SCHEDULE-ANAME OF WORK: Cleaning of Boiler from inside, outside, pent house repairing, Refractorylining work & tube thickness survey of Boiler Unit No.3, 4 & 5 at Ukai TPS.SCOPE OF WORK:01. Cleaning of boiler :Prior to start of work, clearance <strong>for</strong> washing is to be taken from EIC.• Boiler 1 st pass & Pent house consists of elements as below: Thorough water washing in WW & RH slope area be<strong>for</strong>e cup lock erection. Complete removal of ash from penthouse by vacuum.• 2nd pass/Duct/Wind box cleaning following areas Coils of LTSH/Economiser/SCW etc All boiler Air/Flue gas ducts Both side wind box (Inside & Outside) Economiser & APH hoppers• Boiler horizontal convective pass consists of following areas Front & Rear Reheater coils.(Water washing & scraping if required) Screen water wall & screen super heater. Final super heater coils. LTSH terminal tubes & Side steam cooled walls.• Deck, dog house, cat house& outside cleaning consist of following areas Boiler Deck (Area above pent house) cleaning by vacuum & manual method Dog house (8.5 Mtr behind WW front & Rear side slope) and Cat house (Minipent at 40 mtr.) Complete boiler Outside cleaning up to bottom.• Preparation of scaffoldings in horizontal pass & PSH Erection of scaffolding in RH, Screen WW, Screen SH, Final SH,LTSH terminaltubes. Erection of scaffolding in PSH coils (If only sky climber provided) Economiser / LTSH unapproachable areas.• Cleaning of all remaining scrap/debris/refractory/kathi etc. All remaining scrap/debries/refractories/kathis & other unwanted materials shouldbe removed from boiler and shifted to scrap yard or other suitable place as perinstruction of EIC After completion of AOH all boiler area to be completely cleaned.a. Be<strong>for</strong>e starting the cleaning operation all necessary manholes of Boiler/Ducts are to beopened.b. Be<strong>for</strong>e starting the cleaning operation in Boiler, complete de-ashing of the Eco/air preheaterHoppers are to be ensured from the EIC.c. Remove all the deflector plates/ wire mesh placed over the coils.d. The heating surface in boiler 2 nd pass of units is to be first cleaned by Fire water throughfire hose. Fire hose and nozzles <strong>for</strong> water washing will be supplied by GSECL.Aftercompletion of water washing of the second pass, fire hoses and nozzles are to be returnedback to GSECL store in good condition.e. Water wall tube heating surface Boiler 1 st / 2 nd pass water walls / S.H. / R.H. economisercomplete cleaning from inside by water jetting, and by manual scrapping. The ash depositshould be removed from furnace. The bottom ash should be cleaned after furnacecleaning.- 5 -


f. After the completion of the water washing, boiler tubes and bends are to be further cleanedby Kathi/wire brushes. For this wire brushes in sufficient numbers are to be arranged by thecontractor.g. All Eco / APH hopper are to be clean by water jetting.h. All ducting of boiler and its auxiliaries such as I.D. Fan, F.D. fan, P.A. fans, wind box,scaph, E.P. etc. should be cleaned from inside & outside completely.i. Outside cleaning includes removal of fly ash, coal dust, scraps, cut valves, pipes, debris,cotton-waste etc. by sweeping from all girders, buck-stays and boiler structure to be donefrom 56 mtr elevations to 3 mtr elevation from boiler. Ash should be removed in such a waythat it should not be sprayed out all over the floor area.02. Cleaning of ash from pent/Dog/Cat house & deck area above pent house:a. The ash accumulated in the pent house & Deck area is to be removed and the completepent house is to be cleaned <strong>for</strong> removing any debris or <strong>for</strong>eign materials. The ash is to beremoved in the ash disposal system through vacuum line extended up to pent house. Partywill have to connect and extend the flexible pipes in pent house. GSECL will provide flexiblepipes and fittings. If required party will have to carry out repairing/replacement of MSpipes/Bends if vacuum drops during evacuation. However required materials like Bends &pipes will be provided by GSECL free of cost.b. Cleaning of pent house/deck is to be completed within 07 days from the start of unitcleaning work, failing which it will attract a penalty of Rs.1000/- per day, subject to a max. ofRs.5000/-c. Complete Cleaning and removal of insulation debris from inside the pent house is to becarried out up to satisfaction of EIC. Ash is to be disposed properly from Boiler, whereas,debris like insulation wool and any metallic scrap are to be collected in separate gunnybags and disposed to the scrap bins at boiler zero meter. All care shall be taken, whiledisposing ash through ash disposal system, that no spill over of ash, polluting surroundings,takes place.d. The job comprises cleaning of ash accumulated inside penthouse, water wall deadchambers at + 40 m elevation (Cat house) and at +8.5 m elevation (Dog house). Theaccumulated ash is to be removed and the complete cleaning of the above mentionedlocations is to be done by removing any debris or <strong>for</strong>eign materials also. The ash is to becollected in gunny bags and is to be shifted to boiler ‘0’ meter.03 Preparation of scaffolding in second pass:a. Scaffolding may be required to be erected in one or many locations.b. Scaffolding is to be erected along the screen tubes/Reheater coils/Final SH coils/LTSHterminal tubes <strong>for</strong> its inspection and repair.c. Plat<strong>for</strong>ms below the Eco. Coils to be erect <strong>for</strong> inspection of bottom header tubes/bends.d. Shift scaffolding materials like pipes, clamps, wooden planks etc. from GSECL store to site.e. Manila ropes/Fresh Kathi are to be arranged by the contractor.f. Plat<strong>for</strong>m is to be erected at at-least 03 elevations along the second pass all coils such thatall tubes can be inspected.g. Plat<strong>for</strong>ms erected should be firm and safe.h. All plat<strong>for</strong>ms erected shall be provided with hand rails <strong>for</strong> safe movement and working.i. Ladders are to be installed <strong>for</strong> reaching out different locations.- 6 -


