Choice of superannuation fundIf you would like to choose Portfoliofocus – Premium Retirement Service (a fund constituted under the MLC Master Plan Trust)(‘the fund’) as your fund for superannuation guarantee contributions, then give your employer this letter and the “Super Choice –Fund Nomination Form” (overleaf) which has the information they need which includes:• details of your chosen fund• confirmation that the fund is a complying fund• confirmation that the fund accepts employer contributions• details about how your employer can make contributions to the fundPlease be aware that some employees may not be able to choose their own superannuation fund.
