PART A: GENERAL INFORMATIONSAFETY GEAR1. Students undertaking workshop training are required to wear the followings:Safety shoes with n<strong>on</strong>-slip soles and steel toecaps to reduce the risk of being hit by fallingobject or c<strong>on</strong>tact with sharp object or exposure tochemical or sparks.L<strong>on</strong>g trousers to reduce the risk of c<strong>on</strong>tact withsharp object or exposure to chemical or sparks.2. Students should bring their own safety shoes and l<strong>on</strong>g trousers.3. While our Electr<strong>on</strong>ics Workshop is exempted from wearing the safety shoes, robust shoes without the exposureof toes, instep and heels are required to be put <strong>on</strong> during the training in this workshop.4. In some <strong>IC</strong> workshops you will be issued temporarily with pers<strong>on</strong>al protective gear such as gloves, goggles, facevisors, etc., the wearing of which will be compulsory if required by the instructors.5. Students should obey instructi<strong>on</strong>s <strong>on</strong> warning signs, notices and given by Workshop staff.6. Students are not required to wear safety shoes or l<strong>on</strong>g trousers when attending lecture in Engineering Drawing,Safety and Computer <strong>Training</strong>.7. For ease of identificati<strong>on</strong>, students are required to wear a name tag while under training at the <strong>IC</strong>. This willhelp the <strong>IC</strong> staff to assess the performance of individual student.2. Name tags can be purchased at the <strong>IC</strong> Trainee Enquiry Counter at W309. Students can also bring their ownplastic name tag and get a free paper name tag from the <strong>IC</strong> Trainee Enquiry Counter for filling their informati<strong>on</strong>.CHANGING ROOM & LOCKERSWHY: When attending training, students can <strong>on</strong>ly bring al<strong>on</strong>g with them the module reading materials and somewriting materials. All other bel<strong>on</strong>gings should be stored inside the lockers in student changing rooms.WHERE: Male - Room W101 Female - Room W102HOW: 1) Each student should <strong>on</strong>ly use <strong>on</strong>e locker. 2) Students should bring al<strong>on</strong>g their own padlock. 3) Please d<strong>on</strong>ot leave valuables in the locker. 4) Please clear your bel<strong>on</strong>gings at the end of each training week.Last update: August 2008


PART B: TRAINING & ASSESSMENT<strong>IC</strong> TRAININGInclude 5 major areas: 1) Workshop <strong>Training</strong> 2) Engineering Drawing 3) <strong>Industrial</strong> Safety 4) ManufacturingProject 5) Computer <strong>Training</strong>ASSESSMENT1. <strong>IC</strong> training is mandatory and assessed by coursework (assignment, report, project, etc.) and/or test.2. Timekeeping, attendance and work attitude are also important in <strong>IC</strong> training.3. Timekeeping3.1. 100% attendance requested3.2. Prior approval from <strong>IC</strong> Trainee Affairs Officer (see below) must be obtained for leave. For sick leave,document from Registered Doctor should be submitted immediately after the sick leave is taken.3.3. Make up must be arranged if leave is taken.3.4. Absence without approval will be penalized with double make up.3.5. Make up must be completed before the examinati<strong>on</strong> period of that semester. Otherwise resultscannot be counted toward the final subject result.WORKSHOP REPORT & TRAINING LOG BOOK1. Workshop Report1.1. Must be in your own hand-writing and submitted within <strong>on</strong>e week after completing the relevant trainingmodule.1.2. If you score a failure <strong>on</strong> any workshop report, you are required to resubmit a fresh report and c<strong>on</strong>tinue todo so until you score a pass.1.3. Students should retain all marked workshop reports, which will form the main body of <strong>Training</strong> Log Book.2. <strong>Training</strong> Log Book2.1. It is a complete record of your training in the <strong>IC</strong> and important document to your applicati<strong>on</strong> formembership of professi<strong>on</strong>al bodies. <strong>Training</strong> Log Book includes Table of C<strong>on</strong>tents, Introducti<strong>on</strong>, WorkshopReports (marked), Project Notes (if any) and Comments and C<strong>on</strong>clusi<strong>on</strong> (not less than 500 words). Allc<strong>on</strong>tents should be filed in a Log Book Cover.2.2. You are requested to submit your <strong>Training</strong> Log Book within four weeks <strong>on</strong> completi<strong>on</strong> of all <strong>IC</strong> training.2.3. <strong>Training</strong> Log Book is mandatory for majority of <strong>IC</strong> training programmes.3. Download blank report sheet or Log Book Cover: http://www.ic.polyu.edu.hk/student_net/downloadform.htm.COMPUTER TRAINING1. Students should complete the computer training as stipulated in their programme documents or <strong>IC</strong> <strong>Training</strong>Programme in their first year of study. You should plan carefully your schedule when taking the Computer<strong>Training</strong> and avoid overloading yourselves in <strong>on</strong>e semester.2. Free web-enrollment (via <strong>IC</strong> Homepage) throughout the whole year.3. Available of Competency Test. Students who passed the test can be exempted from the training and earn thecredit.4. Computer <strong>Training</strong> is assessed by coursework (70%) and exit test (30%).5. Students who failed in any <strong>on</strong>e module will be required to re-take the module. Students will be requested tosettle a “Late Assessment Fee” of HK$245 per module before allowing to either attempting the Competency Testagain or re-taking the module.6. <strong>IC</strong> Computer <strong>Training</strong> Homepage: http://www.ic.polyu.edu.hk/programmes/computer.htm.DISNOHESTY & PLAGIARISMDish<strong>on</strong>esty and Plagiarism in examinati<strong>on</strong> and coursework is a serious misc<strong>on</strong>duct.