j. Dismantle the plat<strong>for</strong>m after completion of the work and return the scaffolding materials toGSECL store.k. Party will have to erect at least 12 full length plat<strong>for</strong>m in boiler will be treated as onenumber <strong>for</strong> measurement.04 Preparation/Operation & maintenance of sky climber.a. All the Materials of sky climber will be given by GSECL free of cost.b. Shifting of sky climber materials from store to site and then returning back to the store is ofcontractor’s scope without any extra payment.c. Plat<strong>for</strong>m erection inside the furnace <strong>for</strong> erection of sky climber will be of contractor's scope.d. For erection/operation of sky climber, safe plat<strong>for</strong>ms at 7.0 mtr.inside the Boiler is to beprepared by contractor at the quoted rates.e. Plat<strong>for</strong>m material i.e. pipes and planks will be supplied by GSECL if available. Otherwiseparty will have to arrange.f. During the period of the AOH, the sky climber is to be operated and maintained by thecontractor <strong>for</strong> which operators and electricians must be available round the clock.g. The sky climber is to be put into operation within twenty four hours from the time <strong>for</strong>instruction <strong>for</strong> the erection of the sky climber by the EIC.h. Power supply <strong>for</strong> the sky climber shall be made available at boiler 18 M elevation free ofcost by GSECL.i. Pro-rata deduction will be made in case of non-adherence to the above schedule.j. Minimum Three nos. of sky climber to be erected and operated <strong>for</strong> work completion inscheduled time.k. If any of sky climber failed during operation then same will have to be attend immediatelyand put back into service.NB: Payment <strong>for</strong> cleaning in different locations/zones will be done as given blow:a) 1 st pass cleaning. - 10% of cleaning paymentb) 2 nd pass/duct/wind box/stack hopper cleaning - 20% of cleaning paymentc) Horizontal convective pass cleaning. - 15% of cleaning paymentd) Pent house,Deck,dog house, cat house& outside cleaning. - 25% of cleaning paymente) Preparation of scaffoldings in horizontal pass & PSH. - 20% of cleaning paymentf) Cleaning of all remaining scrap/debris/refractory/kathi/ - 10% full paymentropes etc after work completion-06. SCOPE OF WORK (TUBE THICKNESS SURVEY) :This is a job work <strong>for</strong> preparation of thickness survey of boiler tubes of unit no3,4 & 5 insidethe boiler at various places. This include-es preparation of tube surface <strong>for</strong> measurementof thickness by cleaning, emery polishing and grease application. The assembling,dismantling, shifting and operating of sky climber during the work as per conditions. Partyhas to arrange necessary scaffolding/approach <strong>for</strong> measurement of thickness on tubebends, straight pipe of super-heater, water wall, economiser etc. (any where inside 1 st passand 2 nd pass). Each location consists of about 0.5 mtr. Approximate length where minimumfive points are to be checked as per instruction of Engineer-in-charge.CONSUMABLES/EQUIPMENTS (GSECL will supply Free of cost) :01 Grease material from BMD Section.02 Old emery stone / emery paper etc..03 Old plates / pipes / planks etc..04 Sky climber hoist from BMD.- 7 -


Contractor will have to arrange in quoted rates:01 Cotton waste/Kathi <strong>for</strong> removal of grease.02 Any other, thing necessary <strong>for</strong> testing but not listed.07. THE DETAIL SCOPE OF ABOVE WORK FOR REPAIRING OF PENT HOUSE, GOOSENECK, DOG HOUSE, WIND BOX & SKIN CASING:01 After Boiler stop permission from Engineer- in-charge to be taken to work first the entirewind boxes / penthouse / dog house to be opened and detail survey with GSECLsupervisor to be carried out. According to work requirement manpower is to beengaged. No surveyed portion is to be left out02 Complete cleaning of area to be repaired and inspection by light/smoke test.03 Cracked pent house /Dog house/ wind box to be welded after proper cleaning. Party willhave to arrange scaffolding and removal of insulation wherever required.04 Skin casing plate cutting and removal from pent house, goose neck, dog house, Headerboxes as per instruction of Engineer-in-charge.05 Removal of pourable / castable refractory.06 Wherever, voids are there, they are to be closed by fins attachment and plates.07 GSECL will provide required material like bellow, crown plates, scallop bar etc. and ifnot available then same will have to be fabricated from 2.5 mm corton steel/MS plates.08 Fit up of all skin casing material as per instruction of EIC.09 Refractory filled-up and covering with skin casing sheets.10 Party will have to carry out refractory work in soot blower boxes/burner corners/headerboxes/7 Mtr slope or where ever required in the Boiler.11 The scope of work includes skin casing, repairing & refractory lining in all boiler whereever required.12 Other miscellaneous safety related fabrication works like ralling, plat<strong>for</strong>ms, supports,frams, grills, cabin etc will have to be carried out by party as per instruction of EIC.13 Party will have to carry out the complete wind box sealing by patch work of requiredsize.14 Party will have to carry out other works as shown in Schedule-B and instruction of EIC.15 Inside furnace cuplock scaffolding will be arranged by GSECL by separate agency.However, subsequent extension of scaffolding required, if any <strong>for</strong> this work shall have tobe made by the party at his own cost.16 Work is to be carried out at various elevations from 7 Mtr slope to pent house roof.Necessary approach required to carry out the work satisfactorily shall be managed bythe party along with the scaffolding materials where ever approach is not available.17 Plenty of welding and refractory work is involved. Hence, adequate welders andwelding machine with accessories shall be arranged by the agency in his quoted rates.18 Curing of the refractory shall be carried out as per standard procedure and as perinstruction of Engineer-in-charge.19 Necessary steel material and refractory required to carry out the work shall be providedby company free of cost.- 8 -


