We will only share information with those who needto know in order to provide good quality care. Occasionally theremay be exceptions to this, for example to support a clinical audit orto monitor the quality of care provided. We will usually discuss thiswith you beforehand.attention deficit hyperactivity disorder (ADHD)childhood cancersnasogastric tube feeding (through the nose)gastrostomies (tube feeding through the tummy)palliative care and end-of-life carebereavement support.What is the Children’s <strong>Community</strong> <strong>Nursing</strong> <strong>Team</strong>?The Children’s <strong>Community</strong> <strong>Nursing</strong> team consists of specially trainedchildren’s nurses and continuing care assistants. We provide expertnursing care and support to children and young people in their ownhome or community.We have a Demelza Nurse as part of the team who provides specialistsupport for palliative care and symptom control and end-of-life care.We are supported by a team administrator.SmokingWe would appreciate it if you do not smoke within your house justbefore and during the nurse’s visit.


What type of care do we provide?Skilled care - if your child or young person requires proceduressuch as injections, wound care, post surgery and orthopaediccare.Symptom control management - if your child or young personrequires a period of regular assessment and monitoring ofsymptoms, for example pain control in children requiringpalliative or end of life care.Help to manage long term conditions such as asthma, diabetesand epilepsy.Support to oxygen dependent babies and children with complexhealthcare needs.Education - if the family or carer requires training to enable themto continue providing support and ongoing care, for exampleenteral (through the nose or tummy) feeding, oxygen therapy andadministration of medicines.Or any other nursing procedure that can be safely undertaken athome.We work in partnership with the child and family and plan caretogether. We will make every effort to book visits in advance at aconvenient time. Occasionally delays occur and if the delay isexpected to be over 30 minutes we will contact you.Who can refer to the team?Anyone can make a referral. When a referral is accepted theservice will inform the child’s GP and other professionals involved.Some children may have needs that the service cannot meet, inwhich case they will be referred to the most appropriate service.Safeguarding childrenWe have a duty to ensure that children are safe and well cared for,both where the child is a service user and when their parent, careror other person in the child’s family or network is a service user. Weroutinely consider the needs of children as part of our assessmentsand ongoing work. We work closely with other professionalsinvolved with supporting families where extra help or support isneeded.
