Reports on <strong>impairment</strong> need to provide sufficient detail and clarity on <strong>the</strong> <strong>assessment</strong> methodologyand findings obtained. For this reason <strong>the</strong> approved template is to be used when preparing reports and isincluded in Appendix 2 <strong>of</strong> <strong>the</strong>se <strong>Guidelines</strong>.