TECHNICAL TERMS & CONDITIONS:01 Party shall have to prepare surface be<strong>for</strong>e thickness measurements. Any scaffolding /approach required <strong>for</strong> taking the measurement is to be arranged be the party at his owncost, and same should be removed as soon as the work will completed.03 The work of tube thickness survey is to be carried out as per instruction of Engineer-incharge,only and it should be got checked by Engineer-in-charge be<strong>for</strong>e proceeding <strong>for</strong> nextstage.04 For completing the tube thickness work within 07 days party will have to arrange minimumfour separate gangs.05 The contractor can arrange pre-arrangement i.e. be<strong>for</strong>e shutdown like shifting / lifting ofmaterials from stores to working floor and made available after finishing ready to expeditethe work in time limit schedule. This advance arrangement period shall not be considered /included in time limit period.06 The contractor’s resources will be available round the clock during overhauling period on allworking days and holydays to meet the target and time schedule. For this, resources maybe arranged in three shifts.07 Party has to submit details of tools, tackles and manpower available with him be<strong>for</strong>e staringof work and day-day progress report with man power is to be submitted to Engg-in –chargeevery next morning. Engg-in-charge will review work progress, tools tackles & manpowerworking and if any delay found due to any of above factors then same will have to bearranged by the party immediately.08 In COH/AOH several agencies are working in parallel. Party shall co-operate witheverybody and work in harmony with the BMD staff to achieve the common goal ofcompleting the Over hauling of Unit in time bound period.09 Party shall have to arrange adequate tools, tackles, skilled persons to meet scheduledoverhauling within time limit. At any time if GSECL feels that progress of work isunsatisfactory, another agency / personnel shall be put in parallel <strong>for</strong> expediting the work, atthe risk & cost of the party.10 The quantity of work may vary Unit wise or as per site requirement. The payment will bemade as per actual work done only.11 The scope of works includes carting of materials from the GSECL main store / sub store asthe case may be and as directed by GSECL Engineer <strong>for</strong> which no extra payment shall bemade <strong>for</strong> transportation.12 Electrical connection and supply of energy will be in the scope of GSECL. The lighting is tobe arranged by the party. The GSECL shall make arrangement <strong>for</strong> providing power supplyto the contractor <strong>for</strong> working their equipments, tools etc. as and when required as decidedlocations / points by Engineer-in-charge. The contractor may arrange ICTP switches andELCBs. Further distribution shall be at the cost of contractor. Floods <strong>for</strong> lighting will bearranged by GSECL as and where required. However contractor at his cost shall arrange100 Watts / 24 Watts bulbs.13 Party will have to arrange all required tools/tackles such as welding machine, chain pulleyblocks, gas cutting sets, portable grinders, drills, grinding wheel, Kerosene, petrol, cottonwaste,rustolene cloths etc. at his own cost.- 9 -


14 TIME LIMIT :The overall validity <strong>for</strong> the work contract is 12 month, counted from the date ofCommencement of the work. The time limit during AOH is 20 days per unit <strong>for</strong> actual workand 5 days <strong>for</strong> other outside work.15 This is a rate contract <strong>for</strong> a period of 12 months.GSECL reserve the right to extend thiscontract <strong>for</strong> further six months as deemed suitable, considering the balance quantities yetto complete and it will be binding to the party. The rates and other terms and conditions willremain same as per original order <strong>for</strong> the extended period. Rates quoted are firm hence noprice and labour escalation will be applicable.16 The period of contract shall be 12 months. However the duration of the shutdown of anyunit may be decreased or increased as per the site conditions <strong>for</strong> which party will have toarrange sufficient resources to handle such situations.17 07 days time shall be allowed from the date of receipt of telegraphic/fax order/letter <strong>for</strong>mobilization of contractor man, T&P etc. Commencement date <strong>for</strong> Starting the work will bereckoned accordingly or from the date contractor is permitted to start the work.18 The EIC has the every right to deduct a maximum level of 10% of the contract value <strong>for</strong>poor workmanship or <strong>for</strong> a considerable delay in the completion of the job.19 The work carried out against this work order / tender shall be guaranteed <strong>for</strong> a period of(06) Six months from date of synchronization of unit. After synchronization of unit/recommissioningof equipment/system, if any problem arises <strong>for</strong> the work carried out, it shallhave to be set right by the contractor without any extra charges to the GSECL. Contractorshould attend any defect contributory to their faulty workmanship during the trial run and recommissioningof equipment / system / unit, and set right