Assessment Coordination (AANAC)<br />

Judi has been a certified AANAC RAC-CT Master Teacher since 2004 and currently serves<br />

as the vice president of curriculum development for AANAC where she oversees and<br />

coordinates the association’s certification and educational content. Judi’s career began as a<br />

nursing home administrator in 1987. She holds a master’s degree in nursing with a focus on informatics and a<br />

master’s degree in education. She has been a registered nurse since 2000. For the past 29 years, Judi has<br />

focused much of her work on supporting facilities in managing the RAI process, MDS coding, and nursing<br />

system quality improvement. She has served as an operations manager, case mix & quality assurance<br />

consultant, as well as crisis control administrator, for a variety of organizations including facilities in multiple<br />

states.<br />

Robert Latz, PT, DPT, CHCIO<br />

Chief Information Officer, Trinity Rehabilitation Services<br />

Robert Latz, PT, DPT, CHCIO is the only Physical Therapist in the United States to<br />

complete the requirements to receive the HealthCare CIO certification from CHIME<br />

(College of Health Information Management Executives). He is active within NASL as the<br />

Co-Chair of the IMPACT Act Implementation Workgroup and as a member of the IT<br />

Committee. Bob is also active within the APTA as a member of the Orthopedic; Home Health; and Geriatric<br />

Sections. Currently, Dr. Latz serves as the Chief Information Officer for Trinity Rehabilitation Services,<br />

which improves human function by providing contracted therapy services in Post-Acute Care settings. He is<br />

also the CIO for Passage Healthcare, which owns three campuses providing elder care services in SNF, AL,<br />

and IL settings.<br />

Dheeraj Mahajan, MD, CMD, CIC<br />

President and CEO, Chicago Internal Medicine Practice and Research<br />

(CIMPAR, SC); Clinical Assistant Professor of Medicine, University of Illinois at<br />

Chicago<br />

Dr. Dheeraj Mahajan is President and CEO of the Chicago Internal Medicine Practice<br />

and Research (CIMPAR, SC) and a Clinical Associate Professor of Medicine at<br />

University of Illinois at Chicago. He is a Fellow in Geriatric Medicine and certified in Internal Medicine<br />

and Geriatric Medicine. In 2014, he became a Fellow of the Institute of Medicine, Chicago. Additionally,<br />

Dr. Mahajan is a certified medical director and is certified in infection control and epidemiology. He<br />

specializes in cross-continuum provider alignment and is a nationally recognized speaker on post-acute care<br />

and quality improvement. Dr. Mahajan started as a solo practitioner in 2006 and today his group practice<br />

has grown to eight physicians, three APNs, one PA-C and 22 full time employees. He currently is the<br />

medical director at two CMS-rated five star skilled nursing facilities. At one of these facilities, he has<br />

implemented a transitional care unit (TCU) using evidenced based protocols, peer review best practices,<br />

strict admission order sets and protocols resulting in dramatically reducing hospital readmission rates,<br />

improved outcomes, and patient and family satisfaction.<br />
