Welcome! I am excited and pleased to offer you the first edition<br />
of Q Magazine. It’s bold, it’s beautiful, and it showcases many<br />
of the exciting new treatments and services our talented<br />
surgeons are offering. 2016 is a fascinating year in the cosmetic<br />
surgery field, and I think it’s the perfect time to offer this<br />
magazine to our patients. Why? Because new beauty<br />
treatments are coming out every day, many with some pretty<br />
hefty claims, and as consumers, we have no idea what works<br />
and what doesn’t. But we are very fortunate to have worldrenowned<br />
surgeons right here in Rochester, NY, and I can’t think<br />
of a better go-to resource for this type of information. Educating<br />
patients is paramount to our doctors, and if you haven’t been<br />
privy to their knowledge before, read through this magazine and<br />
you will clearly see that they are authorities on everything beauty<br />
related.<br />
You may be wondering why I’m writing this prologue. Outside of the fact that it might just be a job<br />
responsibility of mine, it’s honestly because I believe in this practice. I have worked at the Quatela Center<br />
for over 10 years and have had the honor of having Drs. Quatela and Koenig as my bosses. Yes, you read<br />
that correctly, the honor of working for them. I consider it a privilege. They are first-class surgeons,<br />
buttoned-up in every way, and they have the ability to flawlessly execute almost everything they do. They<br />
have gained international attention and respect from patients and peers alike and that is in large part due to<br />
an unwavering commitment to quality care and a hard work ethic. Most importantly, they deliver consistent,<br />
remarkable results to patients that not only improve their outward appearance, but change their lives.<br />
So what is one of the most important lessons I’ve learned so far during my time at the Quatela Center?<br />
That’s a no-brainer. It’s the definition of beauty (or lack thereof). I have learned that beauty is defined by<br />
each individual and not by society, celebrities or surgeons. It’s unique to each of us and something that is<br />
our own and no one else’s.<br />
As a mother of two, my definition of beauty now is much different than how I defined it as a twenty-year-old.<br />
Having children took a major toll on my body, and trust me when I say I didn’t take those changes lightly.<br />
Since then, I’ve become wiser, stronger and more confident in the woman I am, and frankly, I find that I am<br />
my most beautiful self yet. As women, we need to embrace where we are in life and focus on the positive<br />
aspects that make us uniquely beautiful (men, this applies to your confidence as well!). And if there are<br />
updates or small nips and tucks that need to be made along the way to help you boost your confidence, then<br />
by all means, go ahead and find your beautiful. You deserve it!<br />

ABOUT DR. VITO QUATELA<br />
board certification means that Dr. Quatela has completed education and training for<br />
both facial plastic surgery (certified by the American Board of Facial Plastic and<br />
Reconstructive Surgery) and otolaryngology (certified by the American Board of<br />
Otolaryngology – Head and Neck Surgery). He also served as the past president of<br />
the AAFPRS (American Academy of Facial Plastic and Reconstructive Surgery) in 2007,<br />
a high honor for facial plastic surgeons who reflect the Academy in their surgical<br />
excellence and high quality of patient care.<br />
Dr. Quatela’s overall mission is to improve people’s lives by aesthetically<br />
enhancing their appearance and increasing their confidence. A lot of patients will<br />
ask what signs of aging in their face or facial structure is most bothersome to him,<br />
however he is most interested in what each patient is concerned with because<br />
a patient’s perception of what they see in the mirror is the most important.<br />

Rhinoplasty,<br />
or nose surgery, is considered a challenging operation, and Dr. Quatela is<br />
known as one of the pioneers of modern rhinoplasty. Most of his years of experience<br />
have been spent perfecting his technique which allows him to achieve patient satisfaction<br />
very reliably. He is an author and educator on rhinoplasty and has taught several<br />
generations of surgeons how to be proficient. Dedicated to the training of other plastic<br />
surgeons, Dr. Quatela has conducted in excess of 100 medical lectures over his career.<br />
This has resulted in his teaching surgeons in countries all over the world including Turkey,<br />
Italy, France, Germany, and Switzerland.<br />