without any extra charges to theGSECL. If rectification is not carried out in time as instructed by Engineer-in-charge, thesame shall be carried out by GSECL by engaging GSECL staff or any other agency andcost of which will be recovered including supervision charges from the contractor’s bill asper Corporation’s rules.20 Only special purpose welding electrodes will be given by GSECL free of cost. Party has tocredit end pieces of welding electrodes.If party fails to credit end pieces, the recovery shallbe made from party’s bill.21 Party has to submit detail report <strong>for</strong> material issued and used with location & detailmeasurement to Engineer-In-charge. Joint Measurement will be taken in presence ofGSECL’s supervisor or engineer-in-charge. The signed joint measurement sheets will haveto be submitted along with bill invoice.22 Party has to come-up with concrete suggestion in writing to solve any problem observed inrunning unit previously related to work awarded. If no improvement observed inper<strong>for</strong>mance, dealing with party will be stopped in future.23 Contractor or technically qualified authorized person has to remain present at site & if it isobserved that work is carried out under GSECL’s supervision in his absence. Penalty of Rs.1000/- per day will be imposed.24 Any work related to contract awarded is not mentioned in the scope and is required to becarried out then the same will be in the party’s scope at no extra cost.25 All welding electrodes should be routed through main Store. Necessary gate pass entry atCISF & main store to be done in delivery challan. All these verified/endorsed chalans are tobe submitted along with material account statement without which bill can not beprocessed.26 GSECL reserves the right to <strong>for</strong>feit the security deposit in case of any breach of contract.27 GSECL will provide workshop facility together with operator in GSECL workshop premisesas and when required and as decided by the Engineer-in-charge as the case may be.Toand fro transportation of spares <strong>for</strong> repair at workshop will be arranged at your cost- 10 -


28 The contractor shall have to arrange to remove the day-to-day scrap materials from site andcredit to GSECL main store / site store as per instruction of Engineer-in-charge. The sitemust have to be cleared be<strong>for</strong>e starting / commissioning of equipments / unit; otherwise theamount of Rs.5000/-or actual whichever is higher will be recovered from contractor’s billwithout assigning any reason in the event of non-removal of scraps29 The contractor shall adhere to safety practices against hazardous and unsafe workingconditions and shall comply with the safety rules in <strong>for</strong>ce from time to time of GSECL, UkaiTPS. The contractor has to arrange <strong>for</strong> 24 V trans<strong>for</strong>mer/battery to men to be utilized assafety equipment while carrying out the work inside of equipment/at higher elevation. Alllifting tackles to be utilized are tested as per norms & test certificate to be shown ondemand <strong>for</strong> safety point of view.30 Unit wise detail material account statement is to be submitted to Engineer-in-charge afterwork completion, without which bill can not be processed. Delay in processing the RA/Finalbill due to non submission of material account is the contractor’s responsibility31 The rates given in schedule-B will be firm till completion of work in all units. GUJARAT ELECTRICITY CORPORATION LIMITEDTHERMAL POWER STATIION, UKAI DAM-394 680TERMS & CONDITIONS REGARDING INDUSTRIAL LAWS AND OTHER RELATEDMATTERS TO BE INCORPORATED IN THE TENDER DOCUMENTS1.1 Wages to be paid and time of payment etc. by the Contractor.1.2 The contractor shall pay minimum of wages per day or as may be specified hereafter orrates fixed under the Minimum wages Act, whichever is higher. The wages of everycontract labour employed by him under this contract shall be paid him be<strong>for</strong>e the expiryof 7 th day of the last day of the month in respect of which the wages are payable ( i.e.wages of a month have to be paid by him in the first week of the next month). Thepayment shall be disbursed in the presence of management Representative during theworking hours in factory premises and the contractor shall get the entires certified in theregister of wages by the representative of the Corporation. Any default will result incancellation of contract <strong>for</strong>thwith or else the contractor shall be punishable to the extentof Rs. 400/- fine per each day.1.3 The contractor shall give his telephone number and address to the Corporation so thatin case of labour troubles etc. the contractor can be contacted. The contractor shallarrange to have his office outside the factory premises and the contractor keep himselfpresent through out the working hours “As contractor you will be responsible and liableto pay difference in wages if any or observed the revised service condition that may beawarded by the Honorable Industrial Tribunal effective from the date directly in theAward”.02 Labour Laws:2.1 Person below the age of 18 years shall not be employed <strong>for</strong> the work.2.2 No female worker shall be employed in the night shift between 7 PM to 6 AM.2.3 Contractor shall maintain a valid labour licence under the Contract Labour (Regulationand Abolition) act <strong>for</strong> employing necessary manpower to be required by him. In theabsence of such licence the contract shall be liable to be terminated without assigningany reasons thereof.2.4 The contractor shall at his own expenses comply with all labour laws and amendmentmade from time to time keep the Corporation indemnifies in respect thereof. Some ofthe major liabilities under various labour and industrial laws which the contractor shallcomply with are as under:-2.5 Payment of contribution by way of