Youshould receive your group billing approximately ten (10) days prior to the duedate. Payment for health care protection is, therefore, paid in advance. Forexample, if your due date is the first of the month, payment is received andcredited for the first day through the end of the month. If your due date is thefifteenth of each month, pre-payment would extend from the fifteenth of themonth through the fourteenth of the next month.GROUP BILLING INSTRUCTIONSPLEASE REFER TO THE SAMPLE BILLING (Pages 5-3 through 5-5).PAGE 1 & 2 OF GROUP BILL - INSTRUCTIONSPage 1 is for all adjustments for employees terminating employment prior toreceiving the bill ONLY. to the invoice which change the amount due. Group Billing Summary - The summary includes the roster total, amountsdue/credited from prior billings, adjustments, and the total amount due.Calculate your amount of adjustments and enter in the space provided under theamount due.