Serving as a preceptor for an annual AAFPRS Fellowship, Dr. Quatela trains and<br />
mentors doctors who are completing their training in plastic surgery. Each year he<br />
hosts a fellow at his practice, and has trained 20 doctors since 1995.<br />
Dr. Quatela is considered an authority on facial plastic surgery and has been quoted by<br />
popular media outlets such as W Magazine, Wall Street Journal, The New York Times,<br />
Los Angeles Times, The Washington Post, and ABC News.<br />
5The Lindsay House, previously owned by the Lindsay family and then the Rochester<br />
Health Association, was in dramatic disrepair when Dr. Quatela purchased it in 1992.<br />
He took it upon himself to oversee all decisions in its restoration and carefully<br />
ensured that the original beauty and architecture of the home was maintained. Dr.<br />
Quatela’s passion for restoration extends into every aspect of his life, whether it<br />
is in the meticulous restoration of historic buildings or his thoughtful consideration of<br />
detail and balance in his cosmetic surgery procedures.<br />

Dr. Quatela has a love and compassion for children, especially those who cannot afford<br />
medical care and who suffer from a deformity or conditions that can be remedied through<br />
plastic surgery. One such deformity is microtia, where children are born without an ear or a<br />
portion of the ear, and another is cleft lip and palate abnormalities. For this reason, Dr.<br />
Quatela formed the HUGS (Help Us Give Smiles) Foundation and currently takes<br />
three medical mission trips a year. On these trips his team of volunteer surgeons and<br />
nurses provide surgery to children, repairing cleft lips and palates and forming ears out of<br />
rib cartilage, changing their lives.<br />
Growing up in Rochester, New York, Dr. Quatela is a graduate of St. Thomas Aquinas<br />

and the University of Rochester. Dr. Quatela loves experiencing concerts of all<br />
musical genres and enjoys the outdoors by spending time fly fishing. He often enjoys<br />

national averages, due in part to his use of the<br />

Sometimes patients are looking for cosmetic procedures to be an end-all solution to their<br />
insecurities about a certain part of their appearance. However, at the Quatela Center we do<br />
not view plastic surgery as an easy fix or isolated experience.<br />
Patients are encouraged to be actively involved in the entire process, and it’s mutually<br />
understood that choosing to have cosmetic treatments or surgery is a big decision for most.<br />
Since this decision should not be taken lightly, Drs. Koenig and Quatela ensure that patients<br />
are making the choice for themselves and are committed to that choice.<br />
Assessing if a patient is ready for cosmetic treatments has a lot to do with the patient’s<br />
motivation. Have they thought about this for a long time? Are they motivated by their own<br />

A facelift is an effective and safe procedure performed to restore a more<br />
youthful appearance. Just as our faces are unique to each of us, surgery is<br />
customized to each individual.<br />
The overall goal is to address facial aging, concerns in the mid<br />
to lower face, and the loss of muscle tone. With so many<br />
options available for patients today when it comes to facial<br />
rejuvenation, it’s important to do your homework. Below are<br />
five frequently asked questions about facelift surgery.<br />

1. What is the best procedure for firming the neck and<br />
chin?<br />
A number of different treatments can be used for rejuvenation<br />
of the neck and chin. For the removal of excess fat beneath<br />
the skin, Kybella (which can dissolve fat over the course of<br />
several treatments) or PrecisionTx (a method of laserassisted<br />
liposuction that enhances skin tightness) are<br />
common less-invasive treatments. For skin tightening,<br />
Ultherapy, a noninvasive method of collagen building and skin<br />
tightening, works well both as a treatment and a preventative.
Also a lower facelift can help significantly for the<br />
chin and neck. The name “facelift” may be<br />
misleading, but it’s important to know that a facelift<br />
addresses the lower third of the face, tightening the<br />
skin of the lower face and neck.<br />
2. Is a facelift the only answer for lifting jowls?<br />
There are a number of different ways to address<br />
jowling with aging. As we age, the soft tissue and<br />
skin of the lower face begin to descend. Fillers are<br />
temporary treatments which can work for early and<br />
milder jowling. Hyaluronic acids such as<br />
Juvederm® and Restylane® are a good option.<br />
More invasive options are surgical. A lower facelift<br />
is the best and most long-lasting treatment for<br />
jowling. Done well by a skillful surgeon, a facelift<br />
can last 10 years or more.<br />