Employees (contractor Labour) as well as employer’s)contractor) Contribution towards Provident Fund, Family pension scheme, EmployeesDeposit linked insurance scheme, Administrative charges, inspection charge etc. at therates made applicable from time to time by government of <strong>Gujarat</strong>/Government of Indiaor other statutory Authority.2.6 Labour engaged by you shall be entitled <strong>for</strong> Earn leave as per the provision laid down infactories Act. 1948. Contractor shall extended the facility of E.L. as per the provisionlaid down in factories Act. And shall also have to maintain leave records in prescribed<strong>for</strong>m under factories Act.2.7 No. labours are allowed to enter in power station premises who do not have Identity cardprescribed under Factories Act.2.8 Contractor shall have to comply with the Maternity Benefit if he happens to engagedfemale labours as per the provisions of the Maternity Benefit Act.2.9 Contractors shall have to obtain Insurance of the labourers engaged buy him to carry outcontractual work be<strong>for</strong>e commencement of work under workmen’s compensation Act.2.10 P. F. code Number: The contractor shall have to submit the details of his own P.F codenumber along with offer from appropriate authority. Without P.F. code No. the tender islikely to be disqualified.- 12 -


2.11 Provident Fund & Family Pension SchemesThe Contractor shall submit along with his bill (month-wise) a statement regardingdeductions at the rate of 12% of the wages against employees provident Fund & FamilyPension Scheme in respect of each concerned employee. (or at the rate madeapplicable by the Government from time to time) of the wages. The contractor’scontribution and his workers contribution towards Provident Fund and Family PensionScheme shall be deposited by the Contractor with Regional Provident FundCommissioner, Ahmedabad or to the authority prescribed under the Act. Along with theother charges applicable.2.12 If any workman engaged by contractor meets with fatal or non-fatal accident while onduty then contractor shall be liable to pay compensation Act.2.13 Employees Deposit Linked insurance Scheme: The contractor shall have todeposit ½% of the wages in respect of employees who is member of the Provident Fundas the contribution to the Deposit Linked insurance scheme with Regional ProvidentFund Commissioner, Ahmedabad2.14 The contractors should pay prevailing minimum wages to the labourers engaged by himas per the minimum wages Act. In presence of the corporation officer of representative.2.15 The contractor shall deposited fifteen days salary <strong>for</strong> every completed year of service incase of each worker towards the liability of gratuity.2.16 Administrative Charges:Administrative charges <strong>for</strong> maintaining provident fund A/c. shall be deposited by theContractor with Regional Provident Fund Commissioner, Ahmedabad at the ratesapplicable.2.17 Labour License <strong>for</strong> Labour Contract:The contractor shall have to obtain the copy of labour contract license under contractLabour (Regulation & Abolition) Act. from the appropriate authorities <strong>for</strong> commencementof work2.18. Contractors shell have to issue the contractual appointment letter <strong>for</strong> a specified periodto their labour engaged during the contract period each and every instant & shouldnarrated clear out terms and conditions <strong>for</strong> the liabilities of the concerned employees.2.19. After completion of contract work, the work completion certificate is required to beproduced be<strong>for</strong>e the DGM/IRO <strong>for</strong> the purpose of no dues certificate and cancellation ofgate passes issued to the contractors work men at the time of execution of the contractwork.2.20. The contractor shell have to maintain all the relevant records under labour laws asmentioned here in above and keep ready <strong>for</strong> verification by the corporation authority aswell as the authority of the Govt. official a and when asked <strong>for</strong> the checking.2.21 The contractor shell have to produce/submit the original copy of wedges register/musterroll and relevant record under labour law <strong>for</strong> the contract work as & when demanded bythe corporation statutory completion if arises in future.The above are some of the major liabilities of the contractor in addition to other liabilitiesprescribed under the various labour laws in <strong>for</strong>ce from time to time from statutoryAuthorities like stage Government/Government of India which the contractor shall haveto comply with.03 Paid Leave Facility:Paid leave facility at the rate of one day <strong>for</strong> every twenty days worked by the contractlabour shall be provided by the contractor to his workers. He shall maintain LeaveRecords/Leave Cards, <strong>for</strong> individual laborers which shall be duly verified andapproved/certified by the authorized officer of the Corporation.- 13 -


04 Workmen’s Compensation Fund & Employer’s Liability Insurance:The contractor shall cover all his employees under workmen’s compensation Fund &under the employer’s liability insurance.05 Maintenance of registers & records:The contractor shall employ adequate number of experienced staff at site <strong>for</strong> dailysupervision and <strong>for</strong> maintenance of various registers and records required under the lawand contract. No payment <strong>for</strong> supervision shall be admissible.06 Contractor to indemnify the Corporation:The contractor shall indemnify the Corporation and every member officer andemployees of the Corporation also. Engineer-in-charge and his staff against all action,proceedings, claims, demands, costs and expenses whatsoever arising out of or inconnection with matters referred in herein above elsewhere and against all actions,proceeding