Fraxel Repair Q & A<br />

A few months ago, we introduced Utah to the Fraxel Repair laser, a revolutionary new treatment for wrinkles<br />

and scars. <strong>The</strong> excitement surrounding this non-invasive procedure has generated many questions. Let’s take a<br />

moment to answer them.<br />

What is different about the new Fraxel Repair laser?<br />

<strong>The</strong> new Fraxel Repair laser is the only laser with a roller tip to lay down a random array of laser points in the skin.<br />

This new technology gives amazing results with difficult acne scars, “crepey” skin on the eyelids, “chicken skin” on<br />

the neck, and wrinkles on the face, neck and chest. Healing is also faster and safer over similiar treatment with<br />

a CO2 laser peel. <strong>The</strong> Fraxel Repair laser can even be used to treat wrinkled and scarred skin on non-facial areas<br />

of the body. This is a major advancement in laser treatment of the skin. It’s a patented technology and no other<br />

Is the Fraxel Repair an FDA-approved treatment?<br />

<strong>The</strong> Fraxel Repair laser system has received the industry’s only FDA 510(k) clearance for a fractional ablative laser<br />

system for the treatment of wrinkles, furrows, fine lines, textural irregularities, pigmented areas and vascular<br />

discoloration.<br />

Can it be combined with other treatments?<br />

Yes. Fraxel Repair laser treatments can be combined with light facials, chemical peels, filler, skin tightening, and<br />

How many treatments will I need and how quickly will I see results?<br />

For most people, maximum results are realized in just one treatment. <strong>The</strong> Fraxel Repair treatment was<br />

severe sun damage, we may treat the skin more aggressively, which can mean up to 2 weeks of social<br />

downtime. A more gentle treatment with less downtime is also available to those with mild sun<br />

and skin damage. No matter your situation, Fraxel Repair treatment can create remarkable<br />

results without the prolonged recovery time and complications of other procedures.<br />

What side effects can I expect<br />

after treatment?<br />

Immediately following treatment, you will have some<br />

degree of redness and swelling. Other temporary side<br />

effects may include minor itching, dry skin, peeling,<br />

flaking, and a bronzed skin appearance. We provide each<br />

patient with specific instructions about what to expect<br />

for their particular treatment and how to minimize side<br />

effects.<br />

What areas can I treat?<br />

Face, neck, eyelids, chest and most other body areas for<br />

improvement of texture, wrinkles and scars. This laser<br />

can safely treat non-facial areas, a truly new advance<br />

in the improvement of non-facial wrinkles, scars, and<br />

texture. Treatment of stretchmarks is also very good<br />

with this laser.<br />

How painful is the treatment?<br />

Mild treatments can be performed with topical<br />

anesthesia only. For more<br />

aggressive treatments, oral<br />

sedation and tumescent local<br />

anesthesia is used to make<br />

the skin completely numb.<br />

Why I Love Light Facial Treatments<br />

Elisa Wilcox, PA-C, has been working closely with Dr. Taylor since May of 2003. Elisa<br />