3. What should patients look for in facelift before and after photos?<br />
In reviewing a surgeon’s portfolio of before and after photos, there are a number of important factors to<br />
notice. The photos should be standardized; the lighting, distance and angles of the photos taken for before<br />
and after should be consistent. If evaluating facelift results, the hair should be pulled back from the face so<br />
as not to distract or obscure any of the facial anatomy. The patient should not be wearing distracting<br />
jewelry or excessive makeup.<br />
It’s also important to notice head position. The head should not be tilted up or down but aligned with the<br />
horizontal plane of the photo. This ensures the chin is in a neutral position. If the chin is pointed up, this<br />
can create an artificially more youthful look by stretching out the neck skin. Similarly if the chin is pointed<br />
down, this can accentuate wrinkles in the lower face and neck.<br />
4. What is a weekend facelift?<br />
A weekend facelift is another name used for a mini facelift. A mini facelift addresses isolated problems of<br />
the lower face, but cannot improve aging as well as the full facelift. The major difference is that the results<br />
from a mini lift are more subtle since the muscle layer is not elevated, giving the result less longevity.<br />
5. How long should swelling and bruising last after a facelift?<br />
Typically after a lower facelift/neck lift alone, the majority of swelling and bruising should subside by 10-14<br />
days. It is not unusual for initial swelling to last up to 3-4 weeks in some patients. Swelling and bruising<br />
can be decreased with the use of ice or some herbal supplements, however it’s important to speak to your<br />
doctor about when and how to use these. Dr. Quatela considers a patient fully healed from facelift surgery<br />
after 6 months.<br />

Options for fat removal are constantly evolving, and many patients are looking to utilize the<br />
newest technology. Desirable attributes of a liposuction procedure are for the least amount of<br />
downtime and fast, noticeable results. However, not all patients are candidates for every<br />
procedure.<br />
At the Quatela Center, Dr. Koenig offers all available options for liposuction which<br />
include laser, ultrasonic, and tumescent. Read on to learn more and help clear up the<br />

We sat down with Dr. Quatela for an in-depth look at rhinoplasty, or nose reshaping, surgery. As<br />
a pioneer of modern rhinoplasty, Dr. Quatela has dedicated most of his 27 years of experience<br />
perfecting his technique to achieve uncompromising results. Here’s what he had to say about<br />
this popular procedure.<br />
Q: Who is the ideal candidate for rhinoplasty surgery? A: The ideal candidate for a cosmetic<br />
rhinoplasty is someone who is properly motivated and has thought about the surgery for a long time. The<br />
motivation to do this comes from within them, and that what they see in the mirror is what you see in the<br />
mirror, so they are realistic. I think that the more specific a patient is with their nasal conflict, for example<br />
they address that the tip is too wide or they have a bump on their nose, the better patient they are. The<br />
more nonspecific and vague, “I just want my nose to look better,” “I want it to look beautiful,” “I want to look<br />
like so and so,” the less of an ideal candidate they are. Motivation and specificity about their concern are<br />
most important to me. Some patients also have a functional component where they can’t breathe through<br />
their nose or their nose is crooked, so those are other good indications for having a rhinoplasty.<br />
Q: Do I have to be a certain age to have rhinoplasty surgery? A: I’m often asked about the proper age<br />
for a rhinoplasty. The youngest age that I think you should do rhinoplasty surgery is gender specific. For a<br />
young lady it would be around 13, a male would be more like 14 to 15. But it’s also a judgement you make<br />
based on looking at the development of the nose. As far as the upper end of age, probably the oldest<br />
rhinoplasty patient was a 79 year old, and it was purely for cosmetic reasons.<br />