claims, demands, costs and expenses which may be made against theCorporation or Government <strong>for</strong> or in respect or <strong>for</strong> per<strong>for</strong>mance of his obligation underthe contract documents. The Corporation shall not be liable <strong>for</strong> or in respect of or inconsequence of any accident or injury to any workmen or other person in theemployment of the contractor of his sub-contractor, and the contractor shall indemnifyand keep indemnified the Corporation against all such damage and composition andagainst all claims, demands, proceedings costs, charges and expenses, whatsoever inrespect thereof or in relation thereto.07 Workmen’s Compensation and Employer’s Liability Insurance:Insurance shall be effected <strong>for</strong> all the contractor’s employees engaged in theper<strong>for</strong>mance of this contract. GUJARAT STATE ELECTRICITY CORPORATION LTD.Thermal Power Station-Ukai Dam – 394 680, Dist: SuratGENERAL TERMS AND CONDITIONS01 The ‘Contract’ means the documents <strong>for</strong>ming tender and subsequent agreement thatmay be entered into.02 The tender should be submitted along with EMD without which no tender will beaccepted.03 Conditional tender will not be accepted Unilateral withdrawal of condition after openingof tender will not be permitted.04 The general rules & regulations of the corporation <strong>for</strong> tender and contract <strong>for</strong> works willapply to the extent the same are not modified herein. Bill.08 The contractor shall hold the corporation indemnified against and in respect of any claim(S) arising out of injury to any person(S), loss of life, workman’s compensation or anyother claims whatsoever. The contractor shall be responsible <strong>for</strong> any damages to anyproperty of the corporation due to negligence of the contractor or his employees. Thecorporation will be final and blinding to the contractor.09 The contractor shall submit his Sales Tax registration number and same should bequoted on all his bills.10 The contractor will have to enter into an agreement with the corporation and the cost ofthe stamped paper shall be borne by the contractor, the contractor shall also have toexecute an indemnity bond on required value of stamped paper as per the corporationrules. The cost of the stamped paper shall be borne by the contractor.11 Arbitration Clause : " All questions " disputes or differences what so ever which may atany time arise between the parties to this agreement in connection with the contract orany matter arising out of or in relation there to, shall be referred to the <strong>Gujarat</strong> publicworks contract dispute Arbitration tribunal as per provision of the <strong>Gujarat</strong> public workscontract disputes Arbitration Tribunal Act, 1992”.12 The party shall strictly observe all the Labour Laws and Factory acts in existence andamendments made from time to time.i) Party shall have to strictly observed all Safety rules as per the Factories act 1948 andwhatsoever amendments made from time to time to avoid any chances of accidents.Contractor shall have to strictly follow the instructions of Safety Officer.ii) The safety precaution issued vide this office Circular no, CCUT / Safety / 4 / 90 / CIR /345A dtd.14-12-90 are applicable to this contract also and to be followed strictly.iii) Be<strong>for</strong>e starting the contract, it is advisable that party should have the documents <strong>for</strong>insurance of workers.13 The work shall be completed within stipulated time. 15 The contractor shall have to take all necessary precautions to avoid fire during, cutting,welding ect. In case of any shouldering fire, the same should be extinguished bycontractor immediately using their own labour, tools water and buckets ect.16 The party shall arrange his own adequate tools and tackles, welding accessories drillingmachine, girders, scaffoldings lighting materials including lighting trans<strong>for</strong>mers as preIS-1416 ect. required <strong>for</strong> the work included in Schedule-A.17 Party shall have to arrange all consumable, including required kerosene, rustolene,paints, grinding wheels etct at his own cost.18 An authorized and technically qualified representative of the contractor must beavailable at site white the work is going on. The address of the representative should besubmitted to this office so that party can be contacted whenever required in connectionwith execution of the work.19 Scaffolding material, if available with GSECL will be issued on chargeable basis,otherwise contractor will have to arrange <strong>for</strong> the same at his own cost. After completionof work scaffolding is to be removed immediately.20 After completion of work .Working area and surrounding area should be made clean byremoving M.S. debris and all types of scrap etc. Scrap should be credited to main storeand should be dumped at place shown by E.E. (Stores).21 Electrical connection & consumption will be given free of cost at one mutually agreedpoint.22 Oxygen / Acetylene gas if available, will be issued on chargeable basis from GSECLmain store, otherwise the party has to arrange on its own. The empty cylinders shouldbe returned to main store within stipulated time.23 Party shall submit daily progress report <strong>for</strong> the work carried out by them.24 The contractor, if desires, can seen the plant and site of work with permission ofundersigned on any working day during working hours.25 Party has to carry out the work at site only.26 The tenderer / contractor shall have to submit their own P.F. code number if allotted byRPF commissioner / P.F. authority. In case of P.F. authority has not issued / allottedP.F. code number to you then you shall have to comply with provision of the P.F. Actand submit P.F. contribution of your Employees and contribution of yourself to the P.F.trus of the