My Tasks
To open a task either click this button in your
dashboard or open the tab ‘To Do’s’ and then click
on the specific task. A pop-up will open and here
you can start your session with TAVIE. Click on the
‘Start Session’ button.
The TAVIE Diabetes will open in a different tab
of your browser. Here you can work through the
different sessions of TAVIE Diabetes.
Add Measurements
To add a measurement you can click on the ‘New Value’-
button in your ‘Home’-screen and choose a certain
measurement like ‘Weight’. A pop-up will open with the
current weight measurements. Click on the + to add
a new value. After you add the value choose ‘done’ to
save it.
Communications
Click on the tab ‘Messages’ to see all messages from
your care team. You can also send a message to your
caregiver by clicking on ‘New Message’. Select type of
message you want to send, enter your message and
click on ‘Send’.
My profile
On the upper right you can click on your profile. Your profile shows
your demographics. It is also possible to change your password here.

When creating a new patient fill in the mandatory
fields and click on ‘Save’.
Note: If the patient wants a different language
please change language of patient.
Patient Portal Access and starting TAVIE
Overview Carepath and
Notifications
You can add a Carepath within a consult or
from the dashboard. Double click on care path,
now you can see status of tasks with different
notifications. Note: From the patient notifications
in the dashboard you can also see the latest status,
messages or reports from the patient.
Communications
To send a message to the patient or a care team member you can add ‘New Communication’.
Login
You can log on to CHMiX through a web application
with your username and password. You will receive
the website address from a member of your
integrated care organization.
+ Open the website in your browser and login
screen is shown.
+ Enter your user name and password (which you
received separately) and click Login.
If you do not remember your password you can use
the option ‘Forgot your password?’.
Patient Overview
After login you enter the ‘Your Login’-space. Here you can find an overview of your patients and your upcoming scheduled
appointments. To open a patient card you can click on the patient in your patient overview or click on the tab ‘PATIENT’. This
opens the last opened patient. You can also open the tab ‘My WORK’, were you can see all notifications and todays agenda.
1. To create patient portal access you click on
‘Demographics’, and then click on ‘Create Patient
Portal Access’.
2. To create access to TAVIE select the ‘Active’ button
near the field TAVIE Virtual Nurse and click on ‘Save’.
3. Click on ‘Yes’ after the message: ‘Do you want to enroll
this patient in TAVIE?’
4. After this the patient will be created and there will be a
message: ‘Patient has been successfully enrolled in
TAVIE.’
NOTE: You can also start with patient portal access and
create access to TAVIE virtual nurse after some time has
passed.
After starting TAVIE a patient overview is shown. This
is the dashboard of a patients file. Here you can see an
overview of: Vitals, Labs, Recommendations, Patient
Notifications, Risk Profile and Care paths.

1. Select type of communication
To add a new patient you can click on the button ‘New Patient’ in the bottom of your screen. This button is available in
tab ‘PATIENT’ or ‘MY WORK’.
Add Consult
Click on ‘Today’s visit’ to add a consult. Select type
consult, Intake or Follow Up, and fill out the consult
based on the different sections.
