The objectives are:• Improvement in population outcomes in South Londonfor HbA 1cmanagement• Number of South London CCGs which achieve, maintainor increase LPP thresholds on % non-analogue insulin(long and intermediate acting) prescribing• Number of South London Medicines Optimisation teamswith robust plans for implementation of the projectThis section focuses on how intelligent commissioning cansupport the first two outcomes and drive change at a patient,practice / service and population level.Commissioning of diabetes services is often undertaken on amulti-tiered approach 90 and different models exist across SouthLondon.
