In the Media |Scar Revision with Inna ProkopenkoThey have done morefor me than I can everpossibly explain.During the past year, GatewayAesthetic Institute providershave been featured as keynotespeakers at cosmetic laser andmedical conferences worldwide. From Paris, France to Sydney,Australia, our experts have beeneducating industry and mediaprofessionals about new andextraordinary procedures. One procedure of particularinterest garnered local airtime onGood Things Utah as it highlightedGateway Aesthetic client MaryAnnGerber, a melanoma cancersurvivor. (Though melanoma, amalignant form of skin cancer, represents only 3% of all skincancers, it has the highest deathrate and is the most likely tometastasize or spread.)You can imagine MaryAnn’s reliefwhen her cancer was detected andtreated early. In our discussions with MaryAnn,however, she said she wasn’tprepared for theemotional aftermaththat would followthe destructivecancer. Though shesuccessfully battled the disease,MaryAnn was left with damagingscars on her face and neck.Excerpts from her poignantinterview follow.Tell us about your experiencewith skin cancer.“I was diagnosed with melanomaskin cancer at the age of 24. Ihad a tiny mole on my left cheekthat was about the size of a pencileraser. The melanoma was in astage IV so not only did doctorshave to remove the melanoma, butthey also had to remove all thelymph nodes out of the left sideof my neck.”Once the surgery was over andyou were left with the scars, what were your thoughts?“I was ashamed and embarrassed.I didn’t want to go outside. I didn’twant anyone to see me. I wouldtry to hide my face with my hair. Ihated the way I looked. I hated theway I felt about my looks. It wasthe most uncomfortable feelingI’ve ever experienced.”

How did you find out about our treatments thathelp to reduce the appearance of scars?“I had gone to many doctors seeking any kind oftreatment that would help. All the doctors said theywould need to do another surgery to minimize thescars. However, I didn’t understand how doing anothersurgery would make a scar better. I was so depressedand disappointed. I thought I would forever look asawful as I felt.“Then one of my best friends, who is a patient atGateway Aesthetic Institute, introduced me to InnaProkopenko, R.N. She told me that Inna was amazingand that she could help. It was the first time in over ayear I had a glimmer of hope again.”Talk about your results. Were they gradual?“It was amazing. Within a few days of each treatment, the bruises would fade and the scars would becomeless visible. The redness was also fading as the scartissue was starting to smooth. I was so excited forevery treatment because I couldn’t wait to see whatkind of changes would happen.”When you started to see results, how did youfeel?“Within about six months I noticed that I wasbecoming less sensitive about my looks. I was slowlygaining confidence again. The first day I felt ‘normal’was when Inna took the post picture of my treatmentsand put it next to the pre picture. I started to cry. Iwas a normal person again. It was the best feeling.”It’s been about a year now. Is there anythingelse you’ve noticed?“My friends and family always tell me how beautiful Iam. For the first time in two years, I really do believethem.“I finally feel like myself again. I’m confident in mylooks and I don’t try to cover my face anymore. I wearmy scars now with pride instead of shame. It was abattle that I won and now I am a survivor instead ofa victim.“I love GatewayAesthetics. Ilove Inna. Theyhave donemore for methan I can everpossibly explain.I can’t imaginewhere I would betoday if I hadn’tfound them andthey hadn’t hadthe technology thathas helped me in somany ways. Laser Resurfacing |20 Years Younger, a Single Procedure“A prospective client’sbiggest concernshould be choosinga physician withexperience andexpertise...“Laser skin resurfacing is one of the most aggressive laser proceduresavailable today to treat the signs of aging, including wrinkles, sundamage, uneven pigmentation, and scars. Also known as a laser peel, theprocedure uses laser energy to brush away the outer layers of skin to revealhealthier, smoother, and younger looking skin beneath.Considered by many to be the “gold star” treatment in fighting the effectsof aging, laser skin resurfacing can leave your skin looking naturally vibrantand youthful, without the risks associated with a face lift.Laser Resurfacing vs. Other TreatmentsThe question is often asked, why would I select laser skin resurfacing over achemical peel, microdermabrasion, or other laser wrinkle treatment?Laser skin resurfacing is more aggressive. Laser resurfacing literally peels awayor removes the top layers of skin in a process that smooths deep wrinkles, evens severe pigment, and lightens facial scarring and other skin imperfectionsthat can’t be treated effectively with other cosmetic procedures.Typically, laser resurfacing is best suited for men and women who have deeperwrinkles or more complex skin pigment concerns, or for those who sufferfrom severe acne scarring. Because of the variety and complexity of skinconditions, Dr. Taylor uses a combination of several lasers in his treatments, including the Ultrapulse, CO2 Erbium, and Derma K.Downtime and HealingWhile the results can be dramatic, healing time after laser resurfacing issignificantly greater than for less aggressive options, so you must planaccordingly. Expect at least two weeks of down time, during which a healingointment is applied to protect the newly exposed skin. The area heals muchlike a sunburn, and proper care during recovery is important to avoid risksof scarring or unnatural healing. In the weeks following treatment, it is alsocritical to protect the newly-exposed skin from sun exposure until rednesssubsides and normal color returns.Associated RisksLaser resurfacing yields remarkable results, but like all types of surgery, itcarries risk. A prospective client’s biggest concern should be choosing a8 gatewayaesthetic Institute & Laser Center

