Instructions for Textbook Request Form
*We do not purchase online, compressed video, wisline, or distant education
textbooks.
If you would like a new or used textbook(s) for classes you are teaching face to
face or ITV at UWBC, then, please fill out the form on Page three and submit
Textbook Request Form to the department head for approval. The department
head will approve or deny the request(s).
If the request is approved, then, Department head is responsible for:
enough money in department budget
request is printed out
signed by the department head
Put signed textbook request form in Linda Snider’s mailbox
We try to purchase the textbook(s) as low as price possible, but in good condition.
This means we may purchase used books that are missing CDs or other
supplemental materials. If you require CDs or supplemental materials with the
textbook please write this on textbook form with as much information as
possible or photocopy.
Instructors should request their own Instructor textbooks, test
banks or other materials, so you receive exactly what you need, to
teach your classes. Often, Publishers have power point slides for
lectures, supplemental materials, quizzes, DVDs, publisher website
for instructors and students.
Page 2 shows an example of a completed textbook form except for signatures.
