This form allows you to instruct Macquarie Equities Limited (MEL)
and/or Macquarie Bank Limited (MBL) to pay your nominated financial adviser the service fees specified below, as client directed payments, relating to your
Macquarie Prime Facility. You may terminate or change this direction at any time by giving us at least ten business days notice in writing.
Please complete this form using BLACK INK and print well within the boxes in CAPITAL LETTERS.

amounts of these services.
If there is insufficient available cash in my account to pay the services fees I/we authorise MBL to advance any shortfall to me/us under the terms
of my/our Macquarie Prime Facility to allow the payment to be made to my/our Adviser.
Please note that your elected payment is subject to and inclusive of GST on share trade fees only, and payment elected on other
applicable transactions are not GST inclusive.