physician with experienceand expertise, someone with theability to make critical decisionsregarding treatment depth, accuracy, and the best combination of laserssuited for their individual need. If thetreatment is performed improperly byan unqualified provider, serious sideeffects may result, including severescarring, unnatural texture, unevenskin tone, and more. Our ExperienceAt the Gateway Aesthetic Institute, the procedure is performed under localanesthesia in the comfortable setting ofour in-office surgical suites. As a boardcertifieddermatologist with over 30 yearsof experience, Dr. Taylor has performedcountless laser resurfacing treatments andis considered an expert in the field. He hastrained physicians in the procedure worldwide. In fact, many of the clients Dr. Taylortreats are referred to him by leading physiciansaround the country.Right for You?Still wondering if it is worth it? That’s a personaldecision that should be made only after carefulconsideration of all the facts. However, manyof our clients who have had the procedure areamazed with the results and feel like we’ve turnedback the hands of time.If you are interested in speaking with Dr. Taylorabout whether laser resurfacing is right for you, callour office at 595-1600 and schedule a consultationtoday. Not Just for Women |Men Say Yes to Cosmetic ProceduresAccording to The AmericanSociety for Aesthetic PlasticSurgery (ASAPS), the numberof men receiving nonsurgicalcosmetic procedures hasincreased by 722% since 1997.Opinions differ as to thereasons men are becoming morecomfortable with cosmeticprocedures. Some attribute thetrend to our youth-orientedculture, claiming men areincreasingly more comfortableadmitting they care about theirlooks. Others have settled onthe theory that as divorce ratesclimb, older men feel the needto gain a competitive edge overtheir younger counterparts. Stillothers suggest that becausetheir significant female othersare drinking from the cosmeticfountain of youth and seeingdramatic results, men are finallywanting a piece of the action. No matter how it’s spun, thenumbers don’t lie. Currently13% of all nonsurgicalcosmetic procedurepatients are male, and the numberis rising. The most commonprocedures formen include Botox, laser hair removal, andfacial tightening. Surgicalprocedures gaining popularityinclude liposuction, eyelidsurgery, and rhinonoplasty(nose reshaping).Perhaps a recent maleclient at the GatewayInstitute summed upthe trend best whenhe said, “I considerit an investment. Ilike looking good.”With a grin headded, “Besides, Ihate getting old.” ©Fall/winter 2007-08 11

