Bring Someone Special to LunchOn Wednesday, October 16 th our Kindergarten students will have the opportunityto “Bring Someone Special to Lunch”. <strong>The</strong> invitations for this special day arebeing sent home with the children today. In order to help us plan for this event,please be sure to respond by the due date. Over the course of the school year eachgrade will be given the opportunity to “Bring Someone Special to Lunch”.Fundraising RequirementAll school families of students in Pre-School through 8 th grade are required toparticipate in the school fundraising program, the SHOPPING CART. If yourfamily has decided not to purchase gift cards through this program, your $300.00fundraising buy-out is due by October 1 st . After October 1 st , the buy-out amountwill be $350.00. DIOCESE OF ALLENTOWNSTUDENT AND ATHLETIC ACCIDENT PROGRAM - <strong>2013</strong>-2014ELIGIBILITYAny student who attends day care, pre-kindergarten, kindergarten, elementary or secondary school is eligible. Eligibility shallalso extend to CCD students for student activities and CYO participants for scheduled events.COVERAGE UNDER STUDENT ACCIDENT PLANThis plan provides coverage for all eligible students for Injury during the hours and days when school is in session and whileattending or participating in school sponsored and supervised activities on or off the school premises. This includes participationin interscholastic sports including interscholastic football, religious services, summer recreation activities, travel to and fromschool and other necessary travel.DEFINITIONSUSUAL AND CUSTOMARY - means in no event shall the Company’s payment for medical or surgical procedures exceed theUsual and Customary charges which in the Company’s experience are normally made by the majority of physicians in that area.INJURY- bodily injury resulting directly and independently of all other causes from a specific accident.HOSPITAL - an institution legally operated and approved as a hospital by the responsible state agency. It must: (a) be primarilyengaged in providing medical care and treatment to sick and injured persons on an inpatient basis for which a charge ismade; and (b) provide 24 hour nursing service by or under the supervision of a registered graduate professional nurse (RNs).It does not include convalescent, rest or nursing facilities or facilities for drug and alcohol rehabilitation, custodial or educationalcare.OTHER INSURANCE PLAN - shall mean any plan providing dental or medical expense benefits for or by reason of dental,physician, nurse, hospital care, treatment, or confinement, or the performance of surgery and/or anesthesia, which benefits areprovided by; 1) any type of Blue Cross or Blue Shield plan, Health Maintenance organization or similar organization, any individual,group, blanket, or franchise insurance, employee benefit plan or any plan arranged through an employer, trustee, union,Health Maintenance organization or employee benefit association; or 2) any plan or program created or administered by nationalor state government, or agencies thereof.BASIS OF BENEFITSUnder this plan, the first $100 of covered charges are paid without applying any deductible or collectible benefits under OtherInsurance Plans.$5,000,000 STUDENT ACCIDENT MEDICAL EXPENSE BENEFITSBenefits will cover the Usual and Customary expenses for the medical services listed below. Treatment must commence within <strong>26</strong> weeks of the date ofthe accident and benefits are payable for up to <strong>26</strong>0 weeks from the date of the accident.EXCLUSION AND LIMITATIONSThis policy does not cover: (a) service or treatment rendered as part of the school services by a physician or any other personemployed or retained by the policyholder; (b) any active participation in a riot, terrorist act, insurrection or war; either declaredor undeclared; (c) hernia; (d) self inflicted Injuries, suicide or attempt thereat; (e) vegetation or ptomaine poisoning if resultingfrom an accident or external wound; (f) bacterial infections ( except pyogenic infections due to accidental open cuts); (g) congenitaldefect; (h) Injuries sustained as a result of skiing or snowboarding, unless sponsored, scheduled and supervised by thepolicyholder; (i) Injury or Sickness for which Workers’ compensation or similar occupational benefits are available; (j) any losssustained or contracted in consequence of the insured’s being intoxicated or under the influence of any narcotic unless administeredon the advice of a physician; (k) Injuries sustained as a result of operating, riding in or on, entering into, alighting fromor being struck by a motorized engine driven 2, 3 , or 4 wheel vehicle, go-cart, dune buggy, all terrain vehicle, snowmobile orsimilar vehicle; (l) Injuries sustained as a result of practice or participation in tackle football in any form, except for Injuries whichoccur during practice or participation in interscholastic tackle football which is sponsored and scheduled by, and under thesupervision and control of the master policyholder; (m) Dental Implants.CLAIMS PROCEDURESIn the event of an Injury: 1. Notify the school/parish immediately; 2. Secure a claim form from the school/parish;3. Complete the claim form, sign the claim form in all necessary places and remit with itemized bills to the ClaimsAdministrator, Bollinger Inc., P.O.