corporation.27 All the employees engaged by you will be covered under P.F. Act and if you are deniedthe separate P.F. code number by the P.F. Authority than you will have to submit theP.F. contribution along with administrative charges to GSECL P.F. Trust without fail. GUJARAT STATE ELECTRICITY CORPORATION LTDUTPS – SAFETY - 002 REV - 00 15\12\2008 Ukai TPS“GENERAL SAFETY RULES TO BE OBSERVED BY THECONTRACTORS”All the contractors working at Ukai thermal Power Station shall have to strictly observe ollowingsafety rules. The contractors shall be responsible <strong>for</strong> in<strong>for</strong>ming and observing these rules bytheir supervisors / employees /labours as well as the supervisors / employees / labours of theirSub-agencies / sub-contractors engaged, if any. Prior to commencement of the work,contractor shall have to submit a written assurance on their letterhead to the concernedSectional Head / Engineer-in-charge that they have thoroughly gone through these rules, haveeducated their employees / employees of their sub contractor and will strictly observe the saidrules while execution of work under work contract awarded to them. They will have toindemnify the corporation <strong>for</strong> any loss or damage / accident / injury to the corporation’sproperty / employee or employee of their own in default of non - observing theserules.(01)Persons to be employed <strong>for</strong> carrying out the work shall possess requiredqualification, be fully trained and conversant <strong>for</strong> works to be done. All persons should havegate pass. Register consisting the full details (i.e. address, phone no, details of nearestrelative, etc.) of all persons is to be maintained .during the work execution, one trained &competent supervisor should always remain present at site.(02) The contractor shall all the required safety measures prior to commencement of work ondangerous substances, machineries or area at which cautionary notice is displayed and obtain“line clear” or “work permit” through the concerned Department / Section.(03) Prior to carrying out welding, gas cutting, furnace heating or any other hot work job, thecontractor shall remove all the inflammable material lying at nearby worksite or cover itproperly by suitable protective covrin. Also, special care shall be taken be<strong>for</strong>e carrying outsuch job & see that all possible contributing factors to set fire shall be removed / vanished priorto commencement of the work. Advance intimation shall be given to concerned section / firesection to commence the work in fire prone areas. they should also keep ready all the first aidfire extinguishers / equipments & fire extinguishing media / material like sand / water bucketsor other appropriate equipment at such place.(04) While carrying out work in confined space or inside vessel, the contractor shall obtainnecessary “confined space / vessel entry permit” from concerned department prior tocommencement of the work.For lighting in such areas, 24 volt (ISI certified & with proper guard) hand lamp shall be used.For taking care of the persons working inside the confined space / vessel, a supervisor /person capable to keep continuous watch on person(s) working inside, assist them incase ofemergency or arrange to get immediate outside help. Shall remain present at entry point shallremain present at entry point and shall use full body safety belt without fail.While working inside sewage, trench or in-depth, a person to warn outsiders / entrants/passers etc shall remain available near entry point or the entry point shall be cordoned by abarricaded tape with a cautionary notice. After completion of the works, all the lids / covers /grills / grits opened, shall be re-fixed / re- placed in the original position as it were prior shall bere- fixed / re-fixed / re-placed in the original position as it were prior to commencement of thework and leave the work place in safe condition in all respect, so as to prevent accident tofellow workers.(05) The contractor shall see that he / his persons do not block (by stacking material, spare parts,tolls-tackles, equipments etc), any passages / walkways / gangways / aisles / staircases /ladders / lift or any other approaches / roads leading to plants or its auxiliaries, on which thereis a traffic movements in case of emergency . if it is utmost necessary to carry out work in sucharea with blocking of passage, prior permission of competent authority or the engineer- inchargeshall be obtained. To demarcate /declare the area as UNSAFE, cordon it usingbarricading type & display suitable caution notice or keep a person to restrict / divert the trafficroute through other safe passage.- 17 -


(06) Prior to use power / electrically operated hand tools / equipments / machines / gadgets likewelding machine, hand grinder , hand drill etc, ensure its safe operation & use it only if it isfound safe to use . do not use defective, unsafe or improperly maintained equipments.The electrical power supply required to run such equipments shall not to be takendirectly at their own but shall be obtained through concerned electrical maintenancedepartments or their authorized persons or under their observations / guidance only. Theelectrical section shall provide temporary electrical connection up to contractor’s mains boardon which it is compulsory to install mains switch, ELCB & fuses of adequate capacity. all suchequipments shall invariably be earthed adequately to prevent electrical shock, sparking, shortcircuit etc. power cord to be used shall be of adequate capacity, without any joint & shallconsist of earth wire also. Hence, it is necessary to use adequate capacity 3-wire power cord<strong>for</strong> single & 5-wire power cord <strong>for</strong> three phase power connections. The plugs, receptacles,pins, holders etc shall be of adequate capacity & safe to use.All electrical & mechanical equipments / tools-tackles viz. welding machine, cuttingmachine, grinder drill, chain pulley blocks, hook chooks etc required to be used during workexecution shall be of standard make & bear ISI certification mark on it. The consumables likewelding electrodes, grinding wheels / discs etc which has specific prescribed life span shall notbe used in any case if its expiry date is over.