Tipping the Scales |Win the Fight Against FatFrom a child choosing candy to a family shoppingfor a car, everyone loves a choice. When selecting abody contouring procedure, we are no different.MedSculpt, LipoDissolve, Tumescent Liposuction, and now Smartlipo—allare different names forprocedures aimed atgiving smoothercontour to yourabs, hips, andbuttocks.Choice, however, canalso breed confusion. How do you know whichprocedure is best? Tounderstand the advantagesof each, let’s get a quick lesson.MedSculpt treatments target problem areas of localizedfat with a computerized, state-of-the-art machinethat uses ultrasonic waves to heat and soften fat cells, disrupt cellulite bands, and tighten and tone the skin.Optimal treatments are administered twice a weekfor ten weeks. Though you will not notice inches orpounds lost, the treatments can smooth lumpy, fattyareas or help enhance results with LipoDissolve andliposuction treatments. The pros:• It’s painless.• It shapes and tones the skin.• When used in conjunction with other bodycontouring treatments, it improves results.• It is the least expensive of the body contouringprocedures. The cons:• It doesn’t remove inches, but instead smoothsexisting areas. (It’s likely that those lookingfor significant results will not be satisfied withMedSculpt alone.)LipoDissolve is a non-surgical fat removal treatmentfor body contouring and an alternative to liposuctionsurgery. It features a series of micro injections ofPCDC, a naturally-occurring substance that breaksup and dissolves localized fat deposits, which arethen eliminated naturally through the body’s immunesystem.LipoDissolve is meant to reduce areas of localizedfat—love handles, chin, buttocks, thighs, arms andchin. This means you will notice a loss of inches in the

treated areas, but not necessarily pounds.Ideal candidates are at or near their idealweight, but have pockets of fat that donot seem to go away, even with properdiet and exercise. For optimum results, an average of 6treatments are needed in each area, withtreatments spaced 3 to 6 weeks apart. It is alsoimportant to note that LipoDissolve removes local fatcells entirely. Therefore, should you gain weight, it willshow in areas other than those treated. The pros:• The treatment is noninvasive, meaning it’s not asurgery. Often, those who opt for LipoDissolve overliposuction do so because they want to avoid whatis perceived as a more complex surgery.• It is less expensive than tumescent liposuction andSmartlipo treatments. The cons:• Treatments take time since they must be spaced overseveral weeks.• As the fat dissolves, some discomfort is generallyexperienced.• Fewer areas may be treated at one time than withother methods.Tumescent Liposuction represents the most significantsingle advancement in liposuction in years. The word“tumescent” means swollen and firm. By injecting alarge volume of dilute lidocaine (local anesthetic) andepinephrine (capillary constrictor) into the pockets offat, the tissue becomes swollen and firm (tumescent),making it easier for the liposuctioncannula to travel smoothly beneaththe skin to remove the fat.Today’s procedure carries significantly less riskwith very little down time (usually a few days). It is performed without hospitalization (in thecomfort of our in-office surgical suites), and withlocal anesthesia. In fact, the procedure is so safe it hasbecome one of the most commonly-performed cosmetictreatments in the United States today.Clients see immediate improvement after surgery, with optimum results (significant inches lost) inapproximately 4 weeks. Ideal candidates are within 20pounds of their ideal weight, but with areas of fat thatare non-responsive to diet and exercise. The pros:• Only one treatment per area is needed, rather than themultiple treatments of LipoDissolve.• Immediate, dramatic, andpermanent results arenoticed in the treatedareas.• The tumescent methodis an affordable andsafer alternative toregular liposuction.• A greater numberof areas may betreated at onetime than withLipoDissolve.(cont. page 18.)