Q: What activity restrictions are there after<br />
rhinoplasty surgery? A: One of the most<br />
important restrictions after rhinoplasty is that once<br />
your cast comes off, which is after a week, you<br />
need to be very careful about the sun. Sunburn<br />
would traumatize the skin and cause significant<br />
swelling. It is important for patients to wear<br />
sunscreen. We restrict aerobic activity for three<br />
weeks after surgery because elevating your blood<br />
pressure and heart rate is going to cause more<br />
nasal swelling.<br />
Q: What can I expect after rhinoplasty surgery?<br />
When can I go back to work?<br />
A: With a rhinoplasty you expect some bruising,<br />
surprisingly it’s not with the nose, it’s about the<br />
eyes, and there’s a cast on your nose. For the first<br />
week, most people don't go in public because of<br />
the cast and bruising. After a week, the cast comes<br />
off, the bruising is diminished by at least 50-75%<br />
and it’s more easily covered with makeup. Some<br />
people will go back to work when the cast is<br />
removed and just use camouflage makeup. I<br />
recommend that people take two weeks off<br />
because nasal swelling is down, you're breathing<br />
better and the bruising is almost gone.<br />
I believe that primary rhinoplasty takes a year for<br />
full settling to occur. There are little changes along<br />
the way that result as the skin shrink-wraps to the<br />
framework.<br />
The key is to get the skin envelope to cooperate with<br />
what you’ve done to the structure of the nose so it<br />
translates through the skin, and that’s what happens<br />
during that first year.<br />
The nose looks quite good prior to a year because<br />
most of the result is front-ended so that a month<br />
afterwards you’re looking at 75-80% of the result.<br />
The last 10-15% of the result occurs over the course<br />
of the year.<br />
Q: How do I know if I’m emotionally ready for<br />
nose surgery? A: The most significant change,<br />
structurally, that I can make in someone’s appearance<br />
is a rhinoplasty. It really is an important decision in<br />
someone’s life, and you have to be certain that you<br />
are prepared for this change. To see yourself in<br />
different views is so important. Computer imaging<br />
has enabled us to eliminate some patients for whom<br />
the change could be too drastic or inappropriate. The<br />
other thing that computer imaging does is it prepares<br />
you for the change. You’ve already seen what you<br />
may look like and provided that your end point is the<br />
same result that you derived on the computer image,<br />
you’re more mentally prepared for that. Anybody who<br />
is truly motivated is ultimately happy and gets through<br />
that period after surgery very nicely.<br />

What is it?<br />
Many women are all too familiar with the post-pregnancy belly<br />
bulge that is seemingly impossible to fix with diet and/or<br />
exercise. This “pooch” is actually a medical condition known<br />
as diastasis recti. “Diastasis” means separation while “recti”<br />
refers to the abdominal muscles. The abdominal area sticks<br />
out because the left and right bands of ab muscles have<br />
separated from each other and widened.<br />
This separation is the result of significant pressure on the<br />
abdomen from pregnancy which causes the muscles to lose<br />
their shape. Many times this muscle opening decreases after<br />
giving birth, but changes can take up to a year and often the<br />
muscles do not return to their original position.<br />

Who is affected by it?<br />
Approximately two-thirds of all women who experience pregnancy and childbirth suffer from this<br />

The newest trend in weight loss borrows directly from a practice that phased out in<br />
the early 20th century. The trend of an hourglass figure is back and so is the<br />
method to achieve the look: corsets, currently called “waist trainers.” This practice,<br />
now known as waist training, is advertised as a fitness hack to achieve a tighter<br />
core and shape the abdomen area, while accentuating the breasts and hips/<br />
buttocks.<br />
Khloe and Kim Kardashian, Kylie Jenner, and Amber Rose have all posted selfies<br />
wearing the device and many have jumped on the waist training movement. But is<br />
it worth all the fuss?<br />
Behind the premise of waist training is that when worn for a certain amount of<br />
time each day, the waist will be “trained” into a smaller shape. However,<br />
nothing has ever disappeared just by being squeezed, so the fat is simply<br />
moving elsewhere. And once the trainer is taken off, the body will settle back<br />
into place. At best, results are temporary.
Not only that, but due to the constrictive<br />
nature of the waist trainer, when worn<br />
during exercise the ability of the lungs to<br />
fully expand is limited, which does not<br />
allow the body to get the amount of<br />
oxygen it needs during activity. This can<br />
cause you to become lightheaded, which<br />
can lead to passing out.<br />
The root of the problem with waist training<br />
is that people are forgetting about how it<br />
affects the physiology of the internal<br />
organs. The green line represents the<br />
diaphragm, dark blue line: liver, light blue<br />
line: stomach, red line: colon, and yellow<br />
line: small intestines. As you can see,<br />
with the overuse of shape wear and waist<br />
trainers, the organs are forced to move,<br />
which can cause a myriad of other health<br />

Kardashian after her “Blood Facial.” While Kim’s<br />
after photo was a little extreme and clients can<br />
expect their post-treatment appearance to be much<br />
milder and without the visible pinpricks, her posttreatment<br />
pic brought national attention to a new<br />
non-surgical treatment used to enhance the overall<br />
appearance of the skin.<br />
We may not all be willing to show off our new<br />

