General Information
The registration fees are detailed on the Registration form which can be found at the back of this booklet.
The registration fee for Advanced Cardiovascular Intervention 2011 include:
> Admission to all Scientific Sessions & Angio Review Sessions*
> Admission to the lunchtime breakout sessions on Friday 28th January
> Admission to the Exhibition
> Use of the Internet Café
> Refreshments and Lunch on each day
> All materials relating to the Conference
(*Please note that you will be allocated a place within a designated angio review session. It is imperative
that you do not switch groups due to strict regulations regarding Health & Safety room numbers).
Places for the Gala Dinner on Thursday 27th January can be booked in advance. However, there will
be an opportunity to book on the day before 14:00 hours.
PAYMENT
Payment must be in Sterling by cheque, bank draft or transfer, VISA or Mastercard and sent together with
your completed registration form to Millbrook Medical Conferences Ltd.

CANCELLATION OF REGISTRATION
The registration fee paid will be refunded less 25% (to cover the cost of administration) should delegates
cancel their registration prior to Wednesday 11th January 2011.
No refund will be made for cancellations after this date. If requested, conference material will be sent to
delegates who have paid the registration fee.