Seeing the Light |Light and Laser Facial TreatmentsLight and laser facials offer effective treatments to fade and heal sun-damaged skin, reduce facial pigment and redness, tighten lax or wrinkled skin, and more. In particular, light and laser facials help utilize and maintain the skin’s tone andtexture to keep it looking fresh all year long.Unlike other facilities who only have one or two laser treatments, the Gateway Aesthetic Institute has numerous lasersavailable to our team of highly-skilled providers. This means our board-certified dermatologists can personalize the perfecttreatment to match each client’s individual skin type and concern. If multiple conditions need treatment—conditions thatmay take multiple laser treatments at other facilities—we offer specialty lasers that can treat them simultaneously. This not only saves you time, but it also saves you money. Call our office today at 801-595-1600 to learn more or to scheduleyour customized light and laser facial. ©Fight Against Fat, cont. |The cons:• Despite being extremely safe, the treatment is stillconsidered a surgery. As such, it remains “scary” to somepeople.Smartlipo is a new, innovative, laser-assisted procedurethat can be used alone or in combination with tumescentliposuction. Like tumescent liposuction, Smartlipo removesfat through a cannula inserted into small openings in theskin. However, with Smartlipo, a laser is used to melt thefat, helping with extraction and resulting in less swelling andbruising for faster recovery. In addition to melting pocketsof fat, the laser energy also induces collagen growth whiletightening lax skin.Smartlipo is a minimally invasive treatment that onlyrequires local anesthesia. As such, client safety is enhancedand side effects are less likely. Typically only one shorttreatment session is needed, enabling you to get back to anormal schedule quickly. The pros:• Smartlipo offers the same advantages as tumescentliposuction, but with superior results and quickerrecovery.• In addition to removing fat, the laser tightens and tonesthe skin. The cons:• The added cost of the laser makes the procedure moreexpensive than tumescent liposuction alone. Now that you are armed with information about eachbody contouring procedure, we recommend you schedulea consultation with one of our providers to discuss whichoptions might be best for you.For additional information and to see before and afterphotos of each procedure, visit our website at www.GatewayLaserCenter.com. ©18 gatewayaesthetic Institute & Laser Center

Fight Skin Aging with a Secret Weapon |Topical AntioxidantsEach and every day, the human body’s largest organ, the skin, is assailed by pollution, stress, poor diet, and most importantly, ultraviolet radiation from thesun. Free radical damage generated by our own body’smetabolism compounds the problem, as these freeradicals trigger the release of inflammatory mediatorsthat break down collagen and damage the cell barrier. It gets worse. In addition to causing external damage, free radicals are one of the main factors contributing tocancerous cells. Is it time to wave the white flag in surrender? Hardly. To combat negative environmental and biological skindamage, we can fight back with a powerful weapon—antioxidants. While health professionals have touted the benefitsof antioxidant-rich foods for eons, it’s sometimesdifficult to direct them to specific areas of the body. Forexample, even if a person consumes an optimal amountof antioxidants through diet or supplements, only aminute amount is allocated to the skin. The answer to this problem may be topical antioxidants. It is possible to boost the skin’s external defenses againstharmful free radicals and control environmental agingby topically applying antioxidants. Numerous studieshave proven that when applied to the skin’s surface, antioxidants offer protection from sun damage andstimulate collagen production. One of the most well-known and frequently-usedantioxidants is vitamin C, a multi-tasker for the skin.It prevents water loss, helping to maintain the skin’s20 gatewayaesthetic Institute & Laser Center

arrier function, while building collagen and elastin. Inaddition, topically applied L-Ascorbic acid (vitamin C)deactivates unstable free radicals, preventing much ofthe damage they cause. When applied topically, vitaminE is another powerhouse. Its antioxidant activity is evenmore effective when combined with vitamin C andcoenzyme Q10. Additional products known for theirantioxidant activity are green tea, cacao, soy isoflavones, lipoic acid, pomegranate, mandelic acid, and grape seedextract, to name a few.Currently, companies are focusing on the deliverymechanism of antioxidants for deeper skin penetration.Developments from AGI Labs and its Remergentskincare line are of special interest. AGI utilizes severaldifferent liposome technologies to deliver its patentedmarine enzymes into the skin. Initial studies on theseproducts are extremely promising, especially withregards to their ability to stimulate the skin to repairitself. As science continues to research and develop antioxidanttechnologies, consumers are finding new skincareproducts that increase beauty—softer skin with fewerwrinkles—as well as valuable, life-saving protectionagainst environmental damage.Which topical antioxidant is best for you?