Dark Circles Under the Eyes<br />

Most people<br />

think that<br />

dark under-eye<br />

circles are caused<br />

by staying up late<br />

watching monster<br />

movies, having<br />

that last drink<br />

the night before,<br />

or sitting up with<br />

your laptop trying to finish your quarterly<br />

report. Most people believe that their<br />

behavior is somehow linked to dark undereye<br />

circles.<br />

<strong>The</strong> truth is, most people are dead wrong.<br />

If nothing else, I want you to know that<br />

dark under-eye circles are not your fault.<br />

<strong>The</strong>y don’t mysteriously appear when<br />

you misbehave or are stressed out, only<br />

to vanish when you get 8 straight hours<br />

of sleep. Dark under-eye circles are a byproduct<br />

of aging. Changing your behavior<br />

will, in almost every case, NOT get rid of<br />

your dark under-eye circles.<br />

Tear troughs are semi-circular creases or<br />

depressions which start at the nose and run<br />

along the bottom of the lower eyelid just<br />

above the cheek bone. <strong>The</strong>se depressions<br />

can produce the illusion of darkening<br />

around the eyes, resulting in “dark circles”<br />

or “tired eyes”.<br />

As we age, the eyelids and cheek begin to<br />

sag. <strong>The</strong> tear troughs, however, cannot sag<br />

with the rest of the facial skin because of<br />

ligaments that attach to the bone of the<br />

eye socket. This results in the deepening<br />

depressions which make the shadow and<br />

dark circle more noticeable.<br />

If you have deep tear troughs, an injectable<br />

dermal filler, such as Restylane, can be used<br />

to correct them.<br />

<strong>The</strong> filler is injected with a very fine micro<br />

needle under the skin’s surface along the<br />

line of the hollow. An anesthetic cream<br />

may be applied to the area for 30 minutes<br />

prior to the treatment (although often no<br />

anesthetic cream is required).<br />

Results with dermal fillers are instantaneous.<br />

After the procedure, the treated<br />

area may be slightly red and swollen;<br />

however, you can resume normal activities,<br />

drive yourself home or return to work<br />

immediately after the treatment.<br />

<strong>The</strong> results typically last 6-9 months,<br />

although the exact length of time will<br />

depend on the structure of your skin and<br />

other factors such as lifestyle, age and<br />

the degree of perfection you demand.<br />

Additional touch-up treatments can be<br />

performed at any time after the initial<br />

treatment.

Does life get any better?


Wrinkle Treatments |<br />

Botox—Safe & Effective<br />

Botox was approved by<br />

the FDA-in 2002 for the<br />

treatment of facial wrinkles<br />

(specifically the “frown lines”<br />

between the eyebrows) and<br />

is now also approved for the<br />

treatment of underarm sweating<br />

and some severe neurological<br />

disorders. Medications such<br />

as Botox are tested rigorously<br />

for years to document safety<br />

and efficacy before they can<br />

be FDA approved. Millions of<br />

relaxing the facial<br />

muscles that cause<br />

must only be injected<br />

by an experienced<br />

facial anatomy so that it is not<br />

used in the correct amount to<br />

simply relax the muscle and<br />

Understanding Skin Cancer |<br />

Warning Signs and Prevention<br />

Few words conjure up more images of concern and<br />

desperation than that of cancer—and rightly so. But<br />

did you realize that of the many forms of human cancer,<br />

skin cancer is the most common? <strong>The</strong> National Cancer<br />

<strong>In</strong>stitute notes that skin cancers will affect nearly 1 million<br />