(07)It is compulsory to use standard make personal protective equipments (P.P.Es.) as per the jobrequirement. Do not work without use of required P.P.Es contractor is responsible to providestandard make (ISI approved) personal protective equipments / safety gadgets suitable to givesufficient protection against hazards involved in their work / job to their staff, as per the job totheir staff, as per the job requirement and insist / en<strong>for</strong>ce their work / job to their staff, as perthe job requirement and insist / en<strong>for</strong>ce their staff to put on the same while at works.The Ongoing work is liable to be stopped at any time if the contractors’ staff is found workingwithout P.P.Es. Bs., Hook chooks, winch, <strong>for</strong>klift, mobile crane,EOT crane etc.) & its attachments (like D-shackles, slings, U-clamps, Eye bolts or any fixtures),it shall be checked and used only if found safe to use. Also, ensure that these are tested,examined & certified in <strong>for</strong>m no.9 / 10 by competent person as per the Factory act-1948 and itsvalidity is not expired. Further, it shall be fixed properly and firmly prior to lifting the weight.(09) Scaffoldings to be used <strong>for</strong> working at height shall be adequate size & capacity. Obtain thework permit when working at height. While climbing on such scaffolding or working on anystructure at height, use of full body safety belt & helmet is compulsory. It is also necessary tofasten chinstraps of the helmet.- 18 -


(10) Contractor or their employee shall not interfere in day-today routine plant activities / worksexcept the work assigned to them, shall not loiter in the areas other than their workjurisdictions, as well as shall not temper / operate / touch the machineries / equipments /auxiliaries with which they are not concerned. Also, the contractor shall strictly instruct theirstaff not to sit or take rest at / near / below running plants, auxiliaries, systems or any placewhich is risky, hazardous & prone to accident.(11) The cylinders containing poisonous / toxic or inflammable / explosive gas like oxygen,acetylene, LPG, hydrogen, ammonia, chlorine, co 2 shall be handled safety taking due care. Tohandle / shift such cylinders a special trolley / cage meant <strong>for</strong> it must be used but in no case itshould be rolled. Domestic LPG cylinder shall not be used / permitted.(12) On completion of the work, cotton waste, spilled oil / grease, pieces of welding rod & otherwaste material shall be removed from work site and the area shall be left safe, neat & clean.(13) In case of any injury / accident while working, it shall immediately be reported to safetydepartment through concerned sectional head / Engineer. The prescribed from no.21 may beobtained from concerned section or safety officer.For any incident occurred but have no injury to any persons should also be in<strong>for</strong>med to safetyofficer as “Near Miss Incident.”(14) In all risky jobs, be<strong>for</strong>e starting work, contractor should obtain general safety work permit fromconcerned section well in advance.(15) In case of notising smoke or fire during their work execution, they shall make immediate ef<strong>for</strong>tsto extinguish / control it and simultaneously in<strong>for</strong>m the fire brigade on phone no.4444 or 4344or 5555 or 02624-290049,shall shift the casualty to nearby hospital after rendering first aid incase of accident.(16) Over & above these, contractor shall have to follow all the safety requirements / rules &regulation / norms and legal provisions laid down in various statues, particularly the provisionsof The factories act-1948 & The <strong>Gujarat</strong> states factories rules-1963 (Amended up to date)shallbe followed strictly. The contractor shall also obey the rules / regulations / instructions of thelocal competent authority <strong>for</strong> safety requirements.(17) No women or young person shall be allowed to clean, lubricate or adjust any part of a primemover or of any transmission machinery while the prime mover or any transmission machineryis in motion. Examination or operation of motion machinery shall be made or carried out onlyby a specially trained adult male worker wearing tight fitting clothing as per section 22 offactories act 1948.(18) No women or young person shall be employed or permitted to work in lead-compound area likebattery room etc.as per schedule VI of GFR 1963.(19) All the relevant labour and industrial laws shall also be followed compulsorily.(20) It is felt necessary to deploy safety officers / safety supervisors by contractors / agenciesinvolved in carrying out hazardous activities / operations inside TPS to have better andconstant supervision in terms of health safety and environment activities.(21) in case, it comes to the notice of GSECL UTPS management that the safety guidelines / safetyrules / safety norms are not being followed then a minimum sum of ropes one thousand shallbe penalized on the contractor / Agency and <strong>for</strong> subsequent such violation, a server penalty /action as deemed fit shall be imposed, which may please be noted.(22) The above rules shall be scrupulously followed and where required, they may contact thesafety officer <strong>for</strong> any ambiguity / further guidance in this regard.(23) For per<strong>for</strong>mance evolution of contractor, safety factors of work accident, fire incident & nearmiss accident will be considered.
