Our Strategic Objectives<br />

Our new objectives set our ambitions for the life of our strategy; help<strong>in</strong>g<br />

us to meet the challenges for the NHS over the next five years; provid<strong>in</strong>g<br />

safe, effective care that improves outcomes, delivered by a well engaged,<br />

diverse and skilled workforce, <strong>in</strong> an efficient, timely and accessible way.<br />

We will explore our objectives <strong>in</strong> more detail over the com<strong>in</strong>g months.<br />

Our Service Model<br />

As a large, <strong>in</strong>tegrated healthcare provider, we offer a range of services at<br />

different levels of specialism and <strong>in</strong>tensity, across a range of sett<strong>in</strong>gs, which<br />

<strong>in</strong>clude people’s own homes, community cl<strong>in</strong>ics and specialist hospitals.<br />

By focus<strong>in</strong>g on person centred, co-ord<strong>in</strong>ated care, our service model<br />

ensures that people receive these services <strong>in</strong> a more <strong>in</strong>tegrated and coord<strong>in</strong>ated<br />

way, shift<strong>in</strong>g the emphasis from hospital and <strong>in</strong>patient care, to<br />

support<strong>in</strong>g people to live <strong>in</strong>dependently <strong>in</strong> their own homes.<br />

We can achieve this through:<br />

• Greater use of technology, provid<strong>in</strong>g <strong>in</strong>formation and knowledge to<br />

support lifestyle changes; help<strong>in</strong>g people to understand the impact of<br />

their lifestyle on their health condition.<br />

• Improv<strong>in</strong>g the shar<strong>in</strong>g of <strong>in</strong>formation and engagement of people with<strong>in</strong><br />

care plann<strong>in</strong>g, work<strong>in</strong>g with others to improve outcomes.<br />

• Simplify<strong>in</strong>g and speed<strong>in</strong>g up access to our services. We have set out an<br />

ambitious plan to develop a truly s<strong>in</strong>gle po<strong>in</strong>t of access across our <strong>Trust</strong>, to<br />

help people receive personalised support for physical and mental health<br />

needs, with care organised by a s<strong>in</strong>gle, named co-ord<strong>in</strong>ator.<br />

This is a challeng<strong>in</strong>g strategy that we will<br />

review every year, but we are clear that<br />

these rema<strong>in</strong> the right ambitions for our<br />

<strong>Trust</strong>, our service users, patients, carers,<br />

volunteers, staff and our partners, to<br />

improve lives and the quality of care.<br />

Annual General Meet<strong>in</strong>g<br />

(AGM) and Annual<br />

Members’ Meet<strong>in</strong>g<br />

(AMM) 2016<br />

The <strong>Trust</strong>’s AGM/AMM will take place<br />

on Friday 22 July, 2.00pm-3.30pm at<br />

Duncan Macmillan House, Nott<strong>in</strong>gham <strong>in</strong><br />

Conference Rooms 1 and 2. We will be<br />

look<strong>in</strong>g back over the previous year and<br />

there will be an opportunity for you to ask<br />

questions. If you plan to attend and have<br />

You may have picked up this copy of the newsletter not know<strong>in</strong>g<br />

what Nott<strong>in</strong>ghamshire Healthcare is. We provide <strong>in</strong>tegrated healthcare<br />

services <strong>in</strong>clud<strong>in</strong>g mental health, <strong>in</strong>tellectual disability and community<br />

health services. We also manage medium secure units <strong>in</strong> Leicester and<br />

Rotherham, and the high secure Rampton Hospital near Retford and<br />

provide healthcare <strong>in</strong> prisons across the East Midlands and Yorkshire.<br />

Forensic Services, Health<br />

Partnerships and Local Services<br />

In this newsletter you will see references<br />

to Health Partnerships, Forensic Services<br />

and Local Services. These refer to the<br />

<strong>Trust</strong>’s three operational Divisions. This<br />

means, the way <strong>in</strong> which the services<br />

we provide are structured and managed<br />

<strong>in</strong> the <strong>Trust</strong>.<br />

Briefly, Health Partnerships is the<br />

Division that <strong>in</strong>cludes the physical<br />

healthcare community services we<br />

provide. The majority of these are<br />

delivered <strong>in</strong> patients’ own homes,<br />

but also <strong>in</strong> community sett<strong>in</strong>gs, care<br />

homes and hospitals. They <strong>in</strong>clude<br />

for example the management of long<br />

term conditions, health visit<strong>in</strong>g, school<br />

nurs<strong>in</strong>g, foot care, physiotherapy and<br />

hospice care.<br />

Local Services is the Division that<br />

past editions of Positive, or if<br />

you are hav<strong>in</strong>g any ‘distribution<br />

issues’ with the newsletter –<br />

whether you’re receiv<strong>in</strong>g too<br />

provides our mental health, <strong>in</strong>tellectual<br />

and development disability and<br />

substance misuse services. These<br />

services are for people of all ages<br />

and are provided <strong>in</strong> the community,<br />

outpatient and <strong>in</strong>patient sett<strong>in</strong>gs.<br />

The Forensic Services Division<br />

provides assessment and treatment to<br />

<strong>in</strong>dividuals with a mental disorder who<br />

have committed, or are at serious risk<br />

of committ<strong>in</strong>g, a crim<strong>in</strong>al offence, and<br />

are likely to cause serious physical and/<br />

or psychological harm to themselves or<br />

others. These people are cared for <strong>in</strong><br />

secure hospitals and <strong>in</strong> the community.<br />

The Division also <strong>in</strong>cludes our Offender<br />

Health teams which provide physical<br />

and mental healthcare <strong>in</strong> prisons.<br />
