<strong>How</strong> <strong>to</strong> <strong>Apply</strong> <strong>for</strong> <strong>Corneal</strong> <strong>Refractive</strong> <strong>Surgery</strong> <strong>at</strong> <strong>the</strong><br />

<strong>Warfighter</strong> <strong>Eye</strong> Center , Joint Base Andrews<br />

1. We want you <strong>to</strong> be well in<strong>for</strong>med about refractive surgery prior <strong>to</strong> undergoing an evalu<strong>at</strong>ion.<br />

Please be aware of <strong>the</strong> following highlighted items from <strong>the</strong> briefing:<br />

a. You must be <strong>at</strong> least 21 years of age on active duty st<strong>at</strong>us <strong>to</strong> be eligible<br />

b. If you are Air Force, you must have <strong>at</strong> least 6 months remaining on (active duty) from day of<br />

surgery, not from <strong>the</strong> time of applying <strong>for</strong> surgery. We recommend you allow a couple of<br />

months additional of (AD st<strong>at</strong>us) <strong>for</strong> processing your CRS packet and <strong>to</strong> be booked <strong>for</strong> your<br />

pre op. Required time <strong>for</strong> o<strong>the</strong>r branches-(12 months Navy and Marines/ 18 months Army)<br />

c. Avia<strong>to</strong>rs from o<strong>the</strong>r services are NOT eligible <strong>for</strong> tre<strong>at</strong>ment here. This is available <strong>at</strong><br />

Walter Reed Medical Center.<br />

d. Local Avia<strong>to</strong>rs and Special Duty career fields must obtain additional approval from <strong>the</strong> Aero<br />

Medical Consult Services prior <strong>to</strong> proceeding with surgery. This will be accomplished<br />

through <strong>the</strong> <strong>Warfighter</strong> <strong>Eye</strong> Center after your pre- op exam.<br />

2. You will need <strong>to</strong> turn in (5) documents <strong>to</strong> our center- Incomplete Packets will NOT be<br />

processed.<br />

2. Commander’s Authoriz<strong>at</strong>ion<br />

3. Managed Care Agreement- if you will obtain post- surgical care <strong>at</strong> a loc<strong>at</strong>ion o<strong>the</strong>r<br />

than Joint Base Andrews, please contact your local op<strong>to</strong>metrist <strong>to</strong> complete <strong>the</strong><br />

bot<strong>to</strong>m portion of this <strong>for</strong>m.<br />

4. <strong>Warfighter</strong> <strong>Refractive</strong> <strong>Surgery</strong> In<strong>for</strong>m<strong>at</strong>ional Briefing Form<br />

5. A copy of an eye glass prescription or exam th<strong>at</strong> is a 1 year or older (Please include<br />

this with all <strong>the</strong> <strong>for</strong>ms above)<br />

Once <strong>the</strong> above <strong>for</strong>ms have been received, you will be scheduled <strong>for</strong> a pre – oper<strong>at</strong>ive evalu<strong>at</strong>ion<br />

<strong>at</strong> <strong>the</strong> <strong>Warfighter</strong> <strong>Eye</strong> Center <strong>at</strong> Malcolm Medical Clinic.<br />

You must be out of your contact lenses <strong>for</strong> <strong>at</strong> least 30 days (<strong>for</strong> soft lenses) and 90 days<br />

(<strong>for</strong> hard lenses).<br />

MOST FREQUENT ASKED QUESTIONS<br />

1. If you are not a flyer you do not need <strong>to</strong> fill out <strong>the</strong> left portion of <strong>the</strong> Applic<strong>at</strong>ion Form. This<br />

portion is <strong>for</strong> avia<strong>to</strong>rs only.<br />

2. You need TWO sign<strong>at</strong>ures on <strong>the</strong> Commander’s Authoriz<strong>at</strong>ion Form( Commander and ei<strong>the</strong>r<br />

a Mobility or Supervisor Sign<strong>at</strong>ure, or all THREE sign<strong>at</strong>ures.<br />

3. On <strong>the</strong> Briefing Sheet- <strong>the</strong> d<strong>at</strong>e <strong>at</strong> <strong>the</strong> <strong>to</strong>p is <strong>the</strong> d<strong>at</strong>e you viewed <strong>the</strong> slides on our website it<br />

DOE NOT need a doc<strong>to</strong>rs sign<strong>at</strong>ure.<br />

4. PSP or PRP only needs <strong>to</strong> be circled by AF.<br />

5. Avi<strong>at</strong>ion AASD CRS Applic<strong>at</strong>ion Form- only needs <strong>to</strong> be filled out by an op<strong>to</strong>metrist only if<br />

you do not have a copy of a 1 year or older eye exam.


****MOST FREQUENT REASONS YOUR APPICATION PROCESS WILL BE DELAYED<br />

AND NOT PROCESSED****<br />

1. DOS (D<strong>at</strong>e of Separ<strong>at</strong>ion) does not m<strong>at</strong>ch on ALL <strong>for</strong>ms<br />

2. You must have TWO sign<strong>at</strong>ures on <strong>the</strong> Commanders Authoriz<strong>at</strong>ion Form<br />

3. You are missing your 1 year or older eye exam<br />

4. You must fill out every portion of ALL <strong>the</strong> <strong>for</strong>ms- blank spaces are not accepted<br />

5. Complete <strong>the</strong> entire portion of <strong>the</strong> Applic<strong>at</strong>ion Form which includes initialing and signing <strong>at</strong> <strong>the</strong><br />

bot<strong>to</strong>m<br />

6. If you are a local you and you are planning on having your post op care here, you only need <strong>to</strong> fill<br />

out <strong>the</strong> <strong>to</strong>p portion of <strong>the</strong> Manage Care Form. If you are an out of <strong>to</strong>wner both portions are <strong>to</strong> be<br />

filled out <strong>the</strong> <strong>to</strong>p by you and <strong>the</strong> bot<strong>to</strong>m by <strong>the</strong> doc<strong>to</strong>r th<strong>at</strong> will be doing your post-oper<strong>at</strong>ive care.<br />

7. If you are a flyer- you MUST provide your AFSC and ASC in <strong>the</strong> portion asked<br />

8. You MUST circle <strong>the</strong> question YES or NO is you are deploying or PCS’n within <strong>the</strong> next 6<br />

months