What causes inverted nipples?<br />
An inverted nipple is caused by<br />
tissue in the breast that is attached<br />
tightly to the inside of the nipple<br />
skin. This pulls it inward toward the<br />
inside of the chest. The force of<br />
tight, shortened milk ducts can be<br />
greater than the muscle in the nipple<br />
that pulls it outwards, causing the<br />
inversion.<br />
Nipples can be inverted from birth or<br />
invert later in life. Nipple inversion is<br />
often times genetic and exists at<br />
birth or happens during puberty as<br />
the breasts develop. If it’s not<br />
genetic, inversion can happen due<br />
to constricted milk ducts, scarring of<br />
the milk ducts due to breastfeeding,<br />
or a lack of skin at the base of the<br />
nipple.<br />
It is important to note that if you<br />
have never had inverted nipples<br />
before and they change, this can be<br />
a cause for concern and should be<br />
mentioned to your doctor.<br />

What can I do to fix it?<br />
In order to permanently correct inverted<br />
nipples, a minor procedure is usually<br />
necessary. This is done in the office under<br />
local anesthesia and takes less than an hour.<br />
Two stitches are placed underneath the nipple<br />
to create a pad for it to sit on and remain<br />
outward.<br />
Is the procedure painful or require a long<br />
recovery? A local anesthetic is used during<br />
inverted nipple correction and there may be<br />
slight discomfort when that wears off.<br />
However, discomfort usually does not last<br />
longer than a week. Patients are able to return<br />
to work after the procedure but are asked to<br />
avoid exercise and strenuous activity that<br />
would cause rubbing against the nipple area<br />
for about a week.<br />
Does inverted nipple repair change the<br />
ability to breastfeed? Since the milk ducts<br />
are divided or lengthened during the inverted<br />
nipple procedure, breastfeeding will not be<br />
possible afterwards. This is something that<br />
you need to consider prior to the procedure<br />
and perhaps decide to have your nipples<br />
corrected after breastfeeding is complete.<br />

Recently, we have had an increase of calls from patients who have had surgery performed elsewhere and<br />
who are now dissatisfied with their results or require medical assistance with post-surgical complications.<br />
Some of these patients have gone to a doctor in another city, state or even a different country.<br />
We understand that choosing a surgeon is not always cut and dry. One of the main factors for patients is<br />
price. However, we all know that cheaper is usually not better. There are regional differences depending on<br />
where you live, but many times the price reflects the surgeon’s experience and expertise, along with the<br />
quality of their facility and staff.<br />
An issue that can arise for patients who travel a distance to go to another doctor is that oftentimes they do<br />
not have a comprehensive post-operative care schedule. If complications occur, one needs to be prepared<br />
to handle those issues. Depending on the severity of the complications, travel might not be an option which<br />
makes it difficult to get medical care from the doctor who originally performed the surgery.<br />

We often find patients in this scenario. In fact, an excess of 30% of our practice is revisionary<br />
surgery. While we pride ourselves on excellent patient care and are always happy to help, we<br />
would prefer not to see patients go down this path in the first place.<br />
We want to take this opportunity to inform our patients and educate them on issues they need<br />
to consider when choosing a surgeon. We encourage patients to choose board certified<br />
surgeons who specialize in cosmetic surgery. While it may cost you less upfront to go<br />
elsewhere, having to repeatedly fix something or endure multiple surgeries versus one only<br />
costs you more in the long-run. Patients who have been “botched” spend exponentially more<br />
than those who had an excellent result in the first place.<br />
While many surgeons look great on paper, credentials alone are not enough. It’s also critical<br />
that they are regularly performing the procedure you are interested in. When interviewing<br />
surgeons, ask them about your procedure of interest and how many they perform per year to<br />
get a better idea of the volume that they handle. Even though a surgeon is able to perform a<br />
rhinoplasty, if he is only performing a small number a year, it is in the best interest of the patient<br />
to seek a surgeon who is doing a higher volume. While quality is more important than quantity,<br />
a higher number of procedures show not only a surgeon’s experience, but the fact that many<br />
other patients have chosen him/her to perform their procedure. This is also why it’s beneficial<br />
to choose a surgeon who specializes in specific areas because you have peace of mind<br />
knowing that they focus solely on the body or the face.<br />
Find a surgeon who has satisfied patients, great online patient reviews, impressive before and<br />
after photos, and who can speak to your safety. A dedicated surgeon might even sometimes<br />
say that you aren’t a candidate for a procedure or will recommend something different than<br />
what you may have thought would work for you. Your safety and satisfaction as an individual<br />
patient is what’s most important to them.<br />
