimazole</strong> interacts with 14-α demethylase, acytochrome P-450 enzyme that converts l<strong>an</strong>osterol to ergosterol, <strong>an</strong> essentialcomponent of the yeast membr<strong>an</strong>e. In th<strong>is</strong> way, clotrimazole inhibits ergosterolsynthes<strong>is</strong>, resulting in increased cellular permeability. <strong>Clotrimazole</strong> may alsoinhibit endogenous respiration, interact with membr<strong>an</strong>e phospholipids, inhibit thetr<strong>an</strong>sformation of yeasts to mycelial forms <strong>an</strong>d the uptake of purine, impairtriglyceride <strong>an</strong>d/or phospholipid biosynthes<strong>is</strong>, <strong>an</strong>d inhibit the movement ofcalcium <strong>an</strong>d potassium ions across the cell membr<strong>an</strong>e by blocking the iontr<strong>an</strong>sport pathway known as the Gardos ch<strong>an</strong>nel.Pharmacology: <strong>Clotrimazole</strong>, <strong>an</strong> imidazole derivative with a broad spectrum of<strong>an</strong>timycotic activity, inhibits biosynthes<strong>is</strong> of the sterol ergostol, <strong>an</strong> import<strong>an</strong>tcomponent of fungal cell membr<strong>an</strong>es. Its action leads to increased membr<strong>an</strong>epermeability <strong>an</strong>d apparent d<strong>is</strong>ruption of enzyme systems bound to themembr<strong>an</strong>e.Indications: <strong>Clotrimazole</strong> topical solution <strong>is</strong> indicated for the topical treatment ofc<strong>an</strong>didias<strong>is</strong> due to C<strong>an</strong>didaAlbic<strong>an</strong>s <strong>an</strong>d tinea versicolor due to Malassezia furfur.<strong>Clotrimazole</strong> <strong>is</strong> also available as a nonprescription item which <strong>is</strong> indicated for thetopical treatment of the following dermal infections: tinea ped<strong>is</strong>, tinea crur<strong>is</strong>, <strong>an</strong>dtinea corpor<strong>is</strong> duetoTrichophyton rubrum, Trichophyton mentagrophytes, Epidermophyton floccosum, <strong>an</strong>dMicrosporum c<strong>an</strong><strong>is</strong>.Dosage: Gently massage sufficient clotrimazole topical solution into the affected<strong>an</strong>d surrounding skin areas twice a day, in the morning <strong>an</strong>d evening.Clinical improvement, with relief of pruritus, usually occurs within the first week oftreatment with clotrimazole topical solution. If the patient shows no clinicalimprovement after four weeks of treatment with clotrimazole topical solution, thediagnos<strong>is</strong> should be reviewed.Side effects: The drug has certain side effects that c<strong>an</strong> affect individuals indifferent ways. The following are some of the side effects that are oftenassociated with the drug:• Burning sensation• Itching• Local mild irritation• HypersensitivityThere <strong>is</strong> no evidence of r<strong>is</strong>k to the developing baby during pregn<strong>an</strong>cy or r<strong>is</strong>k tolactating women.


Precautions:General: If irritation or sensitivity develops with the use of clotrimazole, treatmentshould be d<strong>is</strong>continued <strong>an</strong>d appropriate therapy instituted.Pregn<strong>an</strong>cy: Pregn<strong>an</strong>cy Category B: th<strong>is</strong> drug should be used only if clearlyindicated during the first trimester of pregn<strong>an</strong>cy.Nursing Mothers: It <strong>is</strong> not known whether th<strong>is</strong> drug <strong>is</strong> excreted in hum<strong>an</strong> milk.Because m<strong>an</strong>y drugs are excreted in hum<strong>an</strong> milk, caution should be exerc<strong>is</strong>edwhen clotrimazole <strong>is</strong> used by a nursing wom<strong>an</strong>.Pediatric Use: Safety <strong>an</strong>d effectiveness in pediatric patients have beenestabl<strong>is</strong>hed for clotrimazole when used as indicated <strong>an</strong>d in the recommendeddosage.Contraindications: <strong>Clotrimazole</strong> topical solution <strong>is</strong> contraindicated in individualswho have shown hypersensitivity to <strong>an</strong>y of their components.How Supplied: Customized as per request.