Americans this year—and the number of new cases is<br />

increasing. Equally concerning is the fact that nearly half<br />

of all Americans who live to the age of 65 will develop skin<br />

cancer at least once.<br />

Despite the gloomy numbers, there is hope. Unlike cancers<br />

that grow internally, skin cancer has the greatest potential<br />

for early detection and treatment—if we are vigilant to its<br />

warning signs.<br />

Types of Skin Cancer<br />

To understand what to look for, one should first understand<br />

the basic skin cancer types. <strong>The</strong>y fall into three groups:<br />

precancers, nonmelanomas, and malignant melanoma.<br />

Precancers<br />

When dermatologists diagnose precancerous lesions, they<br />

are most likely referring to actinic keratosis (AK), also known<br />

as solar keratosis. While AK is not officially cancerous, it<br />

has the potential to develop into more serious forms of<br />

skin cancer. AK is usually found in the outer layer of the<br />

skin, most commonly on the face, lips, ears, neck and<br />

scalp. It may cause the skin to become scaly, rough, red or<br />

discolored, and may be tender to the touch.<br />

Treatment options vary and are best discussed with one of<br />

our board-certified dermatologists. Common treatments<br />

for AK include freezing with liquid nitrogen, chemical<br />

peels, topical chemotherapy creams and laser resurfacing<br />

therapy.<br />

Nonmelanomas<br />

<strong>The</strong> most common form of skin cancer, and thankfully<br />

the least dangerous, is basal cell carcinoma. It accounts<br />

for more than 90% of all skin cancer in the U.S. Basal<br />

cell carcinoma grows slowly and rarely spreads beyond<br />

its original location. While this cancer is seldom lifethreatening,<br />

if left untreated, it can grow into underlying<br />

tissue and cause serious damage.<br />

Warning signs for basal cell carcinoma may include an<br />

open sore that doesn’t heal, a growth with an elevated<br />

border and central indentation, or a scar-like bump on the<br />

sun-exposed skin of the head, neck, chest, arms or legs.<br />

<strong>The</strong> most common treatment for this type of cancer is<br />

surgical excision.<br />

Up a notch on the danger pole, above basal cell carcinoma,<br />

is the next most common kind of skin cancer: squamous<br />

cell carcinoma. Like basal cell carcinoma, this cancer is<br />

generally not considered a threat to life. However, unlike<br />

its cousin above, squamous cell carcinoma, if not treated,<br />

may potentially spread to other body organs (including<br />

lymph nodes) and ultimately cause death.<br />

This type of cancer appears frequently as a red, scaling,<br />

thickened patch on the arms, legs, lips, face or ears.<br />

Ulceration and bleeding may also occur.<br />

Treatments include Mohs micrographically controlled<br />

surgery, surgical excision, lasers, or radiation therapy.<br />

Malignant Melanoma<br />

<strong>The</strong> most dangerous form of skin cancer is malignant<br />

melanoma. Thankfully it is also the least common, though<br />

incidences are rapidly increasing. If detected early,<br />

malignant melanoma can be completely cured. If it is not<br />

treated quickly, however, it will spread throughout the<br />

body’s organs and ultimately be fatal.<br />

While anyone can get melanoma, fair-skinned people are<br />

at greater risk. It can appear anywhere on the skin, but<br />

is most common on the backs of men and on the legs of<br />

women. Melanoma almost always begins on the skin’s<br />

surface, where it’s easily treated (generally via surgical<br />

removal). If given time to grow, it will move into the blood<br />

and lymphatic vessels, making full recovery difficult.<br />

Melanoma’s warning signs include spots or moles that<br />

change in size, color or feel (often with an irregular<br />

border). <strong>The</strong> cancer may also appear as a small lesion<br />

containing red, white, blue or black spots; or shiny, firm,<br />

dome-shaped bumps (located anywhere on your body).

always best to see your doctor. If you need a<br />

Prevention<br />

Several factors (many beyond our control)<br />

may increase the risk of getting skin cancer.<br />

<strong>The</strong>se include genetic predisposition,<br />

the altitude at which we live, and age.<br />

That said, our risk may substantially<br />

decrease by doing the following:<br />

Reduce exposure to ultraviolet<br />

(UV) radiation, (especially<br />

important for children)<br />

Avoid sunburns and sun exposure during<br />

the day (usually from 10 a.m. to 3 p.m.),<br />

Wear protective clothing (long sleeves and<br />

hats) when outdoors<br />

Use a broad-spectrum sunscreen that blocks<br />

both UVA and UVB radiation<br />

Reapply sun block every 2 hours and after<br />

swimming<br />

Check your skin regularly and report any<br />

changes to your doctor<br />

A Note from Your Doctor<br />

Patients often ask for advice regarding the timing of when it’s best to have a cosmetic<br />

procedure. Recently I performed a lower eyelid blepharoplasty on a 16 year old boy<br />

because he was born with puffy lower eyelids and his friends were teasing him. <strong>The</strong><br />

procedure turned out great and the teasing stopped. For him, the timing was perfect.<br />

Children with birthmarks are often made fun of when they start kindergarten. Treating<br />

their birthmarks early may help them avoid growing up with a damaged self-image.<br />

Teenagers who suffer from problem acne may also be vulnerable. Early and aggressive<br />

treatment will not only help immediately with the acne, but it may also help them to<br />

avoid severe acne scarring that could bother them well into adulthood. Once again,<br />

timing is critical.<br />

Certainly not all cosmetic procedures are as time sensitive as in the examples above.<br />

Treatments that reduce wrinkles or other signs of aging are usually sought after when<br />

a person becomes self-conscious about a problem. It may be early in life or later.<br />

Many patients, for example, come in to the <strong>Gateway</strong> Aesthetic <strong>In</strong>stitute to get their<br />

inherited baggy eyelids fixed when they are just 35. Others may wait until they<br />

are nearly twice as old. <strong>The</strong> point is, no matter your age, if something about your<br />

condition or abnormality), then it may be time to seek treatment.<br />

I remember having a large mole removed from my right cheek when I was just twelve<br />
