<strong>Instructions</strong> <strong>for</strong> <strong>Textbook</strong> <strong>Request</strong> <strong>Form</strong><br />

<strong>*We</strong> <strong>do</strong> <strong>not</strong> <strong>purchase</strong> <strong>online</strong>, compressed video, wisline, or distant education<br />

textbooks.<br />

If you would like a new or used textbook(s) <strong>for</strong> classes you are teaching face to<br />

face or ITV at UWBC, then, please fill out the <strong>for</strong>m on Page three and submit<br />

<strong>Textbook</strong> <strong>Request</strong> <strong>Form</strong> to the department head <strong>for</strong> approval. The department<br />

head will approve or deny the request(s).<br />

If the request is approved, then, Department head is responsible <strong>for</strong>:<br />

enough money in department budget<br />

request is printed out<br />

signed by the department head<br />

Put signed textbook request <strong>for</strong>m in Linda Snider’s mailbox<br />

We try to <strong>purchase</strong> the textbook(s) as low as price possible, but in good condition.<br />

This means we may <strong>purchase</strong> used books that are missing CDs or other<br />

supplemental materials. If you require CDs or supplemental materials with the<br />

textbook please write this on textbook <strong>for</strong>m with as much in<strong>for</strong>mation as<br />

possible or photocopy.<br />

Instructors should request their own Instructor textbooks, test<br />

banks or other materials, so you receive exactly what you need, to<br />

teach your classes. Often, Publishers have power point slides <strong>for</strong><br />

lectures, supplemental materials, quizzes, DVDs, publisher website<br />

<strong>for</strong> instructors and students.<br />

Page 2 shows an example of a completed textbook <strong>for</strong>m except <strong>for</strong> signatures.
